FAR 52.212-3 Offeror Representations and Certifications - to be returned with quote EXPERIENCE QUESTIONNAIRE - must be filled out SCHEDULE OF ITEMS - PRICING INFORMATION TO BE SUBMITTED BY VENDOR This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is AG-84JC-S-17-0018 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This solicitation replaces AG-84JC-S-17-0015, which was originally set aside for small business. Due to lack of competitive quotes, this solicitation is not reserved for any set asides, but is being issued under full and open competition. The associated NAICS code is 238220 with small business size standard of $15M. DESCRIPTION OF SERVICES Monthly preventative maintenance for Five (5) air conditioning units located in Server Room, as well as on-site response and repair as needed. EQUIPMENT - One 5 ton Liebert Challenger 2 Floor A/C Model #CF066A-C00, Serial # P20980 - One 15 ton Lieert Floor A/C Unit Model #DH199A-AAEI18524, Serial # C07H010218 - One 20 ton Liebert DS Floor A/C Unit, Model # DS070ADAOE1798A, Serial # C09G8E0204 - One Mitsubishi Ductless Split Wall-Mount AC Unit, Model # PKA-A36FAL - One Fujitsu Ductless Split Wall-Mount A/C Unit, Model # ASU9RQ HOURS Preventative maintenance will be performed once a month during regular business hours of 7:00 a.m. through 4:30 p.m., Monday through Friday. Dates and times will be mutually arranged between the Forest Service and the contractor. All costs for routine preventive maintenance will be included in the fixed monthly payment under this contract. Emergency service shall be available 24 hours a day, 7 days a week with a 4 hour response time. Repair time should occur within 24-48 hours from the time of the initial service call. The contractor shall diagnose problems with the system(s) and make necessary repairs to bring the affected systems back into operational status in the timeliest fashion, as agreed to by the Forest Service and the Contractor. MAINTENANCE REQUIREMENTS At each monthly maintenance visit, the contractor shall provide a detailed list of items checked and maintenance performed to the Forest Service Contracting Officer's Representative (COR). In addition, the contractor shall list any maintenance recommendations or potential performance issues identified during routine maintenance activities. PREVENTATIVE MAINTENANCE Preventive maintenance shall include all condition and performance checks recommended by the manufacturer; equipment adjustments necessary for optimal performance; cleaning; lubrication; and routine replacement of fluids, filters, belts, and other disposable supplies and parts. After contract award, the contractor shall provide an itemized list of maintenance to be performed, and its frequency, similar to the following representative list. Fan Section • Check and align blower and motor sheaves (quarterly) • Replace filters (as needed) • Check and lubricate motor and blower bearings (quarterly) • Inspect blower wheels for integrity and rotation (monthly) • Check fan belts for integrity and proper tension, replace if necessary (monthly) • Inspect and tighten electrical connections (quarterly) • Inspect contactor and motor starters (quarterly) • Clean accessible area of condensate drain pan, if necessary (monthly) • Clean compartments inside the air handler, if necessary (monthly) Cooling Section • Inspect evaporator coil (monthly) • Clean evaporator coil (as needed) • Check operation of all safety controls (quarterly) • Check and lubricate motor bearings (quarterly or as needed) • Inspect refrigerant levels (monthly) • Inspect and tighten electrical connections (quarterly) • Inspect contactor and motor starters (quarterly) • Clean condenser coils semiannually (quarterly) • Check condenser for leaks (monthly) • Check compressor suction pressure, oil level, discharge pressure and amp draw (monthly) • Check connections at compressor terminals (quarterly) • Check operation of safety controls (quarterly) • Check compressor crankcase heater (quarterly) CONTRACTOR QUALIFICATIONS: The contractor shall be a certified HVAC Technician, shall be experienced and qualified to service various air conditioning units to include Liebert units. The Forest Service requests the contractor to provide proof of certification upon award of contract. WARRANTY AND SERVICE: The Contractor guarantees all workmanship, parts, and materials utilized in the maintenance of AC units under this contract. The Contractor further agrees to replace free of charge any part that may fail in any manner by reason of defective materials or workmanship within 90 days after the acceptance of work by the Government or within the manufacturer's guarantee period, whichever is greater. The period of performance is September 1, 2017 through January 31, 2019. EVALUATION CRITERIA : Award shall be made to the QUOTER whose QUOTE offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: 1. Past Performance. Provide a list of all similar contracts performed within the past three years to include the type of HVAC equipment. Include references with a contact name and current telephone numbers. 2. Technical Capability to perform the work. Describe your experience and qualifications in this type of work. Also address types of HVAC equipment worked on. 3. Price. Prices submitted in the Schedule of Items will be evaluated for reasonableness and the degree to which they reflect an understanding of the work requirements. The Government may screen the quotations on price and those quotations determined to be unreasonably high will not be further considered. Experience Questionnaire: The Experience Questionnaire or different format if used addressing all areas of the Experience Questionnaire will be used to make an evaluation and arrive at a determination as to whether the quotation will meet the requirements of the Government. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and a statement regarding any addenda to the clauses. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
