Description
COMBINED SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for PHLEBOTOMY CHAIRS, BRAND NAME OR EQUAL, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is VA244-17-Q-1555 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 339113 code has a small business size standard of 1000. (v) Contract Line Items (CLIN): ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Powered Reclining Blood Draw Chair 15995-139 1.00 EA __________________ __________________ 0002 Powered Extra-Wide Blood Draw Chair w/ Flip Arm 15983-139 3.00 EA __________________ __________________ GRAND TOTAL __________________ (vi) Comparable products must be brand name or equal in the following specifications: Statement of Need Lebanon VA Medical Center Title of Project: Power Blood Draw Chairs Scope of Work: The vendor shall provide all resources necessary to accomplish the deliverables described in this statement of need (SON), except as may otherwise be specified. Contractor shall provide (3) Brand-Name-or-Equal MarketLab Extra-Wide Power Blood Draw Chairs and (1) Brand-Name-or-Equal MarketLab Reclining Power Blood Draw Chair deliver within 30-days from the date of notice of contract award to the Lebanon Veterans Affairs Medical Center for activation of new Camp Hill VA Clinic. Performance Period: Delivery shall occur within 30 days of receipt of contract Type of Contract: Firm Fixed Price Delivery Order. General Requirement The Lebanon VA Medical Center (LVAMC) Camp Hill new VA Clinic has a need for three Brand-Name- or-Equal MarketLab Extra-Wide Power Blood Draw Chairs and one Brand-Name-or-Equal MarketLab Reclining Power Blood Draw Chair for activation. These chairs will improve blood drawing by lifting the patient to the exact height of the staff member drawing the blood. This will improve the way blood is drawn in the lab by raising or lowering the patient to the exact height. The Reclining chair offers safety for patients with a history of blacking out from blood drawing procedures. Salient Features for Extra-Wide Power Blood Draw Chairs: Seat wide enough to sit a bariatric patient Color: Warm Gray Adjust the height at which the patient is sitting effortlessly Features an electronic lift mechanism for raising any and all patients to a height that best suits the staff during a routine blood draw Spacious width and strengthened lift 110-volt lift mechanism enclosed in smart locking, brushed aluminum casing, raises patients to optimum height for blood draw Single mechanism chair has a load capacity of 375 lbs 5 year limited warranty Salient Features for Reclining Power Blood Draw Chair: Power adjustable chair offers several ways to keep a patient comfortable during a blood draw Fully postured, seated position completely reclined, raise or lower the padded adjustable footrest to offer another option of comfort Color: Warm Gray Raise or lower the entire chair to the height that work best for the staff without altering the set comfort position Multiple ways to keep patient relaxed Better accommodates patients who may faint during procedure Hand controller 110-volt lift mechanism enclosed in smart locking, brushed aluminum casing, raises patients to optimum height for blood draw Smooth electric 30 degree recline Chair length range is 35 when upright to 66 when fully laid back Seat height adjustment range is 22 -30 Single mechanism chair has a load capacity of 375 lbs Powered Multi-Function Draw Chair-20 W and 28 D Delivery: Contractor Shall provide delivery of Blood Draw Chairs within 30 days of contract award to the Lebanon VA Medical Center located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn: Warehouse. The Engineer Department will need to perform a safety check after installed in the floor at the Camp Hill VA Clinic and enter the equipment into the system for annual Preventative Maintenance (PM) Checks. Delivery to the facility shall be coordinated at least 72 hours prior to required delivery date with the Warehouse Supervisor.
