SECTION SF 30 BLOCK 14 CONTINUATION PAGE SUMMARY OF CHANGES SECTION A - SOLICITATION/CONTRACT FORM The required response date/time has changed from 26-Apr-2017 05:00 PM to 22-May-2017 05:00 PM. SECTION C - DESCRIPTIONS AND SPECIFICATIONS The following have been added by full text: DELIVERIES AND RESPONSIBILITIE 2.0 DELIVERIES, RESPONSIBILITIES, AND QUALIFICATIONS 2.1 The contractor shall deliver full medical gas cylinders to designated location in accordance with each delivery order. 2.2 The contractor shall pick up and return to the contractor's site, empty medical gas cylinders from designated location, to be refilled by contractor. 2.3 All size "E" cylinders must be aluminum cylinders. 2.4 The contractor shall make regularly scheduled routine deliveries/returns to take place on every Monday and Thursday. If delivery day falls on a holiday, delivery will take place on the next business day. 2.5 The contractor shall provide emergency deliveries, as specified on a delivery order which will be faxed to the contractor, within two (2) hours during normal working hours (8:00 AM - 4:30 PM Monday-Friday). 2.6 The contractor shall provide driver(s) to pick-up and deliver medical gas cylinders. The driver(s) must have a valid driver's license and is/are required to comply with government driving regulations while on government facilities. 2.7 The contractor shall use vehicles used for transporting the medical gas must meet Department of Transportation regulations. 2.8 The contractor shall provide at a minimum cylinders in quantitites listed in Section B that identify the requirements for the initial month of service. 2.9 Unless otherwise specifically provided for in this contract the quality of services rendered hereunder shall conform to the highest standards in the relevant profession, trade, or field of endeavor. All services will be rendered by Board Certified Professionals, or certified in their field or endeavor and approved by the FDA and OSHA. 2.10 The contractor shall provide an accurate invoice, with copies of signed receipt included, of all medical gases delivered/returned when submitting for payment. When delivering gases, contractor must provide a copy of signed receipt from all delivery sites to Bldg. #2. 2.11 The contractor shall affix labels on the medical gas cylinders stating the type of gas inside. 2.12 The contractor shall be, without expense to the government, responsible for obtaining any necessary licenses and permits for complying with all Federal, State, and Municipal laws, codes and regulations in connection with the performance of services required by the contract. 2.13 The contractor must be physically located within one driving hour, not to exceed 60 miles (based on standard workday driving conditions) of Portsmouth Naval Medical Center. 2.14 The contractor shall fill Oxygen locally. 2.15 The contractor shall fill all miscellaneous and specialty gases other then locally with a 2 week lapse for filling. The following have been deleted: PERFORMANCE WORK STATEMENT SECTION I - CONTRACT CLAUSES The following have been added by reference: 52.232-11 52.253-1 SECTION K - REPRESENTATIONS, CERTIFICATIONS AND OTHER STATEMENTS OF OFFERORS The following have been added by full text: 52.222-18 CERTIFICATION REGARDING KNOWLEDGE OF CHILD LABOR FOR LISTED END PRODUCTS (FEBRUARY 2001) (a) Definition. Forced or indentured child labor means all work or service-- (1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or (2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties. (b) Listed end products. The following end product(s) being acquired under this solicitation is (are) included in the List of Products Requiring Contractor Certification as to Forced or Indentured Child Labor, identified by their country of origin. There is a reasonable basis to believe that listed endproducts from the listed countries of origin may have been mined, produced, or manufactured by forced or indentured child labor. ( ) (2) The offeror may supply an end product listed in paragraph (b) of this provision that was mined, produced, or manufactured in the corresponding country as listed for that product. The offeror certifies that it has made a good faith effort to determine whether forced or indentured child labor was used to mine, produce, or manufacture such end product. On the basis of those efforts, the offeror certifies that it is not aware of any such use of child labor. (End of provision) (End of Summary of Changes)
