Description

1 The Gulf Coast Veterans Health Care System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis/ solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures FAR Part 13. We intend to issue a sole source purchase order to PartsSource, Inc., 777 Lena Drive, Aurora, OH 44202-8025. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number VA256-17-Q-1329 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 effective January 13, 2017. The NAICS code is 334510 and the size standard is 1,250 employees. This requirement is for a sole source and will be firm-fixed-price. The closing date for this notice is 12:00 PM CST Aug. 18, 2017. NOTE: This RFQ has a short notice for response time, three days only. The requirement is for the following: The G.V. (Sonny) Montgomery VA Medical Center, Jackson, MS, is looking to award a one-time firm-fixed contract for the period of performance beginning from date of award until completion of services. The contractor shall furnish all labor, tools, materials, transportation, equipment, parts, testing, inspections and supervision. The equipment to be covered is one Philips Echo Ultrasound, Model IE33, Part # 453561604294, Part Description UMB1 7600, W/O HD. (See Statement of Work below for detailed information). Contractor to provide services in accordance with terms and conditions of the contract for the date of award through August 30, 2017, or sooner. To be eligible for award of a contract, vendor must be actively registered in the SAM database (https://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. Procurement of Replacement IE33 electronic board and Installation for a Philips IE33 echo ultrasound. 3. Background. The G.V. (Sonny) Montgomery VA Medical Center (GVSM VAMC) is requesting the purchase of a replacement part for a Philips IE33 echo ultrasound system, the existing part s de-installation, and the replacement part s installation. The system s electronic board has failed causing Veterans delayed access to care. The board (P/N 453561604294) is in the IE33 ultrasound unit used.. Without the failed board, the system is non-functional and will continue to cause Veterans delays in access to care. The relevant system information is as follows: System: Echo Ultrasound Manufacturer: Philips Model #: IE33 Part Description UMB1 7600, W/O HD 4. Scope. Purchase of replacement Philips IE33 system s failed board (Part #:453561604294), the de-installation of the failed board, and installation of the new board. The electronic board shall be new; GVSM VAMC will not accept a used or refurbished collimator. The board shall have a minimum of 30 days warranty. The labor shall have a minimum of 30 days warranty. Compensation for Exchange Rate of broken board with new board shall be considered during procurement. Description Quantity UMB1 7600, W/O HD 1 Labor for installation To be determined 5. Specific Tasks. Part will be installed by contractor and equipment performance will be verified with system operational specifications by the contractor and Biomedical Engineering Support Specialist at the GVSM VAMC. 6. Performance Monitoring GVSM VAMC Biomedical Engineering Technicians will oversee the installation of the replacement part, and test and validate the proper operation of the repaired system before returning it to service. All documentation on the new board and service reports on the equipment is required to be submitted to the COR within 2 days of repair.
