Page 1 of 6 NOTICE OF INTENT TO AWARD SOLE SOURCE: Department of Veterans Affairs (VA) Network Contracting Office 2 (NCO 2) intends to award a sole source contract award to General Electric Medical Systems (GE), 3000 N. Grandview Blvd., Waukesha, WI 53188-1616. The period of Performance is from October 1, 2017 to September 30, 2018 with a provision of four (4) option years. The Canandaigua VA Medical Center (VAMC), 400 Fort Hill Ave, Canandaigua, NY 14424 and the Rochester Outpatient Clinic (ROPC), 465 Westfall Road, Rochester, NY 14620 require the service contract for maintenance and repair of GE Diagnostic Imaging Equipment for fiscal year 2018 (FY18). The Canandaigua VAMC medical devices include the GE XR Discovery XR650 2-D (XRA606), GE XR Analog R&F (Example Legacy, XRF107) and GE XR Precision 500D (XRF118). The ROPC medical devices include two GE XR FlashPad Digital Wireless Detectors (XRA614), and the GE XR Discovery XR656 2-D (XRA612). The service maintenance plan will include scheduled preventive maintenance replacement parts coverage, remote service support, telephone support, operating system software and hardware reliability reports, as well as, continuing education and support service. The contractor will be responsible for full maintenance coverage by providing all replacement parts, labor, travel expenses, tools, test equipment and other incidentals necessary to maintain or restore the listed equipment to operating specifications according to the manufacturer s recommendations throughout the term of the contract. Due to the proprietary nature, GE is the only company than can access its systems remotely and offer certified parts that meet the original equipment manufacturer s specifications. GE technicians are the only OEM trained and certified personnel authorized to perform work on this HTME (High Tech Medical Equipment). THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE QUOTES; however, any firm that believes it can meet the requirements may give written notification prior to the response due date and time. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to perform the requirements. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Government will proceed with the award. Responses are due by August 18, 2017 by 2:00 p.m. (EST). Responses shall be submitted via e-mail to Janine.Childs@va.gov. Telephone requests or inquiries will not be accepted. Statement of Work Engineering Department General Electric (GE) Medical Solutions Service Contract Background The Canandaigua VA Medical Center (VAMC) and the Rochester Outpatient Clinic (ROPC) require the service contract for maintenance and repair of GE Diagnostic Imaging Equipment for fiscal year 2018 (FY18). The Canandaigua VAMC medical devices include the GE XR Discovery XR650 2-D (XRA606), GE XR Analog R&F (Example Legacy, XRF107) and GE XR Precision 500D (XRF118). The ROPC medical devices include two GE XR FlashPad Digital Wireless Detectors (XRA614) and the GE XR Discovery XR656 2-D (XRA612). These medical devices are used for diagnostic patient care at both the Canandaigua VAMC and the ROPC. Previously, the GE imaging equipment was under two separate service contracts, which expire on September 30th, 2017. Objectives The objective is for the Canandaigua VAMC and the ROPC to improve patient care and diagnosis by having a service agreement on the GE medical equipment for Imaging and OR services. The AssurePoint Rapid service contracts for the one GE XR Discovery XR650 2-D (XRA606) will be used for the principle coverage period of Monday Friday 8AM 9PM, FE Onsite Response time 2 hours, InSite/Tech Phone Support, PM Coverage: Monday Friday 8AM - 9PM, Repair Parts: Included, next day 10:30am LST-GDXR, Software and Quality Updates, Special Parts Handling Fee: No charge for Hard Down, TiP Answer Line, Tip-Ed Online (TV) Subscription, TVA on Demand, and Uptime Commitment: 98%. Included in Service is the Detector: 2-Fixed Detectors, Covered is iLinq Response time of 5 minutes, Table, and tube coverage; excluded are Peripheral devices, printers, tethered detector, uninterrupted power supply and workstation. The AssurePoint Rapid Service will be for one (1) fiscal year (October 1st, 2017 September 30th, 2018, to include four (4) option years) to reduce service costs and improve equipment turn-around time and reliability. The AssurePoint Standard service contracts for the one GE XR Analog R&F (Example Legacy (XRF107) will be used for the principle coverage period of Monday Friday 8AM 5PM, FE Onsite Response time 4 hours, iCenter, InSite Response: 30 minutes, InSite/Tech Phone Support, PM Coverage: Monday Friday 8AM - 5PM, Repair Parts: Included, next day 10:30am LST-GDXR, Software and Quality Updates, TiP Answer Line, and Uptime Commitment: 97%. Included in Service is the Digital, Image Intensifier, Tomography, and tube coverage; excluded are Peripheral devices, printers, uninterrupted power supply, VCR and workstation. The AssurePoint Rapid Service will be for one (1) fiscal year (October 1st, 2017 September 30th, 2018, to include four (4) option years) to reduce service costs and improve equipment turn-around time and reliability. The AssurePoint Standard service contracts for the one GE XR Precision 500D (XRF118) will be used for the principle coverage period of Monday Friday 8AM 5PM, FE Onsite Response time 4 hours, iCenter, InSite Response: 30 minutes, InSite/Tech Phone Support, PM Coverage: Monday Friday 8AM - 5PM, Repair Parts: Included, next day 10:30am LST-GDXR, Software and Quality Updates, TiP Answer Line, and Uptime Commitment: 97%. Included in Service is the Digital, iLinq Response Time: 30 minutes, Image Intensifier, and tube coverage; excluded are