COMBINED SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for EXAM CHAIR, BRAND NAME OR EQUAL, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is VA244-17-Q-1544 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 339112 code has a small business size standard of 1000. (v) Contract Line Items (CLIN): ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Reliance 7000-LFC Chair #20 Pearl (Upholstery Color Required) Trade Marco Chair $750.00 Free Installation With Trade 1.00 EA __________________ __________________ GRAND TOTAL __________________ (vi) Comparable products must be brand name or equal in the following specifications: RELIANCE 7000-LFC EXAM CHAIR- Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for one (1) Reliance 7000-LFC Exam Chair to be utilized in a clinic setting. Scope of Work: The Exam Chair is replacing a very old chair that is beyond its life expectancy. This chair provides the proper positioning to conduct a thorough eye exam on patients at the medical center. Salient Characteristics of the Reliance 7000 Exam Chair: Dual Side Operation can control side from both sides. Adjustable concave headrest with single hand locking capability. Upholstered armrests which lock in place and release on both chair sides. Large hinged footrest which supports patients weight, and tilts out of way. Reclines nearly flat for procedures. Power hydraulic square base which provides patient stability. Finger and corded footswitch raise, lower or recline chair without breaking patient contact. Passes CSA testing and is certified in safety and durability in CSA, C/US certification of compliance. Technical Specifications: Base: 22 5/16 Overall Width 24 ½ Overall Depth (excluding headrest) 36 Table Position: 60 Seat- 19 wide X 18 ½ deep Armrest distance: 19 Base height choice Shipping Weight 325lbs Electrical 115v, 60/50 Hz, 6.3 amperes total. Options available. Additional Information: The Reliance Model is what is utilized in our clinic and is found to be reliable, sturdy, and durable. Delivery: Delivery shall be within thirty (30) days from the time of award. The Exam Chair shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment in to the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through Bio-Med and the Chief, Optometry.
