- Sources Sought Hospitalist 1. Contractor shall provide the services of two (2) Board Certified / Board Eligible (preferred) Hospitalist Physicians in Internal Medicine or Family Practice with Urgent Care experience, for the Oscar G. Johnson VA Medical Center, 325 East H Street, Iron Mountain, Michigan. 2. The Hospitalists shall cover the following shifts on a rotation: Description Time Urgent Care Center 7:00am-7:00pm Medical Officer of the Day 7:00pm-7:00am Medical Officer of the Day 4:00pm-8:00am Urgent Care Center 8:00am-4:30pm Hospitalist 730am-5:30pm (8-10 hours consecutive days from Monday to Monday scheduled three times a year) 3. The Oscar G. Johnson VAMC has low volume and low acuity, with no Intensive Care Unit with an inpatient average census of eight patients per day. Hospitalist Physicians shall provide clinical support to include, but not limited to, the following: Provide prescribed medical treatment and personal care services to ill and injured patients. Patients may also present with occasional life threatening emergencies. Physicians will provide a full range of medical care to patients with a variety of physical problems. Have a diversified background and expertise in the treatment of complicated chronic medical conditions within the facility. Demonstrate the knowledge and skills necessary to provide care appropriate to the adult patients. Ability to use electronic medical records for entry of physician orders, to include ordering medications, ancillary tests, progress notes, and delivery and follow up of medical care. Progress notes to be entered the day the service is rendered, with completion of the electronic encounter forms at the time of encounter. Facilitate positive patient care outcomes; working cooperatively with other providers, registered nurses, and a variety of allied health professionals to deliver comprehensive health care services to civilian and military patients. Establish effective communication with patients, families, visitors, and members of the interdisciplinary team, as well as with outside hospitals and clinics utilized as referral sites to ensure smooth transitions for continued patient care with follow up plans communicated. Ensure admissions are appropriate and timely. Demonstrate organizational skills which enable completion of work, be it during Hospitalist Physician duties, to complete appropriate paper work for any and all transfers to VA facilities or outside facilities. Maintain lines of communication between referring hospitals so that referrals are made without problems. The provider is responsible to initiate and facilitate treatment and transfer during Urgent/Emergent situations. The provider is responsible for completion of Non-VA Care/Choice consults and transfer paper work as required and will refer patients as needed to appropriate specialists, health care facilities, and coordinate home health services with appropriate documentation and forms completed. Ensure that all patient safety incidents are reported using the Patient Safety Report. All incidents shall be reported immediately (within 24 hours).
