- COMBINED SYNOPSIS SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. VA101V-17-Q-0348 The solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-95 This solicitation is set-aside exclusively for Service-Disabled Veteran-Owned Small Business concerns. To be considered for award prospective offerors must be a verified SDVOB concern in the VA Center for Verification and Evaluation (CVE) Veteran Information Pages (VIP) pages at the date and time set for receipt of offers. An Offeror who is not a verified SDVOB concern at the date and time set for receipt of offers will have their offer rejected as non-responsive, and will not be considered for award. The applicable NAIC Code for this solicitation is 238220 Plumbing, Heating, and Air-Conditioning Contractors Size Standard 14 million. HVAC Maintenance, see attached SOW. ITEM NO. DESCRIPTION QTY UNIT UNIT PRICE AMOUNT 0001 The contractor shall provide all supplies, services, equipment, tools, labor, facilities, transportation, and supervision to provide HVAC Maintenance as required in the attached SOW. 1 JB $___________ $___________ 1001 Option Year One shall provide all supplies, services, equipment, tools, labor, facilities, transportation, and supervision to provide HVAC Maintenance as required in the attached SOW. 1 JB $___________ $___________ 2001 Option Year Two shall provide all supplies, services, equipment, tools, labor, facilities, transportation, and supervision to provide HVAC Maintenance as required in the attached SOW. 1 JB $___________ $___________ 3001 Option Year Three shall provide all supplies, services, equipment, tools, labor, facilities, transportation, and supervision to provide HVAC Maintenance as required in the attached SOW. 1 JB $___________ $___________ 4001 Option Year Four shall provide all supplies, services, equipment, tools, labor, facilities, transportation, and supervision to provide HVAC Maintenance as required in the attached SOW. 1 JB $___________ $___________ Description of Requirement Veterans Benefits Administration (VBA) Indianapolis Regional Office (RO) Commercial HVAC Maintenance Statement of Work August 2, 2017 Background: The Indianapolis Regional Office requires a comprehensive preventative maintenance agreement for chillers associated with our training rooms. Based on vendor recommendations one (1) Carrier 30 RAP-1T, located on the roof of the building exists. The AC unit operates throughout the summer to cool the training rooms located on the 3rd floor, independent of AC units that work to cool the entire building. It has been determined that a continual preventative maintenance agreement is required to perform maintenance twice a year in accordance with warranty standards. In addition, emergency service calls are required where operational problems inhibit proper functioning of the unit. Scope of Work: The scope of services includes routine and preventive maintenance (PM) and immediate response to emergency calls. The Contractor shall provide all supervision, labor, administrative support, materials, tools, parts, supplies, equipment, and transportation necessary to effectively and efficiently fulfill all the requirements of this Statement of Work. The contractor shall be responsible for performing all routine maintenance and repair work required to maintain the air conditioning unit in the condition prescribed by the original equipment manufacturers recommended guidelines and/or warranty requirements where the warranty remains in force. The Contractor shall work closely with the Support Services Division (SSD) team. The Regional Office also requires certified service personnel to conduct repairs in the event of a problem, as well as two (2) scheduled preventive maintenance service visits providing a comprehensive visual, environmental and operational inspection of the unit to ensure components are performing to defined technical and environmental specifications. Requirements: The preventive maintenance agreement shall include: Semi-annual inspection Maintenance is to be performed during normal business hours Address, verify and clear all errors pertaining to unit Check condenser coils for debris, clean as necessary Check refrigerant charge Check all refrigerant joints and valves for refrigerant leaks Check chilled water flow switch operation Check all condenser fans for proper operation Check compressor oil level Check all electrical connections, tighten as necessary Inspect all contactors and relays, replace as necessary Check accuracy of thermistors Obtain and test a compressor oil sample Check refrigerant filter driers for excessive pressure drop Check chilled water strainers, clean as necessary Check cooler heater operation, if equipped Check condition of condenser fan blades and that they are securely fastened to the motor shaft Perform Service Test to clear fault codes and confirm operation of all components Check for excessive cooler approach (Leaving Chilled Water Temperature Saturated Suction Temperature) which may indicate fouling Clean microchannel heat exchanger condenser coils Proactive part replacement per manufacturer recommendation The emergency maintenance agreement shall include: Emergency response within 24 hours Emergency call and travel fees Place of Performance: Department of Veterans Affairs ATTN: Horace Jordan 575 N. Pennsylvania St. Indianapolis, Indiana 46204 Period of Performance: Base Year: September 29 2017 to September 28, 2018. Contract will include four (4) option years. The Government reserves the right to exercise an option or cancel contract after the base year. If exercised, these option years will be established and funded by the applicable fiscal year. Contract Administration of Task Order: The on-site SSD Supervisor will serve as Government Project Officer (GPO). They will provide general instructions on limitations and deadlines, and are responsible for the administration of the task order in compliance with the contract to include inspection and acceptance of deliverables. Changes and modifications can only be authorized by a warranted Contracting Officer. Cancellation of Services: The Government reserves the right to cancel whole or in part services or the entire task order. Cancellation will be communicated and executed by issuance of a supplemental agreement modification to the task Order in accordance with the terms and conditions of the providers GSA Federal Supply Schedule. Billing: Services performed under this contract will be billed monthly. The contractor shall provide an invoice that will include: Contract Number Company Name and Address Invoice Date Invoice Number (Note: Each invoice must have a different Invoice Number) Description of items/services provided as well as referencing the appropriate Contract Line Item Number (CLIN). This way the proper CLIN can be billed for the Invoice. Terms for any discount for prompt payment Name, title and phone number of company point of contact to notify in case of a defective invoice As always, this contract is contingent on the availability of funds, and can be modified or ended due to convenience of the government. Period of Performance: 60 Days from issuance of the notice to proceed FAR 52.212-1, Instructions to Offerors Commercial Items, applies to this solicitation. The following provisions and clauses are added as addenda: 52.252-2 Clauses Incorporated By Reference (FEB 1998) 52.204-9 Personal Identity Verification of Contractor Personnel (JAN 2011) Insurance Work on a Government Installation (JAN 1997) (a) Workers compensation and employer s liability. Contractors are required to comply with applicable Federal and State workers compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer s liability section of the insurance policy, except when contract operations are so commingled with a contractor s commercial operations that it would not be practical to require this coverage. Employer s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.) (b) General liability. (1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence. (2) Property damage liability insurance shall be required only in special circumstances as determined by the agency. (c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims. 52.232.40 Providing Accelerated Payments to Small Business Subcontractors (DEC 2013) VAAR 852.203-70 Commercial Advertising (JAN 2008) VAAR 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (DEVIATION) (JUL 2016) Limitations on Subcontracting - Monitoring and Compliance This solicitation includes VAAR Clause 852.219-10, VA Notice of Service-Disabled Veteran-Owned Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.
