- Statement of Work Wage Determination Price List Combined Synopsis/Solicitation - FA9301-17-R-0018 This is a combined synopsis/solicitation which is expected to result in a Firm-Fixed-Price Blanket Purchase Agreement for Testing and Repair of Portable Fire Extinguisher Systems, prepared IAW the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Signed agreements are being requested and an additional written solicitation will not be issued. This is a Request for Quotation (RFQ), and this requirement is conducted as a commercial item procurement using Simplified Acquisition Procedures (FAR Part 13). All responsible sources may submit a firm-fixed price (FFP) quote which shall be considered by the agency. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation (FAR) and Supplements, Federal Acquisition Circular (FAC) 2005-95 (effective 19 Jan 2017), AFAC 2017-0127 (effective 27 Jan 2017) and DFARs Change Notice (DPN) 20161222 (effective 22 Dec 2016). This is a Commercial acquisition, under Full and Open Competitive Procedures. The Air Force Test Center (AFTC) located at Edwards Air Force Base, CA is seeking to award a BPA for Testing and Repair of Portable Fire Extinguisher Systems. An agreement shall be made on the basis of a responsible offeror who submits a fair and reasonable price list in accordance with the evaluation procedures described in this RFQ. Prices will be determined fair and reasonable and shall be in accordance with discounts and prices provided to preferred customers or customers with the same or similar purchasing arrangements/agreements. BPA Number: TBD Description of supplies: Testing and Repair of Portable Fire Extinguisher Systems North American Industry Classification (NAICS) Code: 811310, with a size standard of $7.5M BPA Closing Date: Technical Proposals and Pricelists must be received no later than 4:00PM., (Pacific Standard Time), on 08 September 2017. The entire proposal must be contained in a single e-mail that does not exceed 5 megabytes including attachments, if any. E-mails with compressed files are not permitted. SAM can be accessed at https://www.sam.gov. Addendum to 52.212-1 - Instructions to Offerors-Commercial items (Oct 2015) 1. Price List: Offerors are to return the completed Price List (Attachment #3) along with their proposal submission. Offerors are to specify the effective date of the initial price list which shall be 1 Oct 2017 and specify the expiration date which shall be no earlier than 1 year from the effective date. EVALUATION CRITERIA: The Government reserves the right to award multiple agreements resulting from this solicitation. Technical tradeoffs will not be made and no additional credit will be given for exceeding acceptability. Award will be made to the acceptable offerors with the lowest price, which is deemed responsible in accordance with the Federal Acquisition Regulation and whose proposal conforms to the solicitation/RFQ requirements. The solicitation/RFQ requirements include all stated terms, conditions, representations, certifications, and all other information required by this solicitation. The Government reserves the right to award without discussions Therefore, each initial offer should contain the offeror's best terms from a price and technical standpoint. The following factors shall be used to evaluate offers: Significant Factors and/or Subfactors: 1. Lowest Price PRICE EVALUATION Price analysis will be performed to determine the reasonableness of the offeror's price proposal. Reasonableness will be based on the total evaluated price. The total evaluated price will be calculated as the sum of prices based on the Offerors completed Price List submission. BPA Master Dollar Limit: $150,000.00 BPA Call Limit: $25,000.00 Period of Performance: 1 October 2017 - 30 September 2022 Salient Characteristics of Testing and Repair of Portable Fire Extinguisher Systems - See SOW BPA INSTRUCTIONS •1. DESCRIPTION OF AGREEMENT: This Blanket Purchase Agreement (BPA) is for the testing and repair of all portable fire extinguishers utilitzed on Edwards Air Force Base. The Contractor shall furninsh all supervision, equipment, tools, and labor, materials, and supplies necessary to perform the required work. Extinguishers shall be serviced as needed in lots containing no less than 25 extinguishers of any/all types during the performance period. •2. DELIVERY OR PERIOD OF PERFORMANCE: The BPA period shall be effective for a period of five (5) years from date of issuance. •3. EXTENT OF OBLIGATION : The U.S. Government is obligated only to the extent of authorized calls actually placed against this BPA. •4. PRICING : Prices charged by the contractor to the U.S. Government Payment shall be in accordance with the approved current price list for actual calls. The contractor shall furnish price lists periodically with prices that are firm for not less than one year. •5. PURCHASE LIMITATIONS: No individual call under this Blanket Purchase Agreement shall exceed $25,000.00 unless approved by the contracting officer. •6. NOTICE OF INDIVIDUALS AUTHORIZED TO PURCHASE UNDER THE BPA AND DOLLAR LIMITATIONS: The Contracting Officer will periodically furnish the contractor a list of individuals identified by names authorized to purchase under this Blanket Purchase Agreement and the dollar limitation per purchase for each individual. •7. DELIVERY TICKETS: All services and supplies performed under this Blanket Purchase Agreement require the contractor to prepare delivery tickets for each call made. The original will be attached to the contractor's invoice for the using activity and one copy for the contractor's records. The following information must be shown on each delivery ticket at a minimum: a) Name of Contractor b) Blanket Purchase Agreement Number c) Date of Call d) Call Number e) Name of individual placing call f) Itemized list of services performed/supplies delivered g) Quantity, Unit price and Total Price (less applicable discounts) h) Date when services where performed/delivery was made. •8. PAYMENT: Payment will be made via an authorized call with certify funding document attached. Under no condition shall the vendor make delivery of or process charge for any amount in excess of $25,000 per order. Vendor shall not process charges for more than one delivery on the same charge. Each delivery shall be processed as a separate charge transaction and each transaction shall have a record maintain in the BPA Call Register. •9. PARTIAL PERFORMANCE OF SERVICES: No partial performance of services per call will be accepted unless authorized by the contracting officer. •10. DISCONTINUING BPA: This BPA can be discontinued upon thirty (30) days written notice by either party, the Government or the contractor. •11. ANNUAL REVIEW: This BPA shall be reviewed annually before the anniversary of its effective date and revised as necessary to conform to the requirements of Federal Acquisition Regulation (FAR). Modifying the agreement itself can only change this BPA. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. (End of clause) 5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOV 2012) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and and 1) contract number, 2) location of work, 3) dates access is needed to obtain a vehicle pass. Additionally, Vendors/Contractors with driver's licenses from ME, MN, MO, MT and WA (exceptions are Enhanced Driver's Licenses (EDLs) from MN and WA) will not be able to obtain Base access without another form of identification as an identity proofing document. Dhs.gov/current-status-states-territories. The Contractor shall comply with all security requirements imposed by the Installation. to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. (End of clause)
