Description
Statement of Objectives (SOO) This Statement of Objectives (SOO) provides the basic, top-level objectives of a Purchase Task Order (PTO). This statement is designed to provide potential offerers the flexibility to develop cost effective solutions and opportunity to propose innovative alternatives for meeting the basic required objectives as outlined below. 1. Purpose: Create a pharmacy work environment that maximizes the efficiency of space utilization and enhances the flow of work production for the unique requirements of inpatient, outpatient, logistics, administrative offices, and staff common areas within the pharmacy department. Design must minimize ergonomic concerns for employees' repetitive work functions. Designs will incorporate current work flow and/or operational requirements for the following automation systems: Qflow; Parata; ScriptPro; Pyxis; Rees. 2. Scope: The contractor will propose an efficient workflow design per the specifications of the performance objectives (Section 8) below for inpatient, outpatient, logistics, administrative offices, staff common areas within the pharmacy department, narcotic vault, Drug Information Center, concierge's desk, and front lobby as noted on the attached drawing (Enclosure 1). The contractor will provide all management, supervision, labor, material, and tools to install new furniture and pharmacy modular casework per the contract. Upon installation, casework will have full functional power and data connections, where required, that are compatible with departmental power, data, etc. outlets. The movement of any existing automated systems, electrical outlets, lighting fixtures, etc. that are to be incorporated into the proposed workflow design will be a coordinated effort by the pharmacy department and respective area/service attached to the design's specifications. 3. Background: Due to automation changes; pharmacy operations and workflow production are now limited by current space design. Additionally, occupational health assessments should be considered within design to limit unnecessary employee fatigue and strain. 4. Location: Naval Medical Center San Diego (NMCSD) Pharmacy, Location 1E-22F1. 5. Room Measurements: Size of the lobby, main pharmacy, inpatient, outpatient, common staff areas, Drug Information Center, and office spaces are irregular and spread throughout multiple locations. Total space is approximately 28,000 square feet. The contractor is responsible for the final measurements of each location where the work will be performed. Contractor measurements will be used to ensure that the furniture installed by the contractor will fit properly and be workable. Contractors are invited to attend a site visit as listed below. 6. Site Visit: A site visit will be conducted with a date/time to be arranged by the Contract Specialist. It is vital that contractors attend the site visit to view the work area and perform tasks required, such as measuring the space to accurately assess the Governments requirement for work under the contract. 7. Performance Objectives, goals, and outcomes: The following objectives are minimum requirements needed for the successful operation of the pharmacy department. Any modifications to these objectives will need approval from the Pharmacy Department Chair or appointed representative. General Guidelines and Definitions -All furniture to be replaced will be removed from the Command by the contractor after permission has been given by the Pharmacy Department's Logistics Division. -Shelving cabinets will be gang-able in side-by-side and/or back-to-back configuration, to provide increased support. -All drawers will be full extension and have soft closing drawer rails. -All cabinet/closet hinges will have soft closing function. -The irregular shape of the pharmacy may require adjustments to fit shelving and cabinets. The specifications below are the ideal goals to meet the mission. -All furniture will fit properly within space and be level and serviceable upon installation completion. -Design will need to incorporate current refrigerators and freezers. -The proposed plan/layout will need to incorporate a phased implementation approach to ensure that delivery of pharmacy care is not delayed or interrupted during installation. This will be coordinated by the contractor and pharmacy department representative. -Unless otherwise noted, a minimum of 36 inches is required for passageways between furnishings and main egress routes. -Unless otherwise approved all shelves will be: • Uniformly adjustable in no more than one and one-quarter (1 ¼) inch increments to allow for best placement of shelves and assist gravity flow. • Shelves will be no more than 36 inches in length and no less than 22 inches in length. • Shelves will have mounting clips, such as pegs, which affix the shelf to the cabinet wall to prevent falling in a gravity flow position, but also allow for easy adjustment of shelf when needed. • No less than seven (7) shelves will be provided for each cabinet section. -Overall pharmacy design should include: • Inpatient pharmacist stations away from main