Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a purchase order in accordance with FAR Part 13 for using Simplified Acquisition Procedures. The solicitation number is VA247-17-Q-0803. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-95 effective January 13th, 2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 and the Small Business Size standard is 500 employees. This combined/synopsis solicitation is set aside for Veteran Owned Small Businesses (VOSB s) in accordance with VAAR 852.219-11. Only VA verified VOSB s in the VIP at the time of contract award will be considered. All non-VIP verified firms will be considered non-responsive and ineligible for award. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. The Department of Veterans Affairs, Birmingham VA Medical Center, has a requirement for BRAND NAME OR EQUAL to Hill-Rom Progressa Pro 885. Offerors proposing Brand Name must provide an authorized distributor letter from the manufacturer to verify that they are an authorized distributor of the products they are quoting. STATEMENT OF WORK Introduction to Required Specifications Purpose: The purpose of this Statement of Work (SOW) is to outline the technical specifications and responsibilities of the awarded Vendor, in relation to the provision, integration, and implementation of 27 beds with mattress replacement in the Intensive Care Unit (ICU). The Birmingham VAMC must require removal of the existing hospital beds in the aforementioned locations, and installation and implementation of the new replacement beds. The Vendor must provide all necessary tools, equipment/hardware, installation, maintenance, warranty, and training in accordance with this SOW. Equipment: All equipment must be new, state of the art, and not recertified nor refurbished. Industry Standards: Unless otherwise stated, all equipment must be held to current industry standards for such equipment as present within the current market for hospital beds, supplies, related services, and the most recent International Standards set by the IEC (IEC 60601-2-52). Definitions ICU Bed: A bed specially designed for critically ill patients or others in need of some form of health care in ICU. These beds have special features both for the comfort and well-being of the patient and for the convenience and safety of health care workers. Common features include adjustable height for the entire bed, the head, and the feet, adjustable side rails, and electronic buttons to operate both the bed and other nearby electronic devices. Hospital ICU Mattress: A mattress specially designed for ICU hospitalized. These mattresses have special features both for the comfort and well-being of the patient and for the convenience of health care workers. Common features include a therapeutic surface, pressure redistribution, shear management, moisture management, deep zipper flap, waterproof seams, and delamination protection, has nonskid bottom and or straps to secure to bed frame, ability to support 500 lbs. patient weight. Ability to use for High risk up to Stage IV pressure ulcers. These mattresses are for medical surgical care appropriate for the use in long term and acute areas. Hardware/Software ICU Bed This this solicitation is for BRAND NAME OR EQUAL TO Hill-ROM Progressa Pro885 Must support Patient weight up to 500 lbs. Must support a minimum of 550 lbs. working load Must have integrated motorized drive system Must have integrated patient scales for patient weights with no more than 1% scale accuracy of the patient weight Must have two upright oxygen tank holder/s and pivoting corner mounted frames to ensure fit into elevators Must have integrated electronic bed lengthening ability up to 10 inches with no separate parts installation requirements Must be able to convert to a sitting position Must be able to convert to Trendelenburg position Must provide both chair & side egress Must have head angle indicator Must have a 30/45 Degree Head of Bed Alarm Must have clearance to accommodate a C-Arm Must have Full Pulmonary Surface with X-Ray Sleeve Must have a full functionality battery backup with status indicator Must have brake alarm Must have automatic bed exit alarm silence reset Must operate with standard 120 volt power outlet Must have straight power cord Must have hands-free CPR release Must have hands-free foot controls Must have minimum of two drainage bag hooks Must have dual head end permanent IV pole Must have right and left integrated IV poles and traction equipment sockets Must have IV pole and traction equipment socket Must have line management Must have technology to prevent patient migration from head to foot end of bed Must have nurse control on footboard and side rails Must have a care giver touch screen Must have audible bed exit alarm with minimum 3 zone sensitivity Must have an automatic bed exit alarm reset function for fall prevention, in the event that a staff member forgets to reset Must have automatic bed extension feature Must have side rails with inner controls for the patient, and outer controls for the staff Must have one handed side rail release Must have frame integrated auxiliary outlet/s Must have a frame