- These are the provisions and clauses for this solicitation. This is the Statement of Work (SOW). This is the RFQ document. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this requirement is 2017-09962 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control & Prevention (CDC) intends to award a fixed-price purchase order for the following requirement. This requirement is for twenty (20) male, Rhesus Macaques of Indian origin. All work shall be performed according to the attached Statement of Work (SOW). Acceptance and FOB point. The quote format is at the discretion of the Offeror. It is the Offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website https://www.acquisition.gov/. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-87-2 (6 April 2016). This acquisition is solicited under North American Industry Classification System (NAICS) code 112930 and small business size standard of $75M. A fixed-price purchase order will be issued in writing to the successful Offeror. To be eligible to receive an award resulting from this solicitation, the Contractor must be registered in the System for Award Management (SAM). Offerors may register in SAM at http://www.sam.gov. For questions regarding registration, contact Federal Service Desk at (866) 606-8220. FAR 52.212-3, Offeror Representations and Certifications with Offer is included with the Provisions and Clauses. No telephonic quotes will be processed. All electronic responses must be received by the Government no later than 2:00pm EDT on Tuesday, 22 August 2017. An electronic quote shall be emailed to the Contracting Officer at the email address below, and a copy of the quote on compact disc (CD) should be sent to the Contracting Officer at the following address. The CD shall be received by the Government no later than two (2) business days after Tuesday, 22 August 2017. All questions must be received no later than 11:00am EDT on Thursday, 10 August 2017.