traffic, in a reduced noise area, but near current IV prep room. • Inpatient and outpatient Lead Petty Officer (LPO) office within main pharmacy. Area must be semi-private to accommodate for private conversations. • Outpatient controlled substances workstation near Parata units. • As close to unidirectional flow for outpatient prescription thorough put with consideration for lobby design and patient movement. -Areas requiring sinks will be made of stainless steel or other non-porous material, have "deep sinks" (12 inches), faucet height of no less than 10 inches, and must have a "hands free" option. A drying rack must be within the design to prevent water from pooling. Design must include (supplied by Pharmacy Department) paper towel dispenser, soap dispenser, and hand sanitizer unit. The sink shall be American Disability Act (ADA) compliant and have appropriate caulking and mounting to meet code. -All shelving shall: • Be adjustable. • Have the capability to divide drugs within shelves either designed within the shelf, through a "pusher" style system, or other means to divide medications. Quantity of dividers and/or bins must be sufficient for all medications currently in stock plus a 10% overstock for future medication expansion. Quantities will be listed for each section below. • Have a "front fence" or rail to accommodate one (1) inch labels. • Have sections that do not exceed four (4) feet to accommodate for bowing/weight. • Have a depth will not be less than 12 inches, but not exceed 18 inches except for areas requiring bulk storage. These areas will be identified in objective in Section 8. • Have a kick plate of no less than six (6) inches from the floor and no more than eight (8) inches from the floor to comply with local medication management policy. Vial drawer cabinets may have a kick plate less than six (6) inches because the drawer's face plate meets protection requirements. • If the cabinet is to be wood based, then it will be one (1) inch thick on the sides and backing with shelving no thinner than three-quarters (¾) of an inch. • At corners, shelving will not block other shelving, cabinets, etc. -All counters shall: • Be composed of a non-porous and seamless material. • Have a kick plate of no less than six (6) inches from the floor and no more than eight (8) inches from the floor to comply with local medication management policy. • Have a "lip" or backsplash in areas requiring medications to be counted to accommodate for potential run off. • Have depths that will be at a minimum of two (2) feet unless otherwise specified. • Be at least 40 inches from the ground, but not exceed 46 inches. -All electrical and data cabling will be hidden and/or minimal visibility within casework. -A "standard computer workstation" will be defined as a desk that has a minimum height of 36 inches, but does not exceed 40 inches. Minimum dimensions will be four (4) feet by two (2) feet. Each station will require at least two (2) gang 120v/60Hz electrical plugs and four (4) data ports. If additional plugs or data ports exist at current station then that number of plugs and ports will be the minimum requirement. Work surface will have dual monitor mounts and a three (3) inch hole to accommodate for wiring. Station will require an under counter mount for one (1) Computer Processing Unit (CPU) and one (1) Uninterrupted Power Supply (UPS) unit. -CPU, UPS, and other electrical equipment requiring under counter mounting will be either within a shelf or a solid sided mount. "Strap" mounts will not be allowed in design. -Any above lockable or closable above desk storage requirements will be 110 degree horizontal openings or designed as to not impede with the space above storage cabinet. -"Red power" is defined as electrical outlets that will be supported by the Command's emergency power supply during loss of primary power. When "red power" is specified in casework, the design must include installation near outlets that are designated as emergency power outlets by the Command. -The area within the pharmacy that has blue flooring must remain an open space to receive and sort medication orders from prime vendor. -Corner desk or "L" shaped desk will be used minimally throughout this design. -The scope of work will require multiple furnishing solutions to support the layout and operation of NMCSD Main Balboa Pharmacy Department. Wood tone finishes that are complimentary to the Command's current remodeling project are preferred. Final product to be installed finishes, styles, colors, fabrics, frames, etc. will require the approval of the Pharmacy Department Chairman or designated representative prior to installation. Inpatient Pharmacy Area -General Guidelines for Inpatient Pharmacy Areas • Within design, must incorporate space for a 20 inch wide by 72 inch long by 68 inch high cart that is supplied by Central Supply that needs to be within quick access of IV room. • Incorporate a scrub machine unit. Dimensions will be available during site visit. • Capability to hang up 12 lab coats which can be on a hook or rod. • Include one (1) five (5)-drawer file cabinet and