Analog and Digital Upgrade Kit, Peripheral devices, printers, uninterrupted power supply, VCR, Wireless Detector and workstation. The AssurePoint Rapid Service will be for one (1) fiscal year (October 1st, 2017 September 30th, 2018, to include four (4) option years) to reduce service costs and improve equipment turn-around time and reliability. The AssurePoint Completed service contracts for the two GE XR Precision 500D (XRF118) will be used for the Account Coverage: 1-replacement/contract year-$0; each additional replacement-$6k, FE Coverage Weekdays Monday-Friday 8AM 9PM, FE Onsite Response time 4 hours, iCenter, InSite/Tech Phone Support, Note 01: Unlimited Replacements Due to Normal Wear & Teae, Parts Shipping: Included, Next Day 10:30 am LST-General, PM Coverage: Monday Friday 8AM - 9PM, Replacements Due to Abuse, Theft, Loss, Fire: Excluded, Software and Quality Updates, TiP Answer Line, and Uptime Commitment: 97%. Included in Service is the Battery Replacement Coverage, Detector and Grid Replacement. The AssurePoint Rapid Service will be for one (1) fiscal year (October 1st, 2017 September 30th, 2018, to include four (4) option years) to reduce service costs and improve equipment turn-around time and reliability. The AssurePoint Rapid service contracts for the GE XR Discovery XR656 2-D (XRA612) will be used for the principle coverage period of Monday Friday 8AM 5PM, FE Onsite Response time 2 hours, iCenter, InSite/Tech Phone Support, PM Coverage: Monday Friday 8AM - 5PM, Repair Parts: Included, next day 10:30am LST-GDXR, Software and Quality Updates, Special Parts Handling Fee: No charge for Hard Down, TiP Answer Line, Tip-Ed Online (TV) Subscription, TVA on Demand, and Uptime Commitment: 98%. Included in Service is the iLinq Response Time of 5 minutes, Table, and tube coverage; excluded are Peripheral devices, printers, uninterrupted power supply, wireless detector and workstation. The AssurePoint Rapid Service will be for one (1) fiscal year (October 1st, 2017 September 30th, 2018, to include four (4) option years) to reduce service costs and improve equipment turn-around time and reliability. General Requirements The contractor must comply with the following general requirements: The Contractor shall comply with Federal, State, and Local Laws, plus any Federal Regulations as applicable to the performance of this contract. Citations to pertinent laws, codes and regulations such as 44 U.S.C Chapter 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under or relating to this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. The Contractor shall not accept any instructions issued by any person employed by the U. S. Government, other than: the Contracting Officer (CO), or the Contracting Officers Representative (COR), all acting within the limits of their authority. The Contractor shall be qualified / authorized by the Original Equipment Manufacturer (OEM) in the repair of all equipment listed within this contract. Contractor shall maintain qualifications throughout entire contract period. Qualification includes but not limited to the certification of all repair persons and repair facilities by the OEM as being trained / qualified to perform required repairs. The Government shall maintain the right to seek proof of qualification prior to award and anytime during contract period of performance. Normal hours of coverage are Monday through Friday from 8:00 a.m. to 5:00 p.m., excluding federal holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or his designee. Federal holidays observed by the Canandaigua VAMC and the ROPC are New Year s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Statement of Work The contractor will provide a one (1) year service with up to four (4) option years on GE XR Discovery XR650 2-D (XRA606), GE XR Analog R&F (Example Legacy, XRF107), GE XR Precision 500D (XRF118), two GE XR FlashPad Digital Wireless Detectors (XRA614) and the GE XR Discovery XR656 2-D (XRA612) which provides all the preventative maintenance described in Section II Objectives above for the VA owned equipment located at the Canandaigua VAMC and the ROPC. This will be from the beginning of fiscal year 2018 (October 1st, 2017) through one (1) year (September 30th, 2018). Option years will include through September 30th, 2022. Emergency response to equipment failure will be covered by this agreement during hours of operation, weekdays (Monday Friday) 8AM 5PM. Remote telephone support shall be available during hours of operation. Response time for emergency repairs within covered hours will be as follows for equipment identified by the following GE System ID numbers (described in section VI Government furnished equipment): 585463RAD1 (2 hours), 585393RAD1, (2 Hours), 585393VRF2(6 hours), 585463GI (6 hours), 585463FP1 (6 hours), 585463FP2 (6 hours) Radiographic tubes will be covered for replacement by the contractor in the event of failure. CT tube is covered within projected use parameters (less than 3000 patients/year) The contractor shall provide a no charge invoice summarizing the repair and maintenance of the GE XR Discovery XR650 2-D (XRA606), GE XR Analog R&F (Example Legacy, XRF107), GE XR Precision 500D (XRF118), two GE XR FlashPad Digital Wireless Detectors (XRA614) and the GE XR Discovery XR656 2-D (XRA612). The contractor must have the skills, training from the OEM and experience to be qualified to repair the radiology equipment. If the contractor is required to visit onsite for a repair or maintenance, he/she will need to notify the COR or other Biomedical Engineering personnel at least 2 hours in advance. The contractor will need to register with VA Police upon arrive to the Canandaigua VAMC and the ROPC.