integrated night light Maximum bed length of bed (to fit into elevators and through acute care patient room doorways) (Maximum length of 91 inches & Maximum width of 42 inches including bumpers, rails, & any protrusions) Must Integrate with current Birmingham VAMC nurse call system Novus-Focus Care. Current patient stations SPS 2000/CE Must have minimum of 4 restraint secure points (2 on each side). Life expectancy of beds must meet or exceed 10 years with vendor guarantee of parts availability for the duration. Must have flat mattress deck for compatibility with standard hospital bed and specialty mattresses Must have a minimum clearance height of 6 (below the bed) to allow access for under-bed tables and floor based lift equipment Must have electronic full upright chair patient positioning, allowing for egress from the foot end of the bed. Allow for full upright seated position. Upper torso nearing 90 degree angle. 90 degree flexion at the knees bilateral Foot bed egress allowing patient to move from the upright seated position with lower legs extending downward towards the floor to progress to standing utilizing the integration of patient lift equipment. Full upright seated position to allow patient to be oved into a seated position by electric bed positioning configurations. Bed must reconfigure into an upright chair with lower legs extending downward toward the floor. Bed foot board must transition to near horizontal eliminating the need to remove the foot board. Chair egress with full chair feature Articulation Specifications: Head 0 °-67 ° Thigh 0 °-30 ° Foot 0 °-70 ° Trendelenburg 13 ° Tilt Table Function (Reverse Trendelenburg) -18 ° The hospital mattress must meet or exceed all technical specifications listed below: Must support patient weight up to 500 lbs. Must have a therapeutic surface to assist in management of pressure, shear, and microclimate for the patient. Must have ability to redistribute pressure through buckling and absorbing patient weight and allow immersion and envelopment to maintain comfort in all positions on the mattress as well as preventing developing pressure ulcers Must have full pulmonary surface Must have a breathable cover that wicks away moisture, equalizes pressure redistribution while balancing the microclimate of the patient s skin Must not conduct heat Must have MAX inflate mode for ease in repositioning with automatic return to prior settings after maximum of 30 minutes Must be able to achieve full rotation with HOB at 30 degree position Must have a maximum mattress thickness of 10 for easy ingress and egress of patients Must allow for a side rail height of 16 for maximum patient safety (from mattress deck to the top of the rail). Must have a nonskid bottom surface or straps to secure to bed frame. Must fit selected hospital bed frame Must have firm edges to prevent collapsing for sitting on side of bed and support weight during ingress and egress. Must have ability to view therapy history log Must have adjustable firmness settings Must have ease of use integrated interface for the end user Must have lock out option Must have delamination protection moisture barrier preventing moisture entrapment Must waterproof seams Must have a deep zipper flap Must be latex free Must have turn assist feature to aid with positioning of patients Must have a 40 degree full body turn capability for assistance with early mobility and prevention of pulmonary complications Must have sensor technology for automatic adjustment of pressure for patient weight, height, position changes, and bed frame articulation changes Must meet US flammability standards: 16CFR1632, 16CFR1633, CAL TB129, Boston BFD IX-11 Must have removable and replaceable inner mattress air bladder Must have bleach tolerant cover for cleaning Vendor Must guarantee of availability of replacement mattress for the period of 10 years Installation Considerations Vendor must provide all materials, labor, and travel needed for installation of the beds. Vendor will work during normal business hours or as directed by the facility upon award. Services must include removal and disposal of all beds needing replacement. Vendor must comply with all VA mandated and local permitted/safety requirements. Support Features Installation/Site Preparation: Site preparation specifications must be furnished in writing by the Vendor as a Site Preparation Report after award of the delivery/task order. The Vendor must provide coordinated professional implementation and project management services. The Vendor must provide a firm timeline and schedule from date of delivery to go-live. The initial installation site report must be provided to the Contracting Officer no later than 1 week after the award. Bed Warranty: The vendor must state the warranty terms when responding to this procurement. Warranty must be, at a minimum, 1 year parts & labor. Travel is the vendor responsibility during the warranty period. This warranty will cover the entirety of the bed and all its components and accessories. Extended warranty or general terms of a service contract will be discussed between the Vendor and this facility. Mattress Warranty: The