one (1) four (4)-drawer lateral file cabinet. • Within design, incorporation of 6 feet by 4 feet whiteboard that is within sight of the Medication Check Counter -Medication Check Counter (Unit Dose/IV Check) • One (1) counter working surface that must be five (5) feet by 10 feet to allow for preparation and checking of oral medications and Pyxis batch medication checks. • 40 linear feet of overhead shelving for common use unit dose medications. Shelving must be gravity fed and have sufficient bins to meet medication storage needs. Bins will be of variable size and sufficient for at least 200 medications within this area plus 10% expansion. Bottom of shelving unit will be no lower than 18 inches and bottom of top shelf will be no higher than 30 inches. • Incorporate a lockable narcotic locker with 30 linear feet of storage (shelves approximately 2 foot deep by 3 foot wide) to be placed either underneath counter or within the space. • Storage space for hazardous waste, chemotherapy, and incineration bins. • Each side should have deep drawer storage sufficient enough to accommodate for oral syringes, oral caps, and unit dose bags. Drawers will need to have a minimum of four (4) dividers per drawer. Currently inpatient has six (6) types of unit dose bags, four (4) types of oral syringes, and two (2) types of oral caps. • Each side must have a minimum of two (2) drawers for office supplies. • Under counter storage requirement will require a mixture of drawers and shelves. One (1) set of drawers will need to accommodate Adult and Pediatric Code bags. -Stainless Steel Pass-Through Airlock IV Window and Checking Counter • One (1) two (2) feet by four (4) feet checking counter to review IV medications prepared in the IV room. • Drawers and shelves below for storage. • Need an intercom system for communications into IV room. • Stainless steel pass-through must contain interlocks to prevent both doors from being opened at the same time. • Stainless steel pass-through must be two (2) feet by two (2) feet and seal against non-contaminating, chemically-resistant polyurethane gaskets for a secure sit. Germs and microbes must be effectively blocked from compromising sterile surfaces. Designs must be flush against cleanroom walls to reduce surface area and simplify cleaning -Crash Cart Preparation Area • Assembly/Verification Crash Cart Counter  One (1) five (5) feet by 10 feet counter space for preparing Adult, Pediatric, and Obstetrics Emergency Cart (OBEC) trays. Space under counters will need a combination of drawers and shelves to accommodate for supplies and empty crash cart trays. Minimum drawers with approximate dimensions of at least 20 inches wide and at least 10 inches deep to accommodate for crash cart supplies along with a minimum of three (3) "standard" depth drawers for supplies. The amount of shelving required should be able to hold 25 empty crash cart trays.  Crash cart supplies will need 10 linear feet of shelving. Shelf depth cannot exceed 18 inches.  A total of 60 linear feet to accommodate for crash cart medications. Minimum depth will not exceed 18 inches. Storage will be a mixture of above counter and adjacent to counter storage. Area should accommodate for at least 150 medication bins/dividers. • Three (3) distinct shelving units for verified trays (Adult, Pediatric, OBEC)  Each unit will be capable of hold 10 trays with a dimension of two (2) feet by three (3) feet. Each unit must be clearly delineated as adult, pediatric, and OBEC. -IV Medication-Related Storage Shelving • Minimum of 200 linear feet for medication vials. Maximum shelf depth will need to be 18 inches. Shelving will need dividers for each medication. The minimum number of dividers will need to cover at least 300 medications plus a 10% expansion. • Minimum of 125 linear feet for premade IV medications. Shelf depth should approximately be two (2) feet. Premade bags will need to be in bins. The minimum number of bins will need to cover at least 62 medications plus a 10% expansion. • Minimum of 20 linear feet for IV supplies. Shelf depth should not exceed 18 inches. Minimum number of dividers will need to cover 20 different supply items plus a 10% expansion. -Unit Dose Medication Storage • Shelf space must accommodate for all medications currently located in Pyxis Room. A minimum of 600 lineal feet will be required. Unit dose medication will be required to be within bins ranging from two (2) inch to six (6) inches in width. A minimum of 600 medications will need bins within this area. • Medications must be located in close proximity to the Pyxis/Unit Dose Station and Medication Check Counter. -Four (4) Inpatient Technician Workstations • Four (4) standard computer workstations with access to red power. A three (3) drawer file cabinet will need to be incorporated below workstation. No more than one (1) storage system per two (2) workstations. Two (2) stations can share one (1) file cabinet. • One (1) of the four (4) standard computer workstations must be built within or adjacent to the Unit Dose/IV Counter. • All workstation must have a telephone line. • Incorporate one (1) copier/scanner/fax (supplied by pharmacy department) into design. Must have access to red power. Requires one (1) gang electrical 120v/60Hz outlet, two (2) data ports, and one (1) telephone line. • Eight (8) linear feet of shelving for binder storage. Shelves may be either above workstations or free standing. Cabinet doors will be preferred. -Four (4) Inpatient Pharmacist Workstations • Four (4) standard computer workstations with access to red power. A three (3) drawer file cabinet will need to be incorporated below workstation. No more than one (1) storage system per two (2) workstations. Two (2) stations can share one (1) file cabinet. • All workstation must have a telephone line. • 16 linear feet for binder storage and reference material. Maximum shelf depth will be two (2) feet and width will not exceed four (4) feet. Shelves may be either above workstations or free standing. Mixture of open shelves and lockable cabinets are preferred. -Printer Counter • One (1) counter surface large enough to accommodate two (2) DataSouth printers and one (1) Lexmark printer. Lexmark printer may be incorporated under counter if permissible. Counter will need one (1) "long slot" for paper feeding below surface and three (3) inch hole with cover for wiring (additional hole may be required if all three (3) printers will be on counter). • Must have shelving/drawers for paper (legal paper, letter paper, printer labels, etc.) • All printers must have access to red power -Pyxis Space • One (1) standard computer workstation that is on red power. • Four (4) linear feet of shelving for Pyxis documentation/references. • One (1) "standing" work station for Pyxis server. Counter height will be approximately 38 inches and have one (1) three (3) inch hole with cover to accommodate wiring. Under the counter will need mounts for one (1) CPU and one (1) UPS. A printer will need to be incorporated below the counter with paper storage. • Space for two (2) Pyxis units will need to be incorporated into the design. Dimensions for one (1) unit are approximately 3 feet by 3 feet. • One (1) four (4) feet by six (6) feet counter for current unit dose machine. One (1) three (3) inch hole with cover will need to be incorporated into the counter for power requirements. Unit dose machine has an under counter mount component that will need to be incorporated into design. Storage under cabinets will need to be a mixture of deep and standard drawers for unit does supplies. One (1) triple or greater glove box holder will be incorporated into design. • One (1) four (4) feet by six (6) feet Pyxis Work Island with shelves for tote storage and space to "park" three (3) delivery carts. Cart dimensions are approximately 18 inches by 30 inches and 36 inches in height. -Refrigerator Space Requirements • Must accommodate for three (3) double door refrigerators, one (1) single door refrigerator, and one (1) single door freezer. • All refrigerators will require red power access and connectivity to support Rees monitoring. -Sink Requirements • One (1) sink with pedestal or "hands free" capability that meets the general guidelines listed above. Outpatient -General Guidelines for Outpatient Pharmacy Areas • Floor plan will accommodate for four (4) Parata Max units within the filling area. The Max units' dimensions and additional work space requirements are listed in Enclosure Two (2). • Within pharmacy production area (filling, verification, staging, narcotic/control fill) all stations should have a direct line of sight to a pair of 40 inch ceiling or incorporated into work stations mounted monitors displaying one (1) monitor with the pharmacy production board and the other with general (out-of-stock) information. Number of monitors will be based on final layout. Additional CPU or point to point connection requirements will need to be added / evaluated into plan once final layout is approved. • Three (3) prescription file cabinets located near registration, stagger, and/or phone center. -Outpatient Lobby • Seating capability for 100 patients. Seats will have backs and arms. At least one-third (1/3) of the total chairs will accommodate for bariatric patients capable of supporting at least 500 pounds. Chair fabric will be washable and antimicrobial material. Lobby will incorporate the current patient queuing system flow that has four (4) kiosks and potential six (6) 72 inch television monitors. Kiosks will have hand sanitizing stations (provided by Pharmacy Department) located next to them. • Built-in lighted display/trophy case to feature historical memorabilia and awards. Must have at least six (6) adjustable shelves that are at least two (2) feet by four (4) feet. Minimum height requirement will be 72 inches (flexible requirement). • Need two (2) lighted wall mounted lockable glass encased shadow boards with minimum interior dimensions of two (2) feet tall, four (4) feet wide, and four (4) inch deep. • One (1) wall mounted glass information board/monitor next to each patient kiosk (flexible requirement). • Retain current location for MEDSAFE bin. • Creative and fun children's area. Will need Ultra-Violet (UV) light or other means to reduce spread of communicable "germs." All surfaces must be non-porous and capable of easy cleaning/disinfecting (flexible requirement). • Incorporate current queuing system CPUs and UPS currently located in "secretary space" into lobby space. Hardware must be well ventilated, but not accessible to patients. Must have point-to-point Ethernet cables to display monitors. Design will be reviewed and approved by NMCSD's IT Department prior to implementation. -Outpatient Concierge's Desk • Space will need to have privacy panels to separate pharmacy area from patient administration waiting lobby. Panels will need to accommodate for a door capable of being locked when area is not in use. Desk will need to accommodate for two (2) standard workstations that span the width of the current garage door. Each work station will need a patient privacy barrier. Counters should have a higher level for patients and a lower level for technician's computer, scanner, and area to work. Upper patient counter needs to have a covered 3" hole to accommodate for wiring. Upper patient counter space will need to be at least 18" in depth. This space will be in addition to the minimum standard workstation requirements. The area will need six (6) drawers, two (2) of which must be capable of holding file folders, and lockable. The area must also accommodate for a double drawer printer and paper storage. -Call Center / "After Hours" Discharge Pharmacy • Five (5) standard computer workstations located near frontline/pharmacist verification station(s), but semi-private to not disrupt phone conversations. At least one (1) workstation will need to be on red power. • A three (3) drawer file cabinet will need to be incorporated below workstation. No more than one (1) storage system per two (2) workstations (two (2) stations can share one (1) storage system) • Each station will need to have at least one (1) phone line. • A minimum of 40 linear feet of open cabinets/shelving will be needed for REMS, SOP, and other outpatient binders. • Incorporate one (1) copier/scanner/fax (currently located in front vestibule) into design. Must have access to red power. Requires one (1) gang electrical 120v/60Hz outlet, two (2) data ports, and one (1) telephone line. -Outpatient Production Areas • 14 Registration Windows  Maximize the ability to identify windows from the main lobby area to direct patients to appropriate area.  Technician workspace must have the minimum standard computer workstation requirements in addition to an "upper" counter space for patients. Each desk must accommodate two (2) CPUs and one (1) UPS along with two (2) gang electrical 120v/60Hz red outlets and four (4) data ports. Upper patient counter needs to have a covered 3" hole to accommodate for wiring. Upper patient counter space will need to be at least 18" in depth. Height from floor to patient counter must be within ADA guidelines.  A printer stand and paper storage area will be incorporated within "every other" registration window.  Each registration window must have divides to maximize patient privacy.  Each divide must have electrical and data connections to accommodate for future expansion of electronic visual display monitors at each window.  While an "open concept" is highly desired; staff security should remain the highest concern when developing concept. Design must incorporate panic buttons (provided by Pharmacy Department) along with ability to quickly secure space for CODE WHITES and protect staff from potential combative situations. • Eight (8) Filling Stations  Working counter space must have minimal dimensions of 2.5 feet by 6 feet. Counters will need "lips" to accommodate for pill spillage and have a height of approximately 38 inches. The counter will need to incorporate a 6 inch tall stand to accommodate for one (1) Eyecon pill counter. The stand must be 12 inches wide by 18 inches deep. A single drawer to accommodate pill counters below the stand is preferred. Counter tops will have dual mounted stands mounted on counter and one (1) 3 inch hole for wiring. Power requirements will be for two (2) gang 120v/60Hz red power outlets and four (4) data ports. Under the counter requirements are: mounts for one (1) CPU and one (1) UPS; two (2) adjustable vial drawers similar to current configuration; location for recycle, trash, and patient privacy information bins; and two (2) closing drawers (no less than four (4) inches deep) for supplies and label storage. • Fastfill Shelving  Minimal linear footage for one (1) "fastfill" shelving unit should be 108 feet. Shelf depth cannot exceed 1 foot.  