Description
Crownpoint Cable Plan 2 Statement of Work Crownpoint Visio2 Present Design This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is 17-245-SOL-00016 and is issued as a Request for Quote (RFQ). The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-95 dated January 19, 2017. This solicitation is a 100% Buy-Indian Set Aside under the Buy Indian Act, 25 U.S.C. 47, and offers are solicited only from Indian Economic Enterprises. Any acquisition resulting from this solicitation will be from such a concern. Offers received from enterprises that are not Indian economic enterprises will not be considered and will be rejected. The associated North American Industry Classification System code (NAICS) is 238210 and the size standard is $15M. The Navajo Area Indian Health Service, Crownpoint Health Care Facility intends to award a firm-fixed price contract for the provision of Horizontal and Vertical Infrastructure Implementation. Services shall be provided on-site at the Crownpoint Health Care Facility located at State Road 371 and Route 9 in Crownpoint, NM, in accordance with the attached Statement of Work (SOW). The Contractor shall furnish the necessary transportation, personnel, equipment, supplies and materials to complete the implementation for CHCF. FEE SCHEDULE: 1. 1 JOB at __________________ 2. New Mexico labor tax: ____________________ 3. Navajo Nation Tax, 5%: _____________________ PROVISION FAR 52.212-1 -- INSTRUCTIONS TO OFFERORS -- COMMERCIAL ITEMS, applies to this acquisition. FAR 52.212-2 - EVALUATION - COMMERCIAL ITEMS The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made using the best value continuum considering all evaluation factors other than cost or price, when combined, are significantly more important than cost or price. The following factors shall be used to evaluate offers: (a) Technical Capability: Factor 1: Technical Approach and Methodology: The offeror's shall propose a horizontal and vertical cabling infrastructure design which shall meet the salient characteristics as defined in the SOW. The offeror's shall describe its assessment of the objective to be accomplished under this requirement and detail corporate resources to be applied as well as procedures for coordinating management and on-site personnel. Factor 2: Project Plan - Scope of Work: The offeror's shall describe a project management plan that will fully integrate, manage, control and document all phases of the contract requirements; provide a project management plan that describes a method for providing he necessary managerial and administrative resources. Factor 3: Qualifications: The Government will consider the following elements when evaluating the offeror's key personnel: education, trade, and years of experience relevant to perform contract requirements; and appropriateness of the skills sets of each proposed personnel to successfully perform the contract. Factor 4: Past Performance: The offeror's must provide documented evidence of a minimum of three (3) references from previous or current federal, tribal or private sector clients. (b) Pricing: Price will be evaluated in order to determine if it is reasonable in accordance with FAR Parts 13.106-3 and 15.402(a)(2). Offers should be sufficiently detailed to demonstrate their reasonableness. Offeror's shall meet all the requirements in RFQ and submit their responses to the technical evaluation factors in a separate binder titled and tabbed accordingly. Buy Indian Act 25 U.S.C. 47 Under the Buy Indian Act, 25 U.S.C. 47, offers are solicited only from Indian economic enterprises, any acquisition resulting from this solicitation will be from such a concern. Offers received from enterprises that are not Indian economic enterprises concerns will not be considered and will be rejected. Definitions as used in this clause. Indian means a person who is a member of an Indian Tribe or "Native" as defined in the Alaska Native Claims Settlement Act (PL 92-203; 85 Stat. 688; 43 U.S.C. 1601). Indian Economic Enterprise means any business activity owned by one or more Indians or Indian Tribes that is established for the purpose of profit, provided that: (i) The combined Indian or Indian Tribe ownership shall constitute not less than 51 percent of the enterprise; (ii) the Indians or Indian Tribes shall, together, receive at least a majority of the earnings from the contract; and (iii) the management and daily business operations of an Indian economic enterprise must be controlled by one or more individuals who are members of an Indian Tribe. To ensure actual control over the enterprise, the individuals must possess requisite management or technical capabilities directly related to the primary industry in which the enterprise conducts business. The enterprise must meet these requirements throughout the following time periods: (1) At the time an offer is made in response to a written solicitation; (2) At the time of contract award; and, (3) During the full term of the contract. Indian Tribe means an Indian Tribe, band, nation, or other recognized group or community which is recognized as eligible for the special programs and services provided by the United States to Indians because of their status as Indians, including any Alaska Native village, regional or village corporation established under the Alaska Native Claims Settlement Act (PL 92-203, 85 Stat. 688; 43 U.S.C. 1601). Under the Buy Indian Act, offers are solicited only from Indian economic enterprises. The government will reject all offers received from ineligible enterprises. Any award resulting from this solicitation will be made to an Indian economic enterprise, as defined above. If a contractor no longer meets the definition of an Indian economic enterprise after award, the contractor must notify the CO in writing. The notification must include full disclosure of circumstances causing the contractor to lose eligibility status and a description of any actions that the contractor will take to regain eligibility. Failure to give the CO immediate written notification means that: (i) The economic enterprise may be declared ineligible for future contract awards under this part; and (ii) Indian Affairs may consider termination for default if it is in the best interest of the Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item. Responsible sources must be registered and active in the System for Award Management (SAM) database, website accessed through http://www.acquisition.gov. Participation in the Mentor-Protégé Program established under section 831 of the National Defense Authorization Act for Fiscal Year 1991 (25 U.S.C. 47 note) does not render an Indian economic enterprise ineligible for contracts awarded under Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item. Responsible sources must be registered and active in the System for Award Management (SAM) database, website accessed through http://www.acquisition.gov. Applicable FAR provisions and clauses that apply to this acquisition: Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer, or submit a printout to their Online Reps and Certs. Clauses included in this combined solicitation/synopsis are not all inclusive. Clauses included on any ensuing award may include all or some of the below clauses. Contractor's should assume that all of the below clauses are applicable to this acquisition. Additional clauses may also be added at the discretion of the contracting officer upon award.
