Solicitation VA262-17-Q-1749 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. The Government is seeking a vendor who can provide the following item for the VA Long Beach Healthcare System: Item Number Description QTY Unit Price Total Price TTT7 Provel - Auto Thermoformer with Rings Automated prosthetic socket forming machine designed to use standard prosthetics socket preforms. Features Required: Infrared temperature sensing of the preform itself of process control; One piece spun aluminum over for heating; Built in clamp ring and storage table; Illuminated forming area; 16,18, and 25 cm clamp rings and tables; Computerized monitoring of preform temperature oven, temperature door, carriage position, carriage drive and vacuum system function. 1 This solicitation is for brand name (exact match) only and offerors shall be an authorized distributor by the manufacture and must be able to offer full manufacture warranty all at a fair and reasonable price offering best value to the Government.
