- STATEMENT OF WORK (SOW) as of 12/05/2012 Contract Number: Task Order Number: IFCAP Tracking Number: Follow-on to Contract and Task Order Number: 1. Contracting Officer s Representative (COR) Name: Section: SPS/Logistics Address: 1501 San Pedro SE Albuquerque, NM 87108 Phone Number: Fax Number: E-Mail Address: 2. Contract Title. Raymond G. Murphy VA Medical Center (Albuquerque, NM) request 27 Bladder Scanners to replace current outdated models 3. Background. The current Verathon BVI 3000 bladder scanner used by the Albuquerque VAMC are past their life expectancy and have been discontinued as of August 1, 2016, and support for the model BVI 3000 will end. This overall replacement will also standardize the bladder scanners in the facility. Bladder Scanner Probes: There is a seam in the current probes we are using that do not allow them to be cleaned effectively (they have dried gel in the seams that is nearly impossible to get out). They are used on bare skin in areas where there is contact with patient hair. Some of them have been broken and need to be replaced, this due to them being dropped because they are very slippery when the glove and probe are cover with gel and they have a poor design and are heavy like a pendulum. Biomedical has recommended replacement for all bladder scanners for the inpatient units. 4. Scope. The overall scope of work is the procurement of 27 Bladder Scanners. The selected equipment manufacturer will furnish: Pkg Speq T-Probe, Tablet, Mobile Cart, Citizen CMP-20BT Printer, and Standard 5 Year Warranty.