vendor must state the warranty terms when responding to this procurement. Warranty must be, at a minimum, 1 year parts & labor. Travel is the vendor responsibility during the warranty period. This warranty will cover the entirety of the mattress and all its components and accessories. Extended warranty or general terms of a service contract will be discussed between the Vendor and this facility. User and Service Manuals: The Vendor must provide to each station at no charge, two (2) completed and unabridged printed copies and one (1) electronic version (CD) of operator manuals, service manuals, electronics schematics, troubleshooting guides, and diagnostic software and tools, and parts lists for each piece of equipment purchased to the Biomedical Engineering department with delivery of equipment. Additionally any upgrades to these documents must be provided by the Vendor free of charge. These manuals must include all components and subassemblies, including those not manufacturer by the Vendor. These manuals and documentation must be identical to the ones supplied to the manufacturer s service representatives, and must contain the diagnostics codes, commands, and passwords utilized in maintenance, repair, and calibration of the equipment and provided for the life of the system at no additional cost. Licensing Software updates: The Vendor must provide at no charge, for the lifetime of the device, all routine software updates. All applications software licenses are included in the purchase of the equipment and must not require a renewal charge for the period of time the equipment is in the use in the facility. Clinical Training: The Vendor must provide a training program that is coordinated with and timely to the equipment installation, sufficient to the size and scope of the facility s services. Training must include initial setup and user-training; onsite training for go-live support, and follow-up training upon request. Training will include ongoing customer support after the implementation of the equipment. Biomedical Engineering Training: The Vendor must provide training for two (2) Biomedical Equipment Support Specialists (Hereafter referred as BESS) during initial setup and operations of the equipment, as well as follow-up training, during everyday operation. The Vendor must provide 24/7 technical customer support service for the duration of the equipment at the facility. The on-site training, following installation completion, must include product nomenclature, printed circuit board assembly and subassembly identification and location, interpretation and usage of built-in system diagnostics, and the use of the troubleshooting manual. The Vendor must also provide service seminars at the Vendor s facility tuition-free while the hospital owns the equipment. Support/ Maintenance Agreements 2.1.1 Support/Maintenance Agreements: Vendor must provide service/maintenance agreement for 8am-6pm PST coverage and provide 24/7 technical and clinical phone support. Period of Performance 3.1.1 The vendor MUST estimate equipment time of delivery when bidding on this procurement ID Badges/ Parking/ Smoking Policy CONTRACTOR SIGN-IN Contractor personnel shall report to PIV Office, before visiting the facilities, to sign-in and receive a contractor s security badge. BADGES All Contractor personnel are required to wear identification (ID) badges during the entire period during which they are on VA grounds. ID badges must show an identification picture, name of the individual, and the represented company. PARKING It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Security Service. The VA will not invalidate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. SMOKING Smoking is not permitted within or around the VA Healthcare System facilities, except in designated areas. Delivery Date: ARO 30 days Contract Type: Firm Fixed Price (FFP) FAR 52.211-6 Brand Name or Equal (AUG 1999) (a) If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation. Offerors shall provide the information required by FAR 52.212-1 (APR 2014), Instructions to Offerors-Commercial Items, which is incorporated by reference. If the end product (s) is other than domestic end product(s) as defined in the clause entitled Buy American Act Supplies, the offeror shall so state and shall list the country of origin. FAR 52.212-2 (OCT 2014) Evaluation Commercial Items- The following factors shall be used to evaluate offers: Only certified Veteran Owned Small Businesses (VOSB) in VetBiz will be evaluated Technical Capability of the item offered to meet the Government requirement Price Offerors, if not registered in SAM, must include completed copies of the provision at 52.212-3 (MAR 2015), Offeror Representations and Certifications - Commercial Items with their offer. These representations and certifications will be incorporated by reference in any resultant contract. FAR 52.212-4 (DEC 2014), Contract Terms and Conditions-Commercial Items is applicable. The following identified clauses are incorporated by reference: 852.211-73, 852.219-11, 852.232-72, 852.237-70, 852.246-70, 852.246-71, 52.211-16, 52.216-1, 52.222-41, 852.270-1, and 852.273-74.
