- REQUEST FOR INFORMATION Purpose and Objectives: The intent of this Sources Sought Notice is to identify any business size that can potentially provide the items listed below. This includes all small businesses, Service Disabled (SDVOSB) and Veteran Owned (VOSB) as well as any business size capable of providing CB Simulation Center AV System. These items are for the Department of Veteran Affairs, PITTSBURGH VA Medical Center, UNIVERSITY DRIVE, PITTSBURGH, PA. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Business name, address, Point of Contact, Business size, Duns Number. All information is to be submitted via e-mail to amanda.saunders@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by Noon EST (EST) on Monday, August 14, 2017. No submissions will be accepted after this date and time. Questions can be submitted electronically to amanda.saunders@va.gov This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does NOT constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Performance Work Statement UD CB Simulation Center A/V Contract Title. CB Simulation Center AV System Background. The CAREs major construction project building the Consolidation Building left shell space to build a medical simulation training center. This project will provide all of the audio visual needs for the entire project. This project was terminated and not completed, this contract is for completion of the Simulation Center Audio and Visual package. Scope. The VA Hospital in Pittsburgh is building a Simulation (Sim) Center in the Consolidation Building on the VAPHS University Drive Division. The plan includes state of the art Simulation Center that has an OR, ICU, specialized, patient and basic procedure rooms. Unparalleled recording and review capability, simulator integration with the existing Learning Exchange (LEX), and ease of use/operation; with future expansion capabilities as well. The audiovisual systems of this project will be implemented using the design/install method providing complete design and a turnkey Audio/Visual System, performing all functions as described herein. The following is a minimum and not all inclusive of what is needed for entire system, which is the responsibility of the Contractor to design and install a complete system. Specific Tasks. A. AV Contractor will design, deliver and install the following capabilities: † The remaining installation/configuration of the existing rack mounted AMX 64x64 digital matrix switcher will be performed. This will allow for all sources within the Simulation Center to be routed to any display/device. † The Team Training 1 simulation/classroom rooms will have (1) fixed and (4) PTZ cameras per user specifications. † The Team Training 2 simulation/classroom rooms will have (1) fixed and (3) PTZ cameras per user specifications. † The two conference/classrooms rooms will have (2) PTZ cameras with video and audioconferencing capabilities. † The hallways will have two (2) PTZ cameras in each hallway. † The two large simulation rooms will have (1) 80 display and (1) 70 display. † The three small simulation rooms will have (1) 70 display in each room. † The two conference rooms will have (1) 70 display in each room. † The four Sim Lab Control locations will each have a 32 display above each control panel. † A high definition video system will be installed in both Classroom 1 and Classroom 2. † The customer will supply the Simulators/Trainers and all associated hardware/software. † The remaining installation of the B-Line simulation software solution will be performed provided the VA receives required authorization from their OIT Division regarding network approval. † The remaining installation of the (4) B-Line Sim-Capture devices with SDI video will be installed in the equipment rack provided the VA receives required authorization from their OIT Division regarding network approval. † VAPHS hereby acknowledges and accepts that the configuration of loading Windows 7 on the SDI form factor is not a fully supported SimCapture implementation. This custom configuration may limit the capabilities and functionality of the SDI SimCapture. If issues arise where the cause cannot be determined by reasonable technical diligence by B-Line Medical, the VAPHS accepts the Program Error until it is able to upgrade to an operating system fully supported by B-Line Medical † A Polycom capture/streaming unit will be installed for the Simulation Center provided the VA receives required authorization from their OIT Division regarding network approval. † A table box with HDMI,VGA with audio, USB for keyboard mouse, (4) data and (2) power receptacles will be installed in the table in both Classroom 1 and Classroom 2 † One 34TB server will be provided for capturing of events. This server will also be used for Management of the B-Line simulation software † Nine (9) AMX 10 touch panels will be installed. The panels will be: (4) in the Sim Labs, (1) each in Conference Rooms 2010 and 2005, (1) in Team Training 2006, (1) in Office 2002, and (1) in hallway near Reception † Four AMX 19 touch panels will be installed for master control. The panels will be: (4) in the Sim Labs Control Area. These panels will be able to control any functionality in the Sim Labs. † Eleven Polycom Sound Structure audio processors will be installed in the main rack. The units will provide connection for the microphones in all of the rooms. A telephone interface will be installed for the Sound Structure on a room by room basis. † Polycom HDX Ceiling microphones, using cable retractors, will be installed in each room. A total of (38) microphones will be used. † Seven wireless lapel microphones will be installed. † Nine each audio amplifiers will be installed in the main rack to support the audio. † A total of (36) JBL speakers will be installed in the rooms. † Three each wall plates will be installed by contractor in the four (4) Simulation Labs/Rooms to feed video from the room to the master switcher. † One transmitter will be installed by contractor in each of the (2) Conference Rooms to feed video from the room to the master switcher. † The installation of cabling and the equipment mounting will be performed by contractor. † Onsite training for up to (5) participants have been quoted. The training will be in four sessions of up to (4) hours each. The training will be delivered by the contractor. A User Guide will be included. † All Control system devices will be put on a private Network and switch not connected to the VA infrastructure for stability of the Audio/Video switching and for ease of maintenance not involving the OIT Department of the VA. This control network will be done by contractor and be autonomous from the VA network. Additional installation work to be performed by contractor: † The cabling and hardware mounting will be provided and installed. † The speakers will be installed. † The ceiling microphones and retractors will be installed and wired. † Adequate shelving, access holes and ventilation must be provided in the instance of a credenza. † Provide and install table boxes and any cutouts in furniture required for mounting of interfaces etc. † All installation work is quoted to be completed during normal business hours. † Contractor is responsible for all trash removal. B. Control / Operation / Software The systems interface and the actual control should be user friendly. Provide single point of access management solution that is accessible from any authenticated computer within the Simulation center and LEX. Must be B-Line Medical SimCapture software or equal. Software system to