- STATEMENT OF WORK Title of Project: Arxium Optifill II Training for staff at James J. Peters VA Medical Center Background: Arxium Optifill II is an automatic medication dispensing device. Any repair/diagnosis on this equipment is costly and has a high turnaround time. This training will allow for decreased down times to ensure patients get their medication timely. Scope of Work: Contractor must provide training to biomedical staff to properly maintain the Automed/Arxium Optifill II Pill Dispenser System (see equipment description below). The contractor shall provide all resources (labor, equipment, tools, materials, preventive maintenance service, and parts) necessary to accomplish this deliverable, except as may otherwise be specified. Equipment Description: Manufacturer: Arxium Device Name: Optifill II ID#: 18772 Model: Optifill II Serial# B00013 Site ID# VABRO Performance Period: Date to be determined. Place of Performance: James J. Peters VA Medical Center 130 West Kingsbridge Road Bronx NY 10468-3904 Contractor Response Time: Two (2) calendar days (24 hours/day) from date of award Type of Contract: Firm Fixed Price Additional Requirements: Contractor shall provide a syllabus that must be approved by the COR before training takes place. Contractor shall provide all necessary training per the original equipment manufacturer s training syllabus, including in-depth training on the diagnosis and repair of the Arxium Optifill II, including the knowledge, skills and abilities required for successful project management, preventive maintenance, common problems, etc. Contractor shall provide for all costs of training, including flight, hotel, transportation and food within this contract. The contractor must respond to the Biomedical Engineering Designee and/or COR with a phone call within [two (2) hours] [24 hours per day], after receipt of telephoned service call notification, by Contracting Officer (CO). Responses to this notice must be submitted in writing via email and must be received not later than Friday, August 18, 2017, 1:00pm EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing, and received not later than Friday, August 18, 2017, 1:00pm EST.
