Description
ITO Pricing Sheet Table 1 for SOW estimates SOW The SOW defines the effort required for the provision of the services of fitting, dispensing, adjusting, and repairing (including the provision of parts and services) of PSE to the Fleet Readiness Center Southeast (FRCSE) workforce. The intent of the program is to protect worker vision in a variety of sight-hazardous areas of operations at FRCSE and remote locations. The SOW will provide for PSE procurements and upkeep only for FRCSE personnel located in the greater Jacksonville/NE Florida area. (FRCSE employees at remote locations in other states have their PSE needs addressed by another local purchase program. APPLICABLE DOCUMENTS Specifications American National Standards Institute (ANSI)/International Safety Equipment Association (ISEA) Z87.1-2015 (or latest edition), "Occupational and Educational Personal Eye and Face Protection Devices" or other appropriate National Consensus Standard. Regulatory requirements 29 CFR 1910.133, ''Eye and Face Protection" Other governing documents. OPNAVINST 5100.23 series, Navy Occupational Safety and Health Program Manual, Chapter 19, "Sight Conservation" Availability of Documents 1. ANSI/ISEA: available directly from ANSI 2. OSHA: available from the Government Printing Office (GPO) 3. OPNAVINST 5100.23G Chapter 19 REQUIREMENTS The vendor sha1l fulfill or otherwise conform to all of the requirements of the SOW. Furnish a wide range of selection (at least 12 different styles) of ANSI/ISEA Z87.1-2015, Z87-2+ compliant frames to appropriately fit employees. Provide all necessary measurement and fitting services on site at FRCSE via a licensed optician or other fully qualified person according to the laws, rules and regulations of the State of Florida. Fittings shall be conducted at a designated location within FRCSE a minimum of once every two weeks. The vendor shall not provide safety eyewear directly to FRCSE employees unless the FRCSE employee has obtained prior approval from the FRCSE Safety Eyewear Coordinator. All eyewear shall be furnished with permanently attached sideshields. When an employee requires lenses for a respirator, the vendor shall take the appropriate measurements to ensure the lenses fit the inserts provided by FRCSE. All necessary adjustments to ensure employee satisfaction with PSE provided shall be performed at the time the eyewear is dispensed, or at a time agreed upon with the FRCSE employee. Provide comprehensive repair capability at the vendor's facility including a full line of replacement parts either kept on hand or obtainable from the eyewear manufacturer(s) within a reasonable period of time agreed upon between the vendor at technical points of contact (TPOC) listed in Section 6. Repair items include, but are not limited to, sideshields, screws, and nose pads for all styles of eyewear currently dispensed as well as PSE previously in service at FRCSE. Provide comprehensive repair services, including parts, on-site at FRCSE a minimum of once every two weeks. Repair sessions shall be available during the scheduled on-site visit times, which may be conducted concurrently with fittings. If repairs do not fall under the warranty as described in Section 3.10, the dispensing fee may be assessed, including costs for repair parts. Pricing of complete sets of PSE to the FRCSE shall be based on polycarbonate lenses rated for "high impact" for best durability in the industrial environment and shall be structured according to Table 1. A frame warranty for a period of two (2) years shall be included with the price of each pair of frames. The warranty shall cover frame replacement in the event they are no longer serviceable from two years after the frames were first delivered to FRCSE. Prior to processing all orders, the vendor shall submit a list in the format below to the technical points of contact (TPOC), listed in Section 6, for review and approval. Only after approval is granted by the primary TPOC shall the vendor process the order. If an order is processed without the prior written approval from the primary TPOC, the vendor risks not being paid for any completed work.
