- STATEMENT OF WORK DEFINITIONS VISN 7: Veterans Integrated Service Network VISN 7 Southeast Network VAMC: Veterans Affairs Medical Center Special Handling: Unusual circumstance may dictate the need for a dental order to be picked up specially, specially shipped and returned overnight, or processed within a shorter time frame. Routine Handling: A dental laboratory work authorization that is usually performed and returned within a specified time frame. Turn Around Time (TAT): The length of elapsed time between pickup of the dental lab order by the contractor until the receipt of the complete order by the VA Dental Service VA Dental Service: The Tuscaloosa Veteran Affairs Medical Center, Dental Service, Tuscaloosa, AL Using Service: Same as VA Dental Service CO: Contracting Officer COR: Contracting Officer's Representative Dental Laboratory Services: Those laboratory procedures that are required for the treatment of dental patients. These would include, but are not limited to, all laboratory fabrication aspects of complete dentures, removable partial dentures, fixed partial dentures and crowns, and dental implant prosthesis. Working Day: Any twenty-four hour period of the typical administrative work week, i.e., Monday through Friday (M-F). For example, if the contractor is allowed 5 working days for a prosthesis and he picks it up on a Monday, it would be due back to the VA the following Monday. Federal holidays occurring M-F are not considered a working day. VA observes the ten (10) Federal holidays listed below. Scheduled clinic appointments will not be available at VA during these holidays. Those holidays are: New Year's Day Martin Luther King's Birthday President's Day Memorial Day July 4th Labor Day Columbus Day Veteran's Day Thanksgiving Day Christmas Day GENERAL REQUIREMENTS The contractor shall furnish Monday through Friday between the hours of 7:30 a.m. and 5:00 p.m. local time, all labor, transportation, materials, general dental laboratory tools, equipment and supervision required to provide Dental Prostheses to the VA Medical Center Dental Services, 3701 Loop Road, Tuscaloosa, AL 35404. The contractor shall fabricate dental prostheses in strict accordance with the VA Dental Service order, which shall include but not be limited to the following: all laboratory fabrication aspects of complete dentures, removable partial dentures, fixed partial dentures and crowns, and dental implant prosthesis. They must be made locally and not be shipped out of country for fabrication. Special handling may be required on specific orders and will be authorized on a case by case basis by the VAMC and paid for under a separate purchase order. VA Dental Service staff will package the lab cases in the clinic area and place them in sealed, biohazard bags. The bags are placed in boxes and a shipping label is affixed. The boxes are picked up by VA Medical Center warehouse staff and shipped via VA contracted shipping company to Contractor. VA Dental Service will track the delivery of shipments. The completed cases will be shipped back to VA Dental Service, via VA contracted shipping company, from Contractor to VA Medical Center warehouse. VA Medical Center Warehouse staff will deliver the shipment to the VA Dental Service. The VA Dental Service program support assistant will sign for the package. The contractor shall furnish dental prostheses to the VA Dental Service Monday through Friday between the hours of 7:30am to 4:30pm. The normal turnaround time (in working days as defined) for dental prosthetic items shall be: Twenty-four hours: Complete and Removable Partial Denture, acrylic resin repairs Complete and Removable Partial Denture, tooth additions/repairs Removable Partial Denture, clasp repair/replacement and wrought wire clasp retained by acrylic resin Custom impression trays Custom impression trays, altered cast impression Poured Impressions Articulation of cast Cast duplications Fluoride application tray Occlusal nite-guard, vacuuform Forty-eight hours: Complete and Removable Partial Denture, relines Complete and Removable Partial Dentures, rebase Complete and Removable Partial Denture, record bases and occlusion rims Complete and Removable Partial Denture, record bases and occlusion rims, stabilized/processed Transitional Removable Partial Denture, processed Post and Core, cast Solder Connectors Fixed Partial Denture (FPD) crown working casts Three Working Days: Implant Fixed Partial Denture, veneer application Surgical Stents, processed Diagnostic wax-up, four teeth or less Tooth Rearrangements, Complete and Removable Partial Dentures Fixed Partial Denture, framework Implant Fixed Bar Porcelain to metal crowns Five Working Days: Complete and Removable Partial Denture, process acrylic resin Obturator, process acrylic resin Removable Partial Denture, framework Porcelain butt margin crowns Surveyed Crowns Crowns, to fix existing Removable Partial Denture Tooth Arrangements, Complete and Removable