Two (2) or more "fastfill" shelving units may be used in design as long as they are within "easy reach" of all filling stations. • Control/Robotic Workstation  Workstation will have 17.5 cubic feet (two and one-half (2.5) feet by seven (7) feet) of counter space which will comprise of a solid non-porous and seamless surface. Counters should have a backsplash and/or "lip" for all four (4) sides of the counter to prevent medication loss. Counters will be approximately 38 inches in height. Counter depth will not exceed two and one-half (2.5) feet. Mounts for one (1) CPU and one (1) UPS below the counter with one (1) three (3) inch hole with cover for cabling. Counter tops will have dual mounted stands for monitors. Power requirements will be two (2) gang 120v/60Hz power outlets and four (4) data ports. The counter will need to incorporate a 6 inch tall stand to accommodate one (1) Eyecon pill counter. The stand must be 12 inches wide by 18 inches deep. A single drawer to accommodate pill counters below the stand is preferred. Under the counter requirements are: two (2) drawers (no less than four (4) inches deep) for supplies; two (2) deep drawers/closed cabinets (no less than 10 inches deep) for compressed air, labels, etc.; and location for recycle, and trash bins.  Open shelving for labels will be 12 linear feet with a minimum depth of shelving to be 12 inches.  The controlled cabinet shelving will require 48 linear feet (three (3) two (2) foot wide sections with seven (7) adjustable shelves and one (1) static bottom shelf) of lockable shelving. Cabinet doors will have security glass or similar material to have visibility of medications while maintaining integrity of door. The shelf depth should be at least 12 inches, but not to exceed 18 inches. • Narcotic Workstation  The one (1) narcotic station will require 17.5 cubic feet (2 feet by 7 feet) of counter space which will comprise of a solid non-porous and seamless surface. Counters should have a backsplash or "lip" on all four (4) sides to prevent medication loss. Counters will be at 38 inches in height. Counter depth will not exceed 2.5 feet. Mounts for one (1) CPU and one (1) UPS below the counter with one (1) three (3) inch hole with cover for cabling. Counter tops will have dual mounted stands for monitors. Power requirements will be two (2) gang 120v/60Hz power outlets and four (4) data ports. The counter will need to incorporate a 6 inch tall stand to accommodate one (1) Eyecon pill counter. A single drawer to accommodate pill counters below the stand is preferred. The stand must be 12 inches wide by 18 inches deep. Counter will have a "slot" for Parata paper feed. Under the counter requirements are: three (3) drawers (no less than four (4) inches deep) for supplies; two (2) adjustable vial drawers similar to current configuration; and a location for recycle, HIPPA, and trash bins. Above counter will need to incorporate room for one (1) open four (4) feet by one (1) foot shelf to accommodate narcotic records.  Narcotic cabinet shelving will require 48 linear feet (three (3) two (2) feet wide sections with seven (7) adjustable shelves with a static bottom shelf) of lockable shelving. Cabinet door will be single door and lockable for each section. Cabinet doors will have security glass or similar material to have visibility of medications while maintaining integrity of door. The shelf depth should be at least 12 inches, but not to exceed 18 inches.  The station will have an additional counter with "honeycomb" boxes to accommodate prescriptions needing to be filled. The space will have nine (9) oversized honeycomb/pass-through holes. Dimensions for the holes from inside measurements will be 12 inchby 12 inch by 12 inch. The lowest honeycomb will be no lower than 36 inches.  Need to incorporate into the design a potential expansion for a Pyxis Narcotic Unit and two (2) towers with dimensional requirements similar to current Narcotic Vault. Depth of workspace can match sink depth.  Storage and/or drying rack for up to 6 graduated cylinders.  Space for two (2) "Fillmasters" or similar automated water for reconstitution dispensing system(s).  Storage for six (6) different sizes of amber bottles (2oz, 4oz, 6oz, 8oz, 12oz, and 16oz). Can be incorporated under sink or stand alone. • Chemo Workstation  A minimum of 18 linear feet of shelving that does not exceed 1 foot in depth.  Counter work surface with a minimum of 4.5 square feet (1.5 feet by 3 feet) of working counter. No electrical power or data requirements. Must have three (3) standard drawers for supply storage. Need space under counter for chemo med and chemo supply bins.  Need to install a triple or larger glove box holder. • Parata Packing Box Stations  Need three (3) low-counter work centers for the pharmacy automation's packing boxes. Height will need to be 36 inches for the portion that will support the packing boxes. They will have a width to be no less than the packing boxes. Overall counter dimensions should be approximately four and one-half (4.5) feet by one (1) foot.  The stations will also include three (3) oversized honeycomb/pass-through holes. Dimensions for the holes from inside measurements will be 12 inch by 12 inch by 12 inch. The lowest honeycomb will be no lower than 36 inches.  