This is a COMBINED SYNOPSIS/SOLICITATION for commercial services prepared in accordance with the format in FAR subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This purchase is unrestricted. 2. Solicitation number 17-247-SOL-00083 is being issued as a Request for Quote (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vmfara.htm (FAR) and at http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. The North American Industry Classification System (NAICS) code is 517110-Wired Telecommunications Carrier, and the applicable size standard is 1,500 employees. 5. This requirement consists of five (05) line items: 0001 - Base Year. Telecommunications Service in Peach Springs, AZ 0002 - Option Year One (1): Telecommunications Service in Peach Springs, AZ 0003 - Option Year Two (2): Telecommunications Service in Peach Springs, AZ 0004 - Option Year Three (3): Telecommunications Service in Peach Springs, AZ 0005 - Option Year Four (4): Telecommunications Service in Peach Springs, AZ 6. This requirement is for a service agreement for telecommunications services as described in the attached statement of work. 7. The location and address of contract performance is listed below. The Period of Performance will be 12 months from the commencement of service along with four (4), 12-month option periods. The total length of contract will not exceed 5 years from commencement of service. 8. INSTRUCTIONS TO OFFERORS: Submit completed SF 1449 and Price Schedule with Quote. The Defense Priorities and Allocations System (DPAS) is not Applicable to this acquisition. 10.
