STATEMENT OF WORK ARCHITECTURAL AND ENGINEERING SERVICES PREPARATION OF CONSTRUCTION BID DOCUMENTS For Project MUHJ 17-4039 Repair Facility Model, Conference Room 633 CES, Fac. 328 JBLE - Langley, VA 21 April 2017 STATEMENT OF WORK PART I - STATEMENT OF WORK (SOW) 1.1 GENERAL: This Statement of Work supplements Block 6 of Standard Form 252, Architect- Engineer Fixed-Price Contract and sets forth the general requirements for the performance of the various services required under the contract. The Architect-Engineer shall perform all or part of the following services as specifically identified in the individual work orders (DD Form 1155) and as more specifically described herein. 1.1.1. GENERAL SCOPE OF WORK: The project entails performing miscellaneous repairs, described below, to the base model in Facility 328 Conference Room, JBLE - Langley, VA. The A-E design shall include, but shall not be limited to, the major items of work identified in this SOW. Design services shall include all travel, labor, investigation, materials and other work as required to produce complete project documents as described in the SOW items below. 1.1.2. MUHJ 17-4039 Repair Facility Model, Conference Room, 633 CES, Fac. 328: The intent of this project is to perform miscellaneous repairs to the existing 3-dimensional base model as specified in the SOW items listed below. The scope of work for this project includes but not limited to the required repairs and updates. 1.1.3. Model Adjustment/ alignment: Adjust the model to display a more accurate alignment of the facilities and roads on both two halfs of the model. 1.1.4. Repaint: Repaint the frame. Repaint facilities, roads, parking areas and green spaces to have a consistent hue and color through the model. 1.1.5. Color Patterns: Provide new existing structures to match existing color pattern (tan). Future structures shall be identified with blue roof surfaces. Facilities to be demolished shall be identified with a red roof surface. 1.1.6. Remove Existing facility: The following facilities shall be removed from the model; Facilities 141, 142, 143, 81, the barn behind Fac. 252, two (2) NASA Tunnels, portion of Fac. 617 Old Vet nary Clinic, Arts & Craft, and the New LFCU. Remove parking that was demolished and provide new parking that have been installed. 1.1.7. Remove Blue Color: Remove the blue color from the roof surface of Facility 44 and 361. 1.1.8. Add Red Color: Provide red color to the roof surfaces of facilities 147, 148, 223, 265, 266, 267, and the existing Fuel pier. 1.1.9. Addition to Existing Facilities: Provide addition to facility 92 (Dental Clinic) and Hospital. 1.1.10. Add New Future facilities: Provide new structures that replicate the Fuel Pier and Government Vehicle Fuel Station. The design layout will be provided for these facilities. 1.1.11. Update Facility: Rotate Facility 61 to depict its current position and parking areas. 2.1 A-E PROPOSAL and REPAIR DOCUMENTS/MEETINGS/REPORTS/STANDARDS REQUIREMENTS: A Proposal shall be submitted with a repair cost summary for each of the items described in the individual work orders. The cost estimates, and repair schedule, shall be developed as separate contract documents. 2.1.1 Model Repair Submittal (35%): With each design submittal subsequent to the initial (35%) submittal and for all Government comments provided to the A-E, the A-E shall provide a record that includes a written response which explains how each respective repair comment was re- solved. This record of comment resolution shall be submitted with each set of design submittal documents. 2.1.2 35% Model Repair Submittal: The A-E shall allow the government to view the 35% completed model repairs and other supporting documents for approval, as are more fully specified herein. The A-E shall incorporate information provided by the Air Force as part of completing the 35% design submittal. The model repairs may not proceed beyond 35% without written authorization from the 633 Civil Engineer Squadron Project Manager (633 CES/CENMP). The 35% design submittal shall include documentation noted in Part III and be provided per the Submittal Matrix. If this submittal is not complete, it shall be rejected and the A-E shall resubmit at no cost to the Government. 2.1.3 35% Model Repair Review Meeting: The A-E shall coordinate and schedule for a 1-day review conference with the design team. The purpose is to examine each design team comment and de- termine an appropriate resolution to each functional and comments. The 633 CES/CENMP may provide comments in advance of the meeting to familiarize the A-E with the comments. Copies of minutes and annotated responses to the review comments shall be distributed to the design team as part of the next design submittal. 2.1.4 Pre-Final Model Repair (90%) 2.1.5 90% Model Repair Submittal: The A-E shall allow the government to view the 90% completed model repairs and other supporting documents for approval, as are more fully specified herein. The A-E shall incorporate information provided by the Air Force as part of completing the 90% design submittal. The model repairs may not proceed beyond 90% without written authorization from the 633 Civil Engineer Squadron Project Manager (633 CES/CENMP). The 90% design submittal shall include documentation noted in Part III and be provided per the Submittal Matrix. If this submittal is not complete, it shall be rejected and the A-E shall resubmit at no cost to the Government. 