2 679-17-4-614-0023 Statement of Work Director s Office/Innovation Program Building 137/2nd Floor Classroom Audio Visual system Upgrades Background: The Tuscaloosa VA Medical utilizes the 2nd floor classrrom in Building 137 (A2-137) for classes, meeting space various events that include outside presenters, Director s Town Hall Meetings and all-staff trainings. The audiovisual equipment including the projector and drop-screenare utilized on an almost daily basis however, the current technology in the space is outdated and becoming increasingly poorer in quality than when first installed around 10 years ago. There is currently no good sound solution in the space other than an existing sound bar speaker connected to the PC. The current AV rack in the space is an eyesore and the current set-up is often dismantled by employees utilizing the space resulting in porr functioning equipment. Purpose of Project: The purpose of upgrading the Audio Visual system is to bring the existing rack system up to industry standard for audio visual presentations and to enhance the sound quality of the system as well as convert the space into the Innovation Center. The transformation of the Innovation Center is part of a funded grant from the Innovator s Network and will serve as a space for employees to train, collaborate and work on innovation projects at the Tuscaloosa VA Medical Center. It is meant to be a bright, inspiring space that provides all of the tools necessary for teaching, training and collaboration. Statement of Work: Contractor will install a flatpanel television (approximately 70 -80 screen) at the front of the room. The television will contain a tuner and will not be categorized as IT equipment. The flatpanel television will connect to the existing PC in the space similar to how it currently connected to the projector. The contractor shall also install an integrated ceiling audio system that includes a 70v amplifier and enough speakers that will provide sound to the entire room rather than speakers only at the front of the room. The system will allow for an existing Videoconfereing system to integrate as well integration of existing conference phone for full audio sound quality. The contractor will install new speakers that are aesthetically pleasing and that provide adequate sound quality to the space. The new audio visual system should be a turn-key system that is easy to operate by most users with limited audio-visual skills and abilities. Schedule of Deliverables: Period of Performance (2 weeks). The contractor will survey the location prior to install to ensure proper equipment, materials and labor are in place. The contractor will communicate in advance of the install with the point of contact so that the location can be secured. The contractor will ensure the area is clean once all installation is complete. The contractor will provide all training is provided at completion of install and that the users are aware of warranties and technical support that is included. The contractor must provide service or respond to service calls within 3 hours of intial call. Program Office Point of Contact and/or Contracting Officer Representative (COR): Government Furnished Property: The contractor will be utilizing some Government furnished property such as, microphones, amplifiers etc. but will not have access to any VA network systems of patient data. Records Management: Upon completion or termination of the applicable contract(s) or agreement(s), return and/or destroy, at Covered Entity s option, VA information gathered, created, received, or processed during the performance of the contract(s) or agreement(s). No data will be retained by Business Associate, or contractor, subcontractor, or other agent of Business Associate, unless retention is required by law and specifically permitted by Covered Entity. As deemed appropriate by and under the direction of Covered Entity, Business Associate shall provide written assurance that all VA information has been returned to Covered Entity or destroyed by Business Associate. If immediate return or destruction of all data is not possible, Business Associate shall notify Covered Entity and assure that all VA information retained will be safeguarded to prevent unauthorized Uses or Disclosures. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract. Contractor Personnel Security Requirement: The vendor will be on-site at the Tuscaloosa VAMC. They will not access sensitive patient data or any Tuscaloosa VAMC data systems however, VA 10176 (VA Contractor Privacy/Security Training TMS Module) is required for vendors who do not handle PHI. The vendor/contractor is required to provide a copy of the completed course certificate prior to their assignment and arrival on-station. Contractors shall report to the Director s Office upon arrival and shall be issued temporary Identification badges prior to installation in classroom. The taking of videos, recordings and photographs are prohibited while on VA premises. At the completion of the work, all contractors must return their ID badges to Medical Media staff. Delivery and Packaging: The Tuscaloosa VAMC requests delivery of a finished product be installed no later than September 30, 2017.