provide unparalleled recording and review capability, annotation, simulator integration and ease of use. Ability for simulation center to connect seamlessly using their VA existing user name and password to log in, and automatically kept current. C. Furnishings / Other The system should include all equipment racks and lecterns/credenzas to house all equipment either portable or fixed as required by the function of the system. All cable and installation materials are to be provided including microphone cables, laptop interface cables, rigging and mounting hardware to provide a complete turn-key system. Provisions for keeping equipment cool in closed-in cabinets/racks. Performance Monitorin COR will monitor the project by routine inspections of the work and the product submittals. Security Requirements Contractor is required to check in with VA Police on the first floor and sign in daily to receive a temporary badge to work onsite. Delivery Schedule. Project has a 120 day duration starting on award of purchase order. This includes ordering of parts and any long lead items. Warranty The contractor shall make known, in writing, at time they submit their Quote/Proposal of any exceptions that might exist between conditions described herein and contractor s policy of warranty. After award of contract, all conditions and requirements of warranty described herein shall apply. The contractor shall guarantee all equipment, materials, and labor for a period of 1 year from the date of final acceptance. During the warranty period, within 24 hours of notification, the contractor shall answer all service calls and requests for information. During the warranty period, within 48 hours of original notification, the contractor shall provide emergency service to restore operation of the system, replacing defective materials, repairing faulty workmanship, making temporary repairs, and providing loaner equipment as necessary, all at no charge. The contractor shall notify the owner after any service call whether such call is or is not covered under warranty. The Owner may be billed for non-warranty calls. The contactor shall notify the owner of any service call or work to be performed for which charges may be incurred before such work commences. Improper functioning, for warranty purposes, means failure of the system to meet the intentions of the RFQ because of internal defects. It does not include Owner caused malfunctions such as re-adjustment of the controls, re-tuning of the system, or injury to the system beyond normal wear. Nor does the warranty cover paint, exterior finishes, fuses, lamps (including projection lamps) or associated labor, unless the damage or failure results from defective materials or workmanship covered by the warranty. The contractor shall take such actions at the time of installation to insure that all equipment is installed in accordance with the manufacturer recommended environmental and electrical operating conditions and requirements. After installation, the contractor shall be responsible for the repair or replacement of said equipment that the Contractor installs which fails due to environmental or electrical conditions, even if not covered by the manufacturer s warranty. The Contractor shall not be held responsible for damages due to changes in environmental conditions, which occur after system acceptance. Unless otherwise directed, the Contractor shall activate all manufacturer warranties in the Owner s name. The start date of the warranties shall be the date of final acceptance. If the Contractor has modified certain components, the manufacturer warranty may be void. In this case, the Contractor is responsible for providing warranty coverage equal to that of the manufacturer. Certain subsystems and system components may require installation by authorized representatives in order for the complete manufacturer warranty to apply. If this pertains to any subsystem or component for this project, it is the Contractor s responsibility to make arrangements for the complete manufacturer warranty to apply. These arrangements are to be at no additional cost to the Owner. Deliverables. Documentation Upon completion of the work, the Contractor shall condense all infrastructure drawings, signal flow drawings, equipment manuals and Shop Drawings into a single As Built document set using AutoCAD 2012. The Contractor shall submit two copies to the Owner and one copy to the Consultant. The final submittal is to be via CD-ROM and hard copy. Drawings submitted via CD-ROM are also to be provided as full size hard copy drawings. All documentation, including drawings, binder cover, binder spine, CD label and so on, shall list the Owner, Consultant, Contractor, revision number and date as applicable. Each As-Built document set shall be comprised, a minimum of: A listing of each supplied item with manufacturer, model number and serial number. Operator's manuals for each piece of equipment supplied by the Contractor Two Training Manuals for system operation. This training manual must also be supplied electronically on a CD and labeld. A complete set of as-built drawings. The drawings shall be D-size, folded and inserted into plastic sleeves within a binder. Software Documentation Where custom software is developed as part of this project, the system source code and any associated files, reference files, and development software (and related documentation and licenses) used to compile, develop, and build, etc. the executable code or source code must be provided to the Owner. The source code should be well documented in accordance with industry software engineering practices. The software developer shall retain intellectual property rights. The Owner shall have a license for perpetuity for use as it applies solely to this project, including the right to modify/enhance. The Consultant shall have no rights associated with the software, other than to test or review on behalf of the Owner. The software code may not be sold or used, in part or in whole, in any other project or application other than that intended by this RFP, in part or in whole, by the Owner or any other party. If a Subcontractor is used to write the software, the Contractor shall include, as part of the As-Built documentation submittal, a signed letter on Subcontractor letterhead, granting the Owner ownership, use, and modification rights of the code and documentation as defined herein. Training The Contractor shall provide a total of twelve (12) hours of on-site training for the Owner s staff at a time that is mutually agreeable for the Owner and Contractor. The Contractor should anticipate three sessions of approximately four (4) hours each. The Contractor should anticipate attending at least two special functions to be determined by the owner and providing technical assistance during the event. System Acceptance Upon successful completion of Final Tests, Documentation and Training, the Contractor shall notify the Owner and Consultant, in writing, that the system is complete. The Owner shall have fifteen (15) days to generate a punch list of omissions, adjustments, corrections and the like and respond in writing to the Contractor. In the absence of such a Punch List, the system shall be considered to be complete. The warranty shall commence on the fifteenth day after the Contractor s notification of completion of work, and the Owner shall process final payment. In the event that further work is required to complete this project, the Contractor shall be prepared to continue work, without additional compensation, until the system is accepted.