2.1.6 90% Design Review Meeting: The A-E shall coordinate and schedule for a 1-day review conference with the design team. The purpose is to examine each design team comment and de- termine an appropriate resolution to each functional, technical, cost, and other comments. The 633 CES/CENMP may provide comments in advance of the meeting to familiarize the A-E with the comments. The design may not proceed to final design without written authorization from the 633 CONS/LGCB. 2.1.7 Final Model Repair (100%): The A-E shall deliver and mount the repaired model, fiberglass cover and wooden frame to its original display pedestal location in the Civil Engineering Confer- ence Room. This includes incorporation of all comments agreed to from the 90% design review. Re-submittals may be required if construction documents have unresolved quality and/or con- structability issues. 2.1.8 Monthly Progress Reports: Provide monthly project development reports which acknowledge the following: (a) Project Management (address duties performed, issues awaiting resolution, actions taken on outstanding items, percent of remaining on this item) (b) Status of 35% Preliminary Design and Review Meeting (address submittal dates, completion schedule, unresolved meeting issues when applicable, note when complete and date) (c) Status of 90% Design and Review Meeting (address submittal dates, completion schedule, unresolved meeting issues when applicable, note when complete and date) (d) Status of Final Model Repair Submittal (address submittal dates, completion schedule, unresolved meeting issues when applicable, note when complete and date) (e) Percent of Project Schedule Complete to Date (note current percent of project complete) (f) Outstanding or Unresolved Issues (note any problem with coordination or matters that lack response(s) that are negatively affecting project schedule and completion of current stage of design) (g) Provide all monthly report as an office memorandum, signed, dated and sent to: 633d Contracting Squadron/LGCB 74 Nealy Avenue, Suite 100 JBLE - Langley, VA 23665 (Attn: Contract Administrator Name) 2.1.9 Certifications: At the minimum, a degree in architecture or three years of experience in architectural model making is required to perform this services. 3 STATEMENT OF SERVICES 3.1 TITLE I SERVICES: The A-E shall provide Title I and Other Services as required. 3.2 TITLE II SERVICES: Not Applicable. 4 DESIGN DOCUMENTS AND SCHEDULE 4.1 DELIVERABLES 4.1.1 Government Furnished Materials The Government will provide the Contractor with data and information concerning all necessary and pertinent functions and principal features of the identified project as deemed necessary for project completion, pending approval from the Government. These items may include: • The installation's most current orthorectified imagery and its geospatial parameters (co- ordinate system, datum, projection, distance units). • Any pertinent and necessary prototype or seed files. • Any other data or schematics deemed necessary for project completion, pending approval from the Government. 4.1.2 Government Review The Government shall review the submitted data and documentation upon completion of all stat- ed work. Missing or incomplete items shall be documented and forwarded to the Contractor for completion. Upon receipt of a complete submittal, the Government shall conduct a quality re- view and notify the Contractor within 14 days of acceptance (along with any stipulations this in- cludes) or rejection of the deliverables described herein. Failure to adhere to any of the stated de- livery specifications could result in rejection of deliverables and nonpayment. 4.1.3 SCHEDULING OF MEETINGS: All meetings shall be held at JBLE-Langley and schedule coordinated a minimum of two weeks in advance with the schedules of 633 CES representatives. 4.1.4 REPAIR SCHEDULE: The A-E shall be required to complete design submittals according to the design schedule. The schedule shall be determined by the 633 CES Project Manager and will vary depending on the complexity of the design. A Sample design schedule is provided as follows: Task Duration 35% Repair Submittal............................................ 15* Government Review 2 35% Repair Review Meeting (1 day) 1 90% Repair Submittal............................................ 30* Government Review 2 90% Repair Review Meeting (1 day) 1 100% Corrected Final Repair Submittal/RTA....... 10* *from Notice to Proceed 4.1.5 Submittal Deliveries: Upon completion of the updates, the model shall be delivered in its frame and glass case and mounted onto its existing pedestal in the CE Conference Room. 4.1.6. Payment Monitoring Plan: The A-E shall develop a payment plan to be submitted with each delivery order proposal. If the A-E's payment plan is not acceptable to the Government, a final payment plan will be negotiated. Payments can be authorized once a design submittal has been approved on the AF Form 3000, Material Submittal Form. The maximum A-E payment shall be the amount equal to the complet- ed design milestone percentage of the contract price.
