This is a REQUEST FOR INFORMATION only. Market research is being conducted to determine the amount of qualified and capable businesses that can provide the item(s) requested below. If you believe your firm is able to provide the items requested below, you are encouraged to respond by the due date with all of the required information indicated below in order to be considered. The vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment-no grey market items. All items must be new. Due date for responses is Wednesday, August 16, 2017 at noon local time (CST). Email responses to Melanie Stockman, at melanie.stockman@va.gov. All questions must be submitted in writing to the Contracting Officer via email. Telephone inquiries will not be accepted/returned. Provide evidence of your authority to distribute the proposed equipment/system. NAICS: 334510 Size Standard: 1250 employees Description/Salient Characteristics OPMI Lumera 700 Surgical Microscope Subject: New OPMI LUMERA 700 Surgical Microscope forophthalmic surgical cases. Vendor will provide a service manual that includes theory of operation, maintenance requirements, repair information, schematics, part lists and troubleshooting information in electronic format for use by the VA Biomedical Engineering Department. Date of manufacture must not be greater than 2 years before the purchase order date.   The equipment must not be used or refurbished and must have the latest manufacturer s software/hardware updates installed if applicable.     Any batteries provided with the unit will not be manufactured more than 12 months before the purchase order issue date. Deliver to Operating Room Manager, Omaha VA Medical Center, 4101 Woolworth Ave, Omaha Nebraska 68105, ASAP after award. Additional items required in the offeror s response: Indicate socio-economic status Include picture of offered item & support literature, brochure etc Indicate any existing Government contract where item may be purchased (i.e. FSS contract etc) No grey market items will be accepted.
