Description

COMBINED SYNOPSIS/SOLICITATION GENERAL INFORMATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation in its entirety; quotes are being requested and a written solicitation will not be issued. This procurement shall be conducted under FAR Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures, and Veteran Affairs Acquisition Regulations (VAAR) as supplemented with additional information in this notice. The award shall be based on the Quoter s ability to provide the specific supplies and services stated in the solicitation. Pursuant to FAR Part 13.303, Blanket Purchase Agreements (BPA), it is the intent of the Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN) 22 to establish a Single Award BPA, for the Cost Per Test (CPT) requirements described within this RFQ solicitation. Solicitation number VA262-17-Q-1530 is issued as a Brand Name or Equal Request for Quotation (RFQ). This RFQ is set-aside for small businesses. The selected NAICS code for this requirement is 334516, and size standard 1,000 employees. The selected Federal Supply Code for this requirement is 6630. Responses to this RFQ shall be due by Friday, August 25, 2017 at 4:00 PM PST. Quotes shall be submitted electronically via email to Ralph Fontela, Contracting Specialist at raphael.fontela@va.gov. Quoters are advised to review and monitor the FBO website for the most up to date information, including all amendments. Correspondence or questions may be directed to Ralph Fontela at raphael.fontela@va.gov, with VA262-17-Q-1530 in the subject line, within forty-eight (48) hours of the posting of this solicitation.
