Description

Annual Maintenance and repairs of two Medrad Stellant contrast injectors located at the Coatesville VA Coatesville Veterans Affairs Medical Center1400 Blackhorse Hill Road Coatesville, PA 19320 Specific Tasks: This service agreement is for full maintenance and repair services of the Medrad Stellant contrast injectors listed below. The contractor shall provide unlimited emergency repair and call-back repair services as well as preventative maintenance inspections (One annual PM inspection per piece of equipment) to include all parts, labor, travel and all other materials for the equipment listed below. Initial period of performance is to begin October 2, 2017 through October 1, 2018 Equipment The contractor shall provide maintenance, as described above, on the equipment listed below: EE# Serial # Manufacturer Model 26986 28926 Medrad Stellant D TBD TBD-located in MRI Building Medrad Stellant D Send a capability statement, no more than one page in length; to include Company Name, Duns Number, Socioeconomic Category, brief description of your business s capability to handle this particular project in the Coatesville area and contact information.