Partial Dentures FPD/crown fabrication or veneer application Occlusal nite-guard, processed acrylic resin All ceramic restorations Diagnostic wax-up, five teeth or more Eight Working Days: Removable Partial Denture, framework w/incorporated occlusion Fixed Partial Denture, eight units and over Implant Bar with acrylic resin base Ten Working Days: Attachment Prosthesis 5. The following materials shall be used in the fabrication of dental prostheses Complete Dentures/Removable Partial Dentures: Lucitonc 199 acrylic resin or equivalent Coelor coloration or equivalent Occlusal Scheme: Monoplane, linear Anatomic, full balanced Lingualized, linear 33 */monoplane 30 *rational Tooth Material: Trybyte TmIPN or equivalent Fixed Partial Denture/Crown: Gold: Harmony hard or equivalent High noble allow for PFM restorations: Olympia or equivalent Implant Systems: Barnemark System or equivalent 3i Systems or equivalent Calcitek TM Systems or equivalent Paragon TM Systems or equivalent Means degrees The contractor shall provide the VA Dental Service with authorization and order forms. 7. The contractor shall pick-up from and deliver to the VA Dental Service once daily Monday through Friday. 8. Contractor shall consult with the appropriate dental care provider at the VA Dental Service regarding any specific laboratory order. QUALIFICATIONS, LICENSES AND INSPECTION CONTRACTOR The VISN 7 contracting officer reserves the right to thoroughly inspect and investigate the contractor's and sub-contractor's facilities and other qualifications. The contractor and all sub-contractor's facilities shall have all licenses, permits, and certifications as required by local and state authorities. Current copies of these must be provided to the contracting officer upon submission of this RFP and successful contractor shall submit any changes throughout the life of the contract. The contractor shall be certified by the National Association of Dental Laboratories (NADL). Certification shall be submitted upon submission of RFP. QUALITY OF WORK Communication back and forth between the VA and the vendor is important to maintain and coordinate the quality assurance aspects of this contract. The contractor will follow the Dental Laboratory Evaluation Form Fixed Prosthesis Pages 46-56. Remakes: If it is found that any laboratory items are not compliant with the Dental Laboratory Evaluation Form Fixed Prosthesis Pages 46-56, the contractor shall remake the item at no additional cost. The COR will notify the vendor in writing on a monthly basis of any issues concerning quality complaints of dental laboratory items providing detailed information for the vendor to correct identified discrepancies. SPECIAL REQUIREMENTS The contractor shall notify the contracting officer of any proposed changes in sub-contracted dental laboratories and have prior written approval (by the contracting officer) before any changes are implemented. The contractor shall absorb any increase in charges as a result of any changes in sub-contracted laboratories. Quality Control: Contractor shall provide a detailed description of quality control policies and procedures followed by the contractor in the fabrication of dental prostheses. The contractor and VA Dental Service shall agree to a pick-up and delivery. The contractor will provide to COR a point of contact to be used by VA Dental Service to inquire about specific dental laboratory orders. The contractor shall maintain acceptable services, reporting systems, and quality controls as specified herein. Failure to comply with the specified terms and conditions and/or failure to perform satisfactorily may be grounds for pursuing termination of the contract. The contractor will complete TMS trainings for Privacy and Information Security at https://www.tms.va.gov/plateau/user/login/jsp Please contact the COR for the instructions on how to self-enroll and complete the course. After the TMS courses are complete a copy will be provided, prior to providing services, to the COR and annually thereafter. Upon completion or termination of the applicable contract(s) or agreement(s), return and/or destroy, at Covered Entity s option, VA information gathered, created, received, or processed during the performance of the contract(s) or agreement(s). No data will be retained by Business Associate, or contractor, subcontractor, or other agent of Business Associate, unless retention is required by law and specifically permitted by Covered Entity. As deemed appropriate by and under the direction of Covered Entity, Business Associate shall provide written assurance that all VA information has been returned to Covered Entity or destroyed by Business Associate. If immediate return or destruction of all data is not possible, Business Associate shall notify Covered Entity and assure that all VA information retained will be safeguarded to prevent unauthorized Uses or Disclosures. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract.
