- THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited. The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following: 1. 10,000 hospital map booklets with 12 pages in each book. The books will have the following exact specifications: o Cover design of the booklet should incorporate a photo of the medical center or a grouping of hospital scenes/activities (will be determined upon contract award) o Color pages printed booklets o Booklet size: 6X8 inches o Booklet pages printed on #80 matte text stock paper 2. One 2-D site map (508 compliance) of the Franklin Community Based Outpatient Clinic (CBOC) 3. One 2-D site map (508 compliance) of the Hickory Community Based Outpatient Clinic (CBOC) 4. One 2-D site map (508 compliance) of the Rutherford County Community Based Outpatient Clinic (CBOC) 5. Interactive map with mobile and web access 6. Yearly maintenance service agreement- required for the interactive map; includes hosting and 10 hours of edits per year. A site visit is encouraged before providing any capability statements to ensure the contractor is aware of the requirements. All responses shall be submitted no later than 4:00 PM EST (Eastern Standard Time) on August 21, 2017. All interested parties should respond, in writing via email to desiree.mills-hicks@va.gov. No telephone inquiries will be accepted. In response, please include the following: Company/individual name Address Point of contact with phone number Information describing your interest Indicate FSS/GSA contract number or Open Market, if applicable Tax ID number DUNS number Capabilities Statement Company Socio-Economic SIZE (example large, small, SDVOSB, VOSB, WOSB etc.) Proof of verification as a SDVOSB/VOSB (if applicable) The Government anticipates this to be a Small Business set-aside however, if sufficient service disabled veteran owned small business (SDVOSB or VOSB) responses are received to establish the basis for setting this action aside for SDVOSB participation, then the action will be advertised as a SDVOSB set aside, if not, then he action will be advertised as unrestricted. All interested parties responding shall be eligible to bid. The NAICS -541430 - Graphic Design Services is applicable to this acquisition; with a size standard of $7.5 million. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award.