The space below the counter surface will have one (1) adjustable shelf. Each center will require individual two (2) gang 120v/60Hz power outlet and two (2) data ports. • Six (6) Pharmacist Verification Stations  Verification counter space must have minimal dimensions of 2.5 feet by 6 feet. Counters will need "lips" and/or backsplash to accommodate for pill spillage. Under counter storage must accommodate for one (1) CPU and one (1) UPS to be mounted below the counter and one (1) three (3) inch access hole with cover for cabling. Counter will have a "slot" for Parata printer paper feed. Counter tops will have dual mounted stands mounted on counter. Additionally, the space will need to have room for recycle, trash, and HIPPA bins; and two (2) drawers (no less than four (4) inches deep) for supplies and label storage. Vertical storage for five (5) different types of brown bags (2lbs, 5lbs, 10lbs, 16lbs, and 20lbs).  A printer stand and paper storage will be at every or every-other pharmacist station.  Magazine rack and/or similar style of mountable holder at each pharmacist verification station for labels (refer, RPh counsel, iPledge, etc.).  Each station must have a phone line. • "Filling Lead" Workstation  Working counter space must have minimal dimensions of two and one-half (2.5) feet by six (6) feet. Counters will need "lips" to accommodate for pill spillage. The counter will need to incorporate a six (6) inch stand to accommodate one (1) Eyecon pill counter. Phone must be easily available to all filling stations.  A printer stand and paper storage will need to be adjacent or incorporated into design. • ScriptPro Telepharmacy Server Workstation  Two (2) three (3) feet by three (3) feet counters to support ScriptPro Telepharmacy computers. Counter must be a standing counter with a minimum height requirement of 38 inches. One (1) station will have mounted or shelving below for 1 CPU, 1 UPS, and current server. One (1) three (3) inch hole with cover will be in counter for wires. If shelving is used then "access" space will be required between shelves to accommodate for wiring to data ports and electrical outlets. Shelf depth cannot exceed 1 foot.  Doorbell or intercom system for patients to alert night staff of arrival.  Three (3) to four (4) chairs identical to lobby chair requirements in hallway.  Must have a phone line at station.  A printer stand and paper and label storage.  Potential work surface and power/data requirements for future "telepharmacy" potential.  All equipment must be on red power. -Compounding Area • Located centrally between inpatient and outpatient operations. • Two (2) standard computer workstations with one (1) three (3) drawer file cabinet incorporated below workstation. Drawers must be able to lock. Each station must have a phone line. A minimum of 12 lineal feet of lockable office supply cabinets. • Print station to incorporate 1 Datasouth printer and 1 Lexmark printer. Station will need to incorporate paper storage and below paper feed "slot" for Datasouth printer. • A minimum of 16 lineal feet for cabinets/shelving for operational binders and references. • One (1) single door refer must be incorporated into space that has access to red power and meet Rees monitoring requirements. • Incorporate one (1) two (2) feet by two (2) feet acid locker and one (1) four (4) feet by two (2) feet flammable locker (current lockers will be used). • One (1) sink meeting general guideline requirements along with an additional 10 lineal feet of working counter space of non-porous material. Drawers must be able to lock. A minimum of 8 lineal feet of lockable office supply cabinets. • Two (2) large 18 inches by 24 inches lockable cabinets for investigational medications. • Need two (2) gang 120v/60Hz red electrical outlet and space for potential investigational refrigerate and freezer. The size of the refrigerator is a Helmer 28 inches by 80 inches by 29 inches. The freezer is 24 inches by 34 inches by 27 inches. Logistics -Inpatient Bulk Shelving • A minimum of 168 lineal feet for bulk inpatient medication storage that is located "near" inpatient. Shelving must be a minimum of 24 inches deep. Shelves must be adjustable and accommodate an average height of 18 inches. -Bulk Medication Supply Shelving • All shelving within the space will require visual signage to delineate area. • A total of 1,646 lineal feet will be the minimum requirement for bulk medication shelving. While shelves are required to be adjustable, reasonable medication heights should be considered when calculating number of shelves. Minimum dimensions will be 6 feet by 2.5 feet. Each station will require 2 data ports and two (2) gang 120v/60Hz power outlets. If additional plugs or data ports exist at current station then that number of plugs and ports will be the minimum requirement. Work surface will have dual monitor mounts and a 3 inch hole to accommodate for wiring. Each station will have one (1) 3 drawer lockable file cabinets that are capable of rolling and padded for seating availability. Each station will accommodate for the under mounting of 1 CPU and 1 UPS unit. Six (6) lineal feet of adjustable lockable overhead storage with lighting. • Two (2) 4 drawer lateral file cabinet. • Accommodate current refrigerator. • Small counter for microwave and coffee maker with underneath drawer and cabinet for storage. • One (1) printer stand with underneath storage for paper. • 18 lineal feet of book shelving.
