Description
THIS IS A SPECIAL NOTICE OF THE VISN 7 CONTRACTING NETWORK S INTENT TO SOLE SOURCE. NO HARD COPY OF THE RFQ IS AVAILABLE FOR DISTRIBUTION. This notice is being published in accordance with Federal Acquisition Regulation (FAR) Part 5.101(a)(1) dissemination of information on proposed contract actions. The Government anticipates entering a sole source procurement with Abbott Laboratories, Inc., to supply the below supplies to the WJ Dorn VA Medical Center in Columbia SC. Abbott is the sole proprietor of the OMNILINK ELITE & SUPERA Peripheral Stent System and does not use distributors in the US. Other persons may identify their interest and capability to respond to this requirement by August 25 at 12:00 NOON CST. This Notice of Intent is not a request for competitive proposals. However, all interest received on or before the date and time indicated above will be considered by the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. Information received as a result of the notice of intent will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. No solicitation document is available. STATEMENT OF WORK PREVENTIVE MAINTENANCE ALARIS INFUSION PUMPS 1. REQUIREMENT: This requirement is for a new base + 4 option preventive maintenance/service for Alaris Infusion Pumps located at Dorn VAMC, Building 100. 2. BACKGROUND: Dorn VAMC has 775 Alaris Pumps (Models 8015, 8100, 8120, & 8300) in service at Dorn VAMC. Currently, there is no service contract in place for the maintenance and service for these pumps. A new base plus 4 year option service contract is required. Having a maintenance contract on these pumps provides the most economical value to the Government, rather than paying for preventive maintenance/service (non-contract) support. 3. PERIOD OF PERFORMANCE: The period of performance for the base year is May 1, 2017 through April 30, 2018. Work shall be performed during normal working hours (Monday Friday; 0800-1630) except federal holidays. 4. PLACE OF PERFORMANCE: Work to be performed at WJB Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209. The Vendor s Service Representative (VSR) will contact Biomedical Engineering (803) 776-4000, extension 7578 or 7583 to schedule an inspection or prior to responding to a service call. 5. PERFORMANCE REQUIREMENTS: The Contractor shall perform and provide the following to support the Alaris pumps: Vendor shall provide all personnel, parts, tools, glassware, test equipment, service manuals, computer software, applications support and schematics necessary to affect repairs and preventive maintenance procedures on all of the identified equipment (listed below) during normal working hours, 8:00 AM 4:30 PM EST Monday through Friday except federal holidays. EQUIPMENT: 214 EA Alaris Pump model #8015 385 EA Alaris Pump model #8100 88 EA Alaris Pump model #8120 88 EA Alaris Pump model #8300 The Vendors Service Representative (VSR) will contact Biomedical Engineering (803) 776-4000 extension 7578/7583 to schedule an inspection or prior to responding to a service call. The VSR will acknowledge receipt of a service call within 2 hours, and will arrive on the premises to effect repairs within 24 hours. The VSR will provide, upon request, suitable loaners for contracted equipment that will be out of service for more than 48 hours. Equipment repaired by the VSR or loaned to the Medical Center by the vendor will be calibrated and safety tested by the VSR before being placed into service. The VSR will not deviate from this protocol without prior approval of the Biomedical Engineering representative. The VSR will present the field service report to a representative of Biomedical Engineering for an authorized signature, leaving one legible copy for inclusion in the individual equipment s Historical Maintenance Record. Vendor will also provide Biomedical Engineering with a legible copy of the calibration reports including numerical values of measurements taken within 5 business days. Scheduled preventive maintenance will include, but not limited to, electrical safety testing, lubrication, adjustments, calibration, testing and replacement of faulty parts and parts which are likely to fail at no additional charge, returning the instruments or equipment to the operating conditions specified by the manufacturer, NFPA 99 and VA MP-3 guidelines. A copy of the Preventive Maintenance Inspection procedure (including Qualitative and Quantitative tests) used by the VSR for each item listed on the contract, shall be made available upon request. All work shall be performed by competent, experienced personnel qualified to work on the specified equipment in accordance with good manufacturing practices. The Vendor shall provide factory training certificates/competencies for all technicians assigned to service the specified equipment, in accordance with JCAHO standards. Such certificates/competencies are to be submitted to Biomedical Engineering within 30 days from the date of contract award. Failure to provide the required documentation will result in non-payment of invoices for this service. Per ISO 9001, the vendor shall list all the test equipment that has been used along with the details including their calibration due date, test equipment name, model, serial number, Mfg., etc. on the service report during any services associated with medical equipment. Training for two Biomedical Technicians will be provided so that the government Biomed technicians can handle all FIRST CALL service on the existing contracted medical device systems, and/or will include all service possible on the medical device system, given that factory quality parts are available under this contract. Training will include all service kit materials and software mode authorizations. Training will include all airfare, meals, transportation and hotel accommodations during the training event. This is an optional item that can be cancelled, if the government makes a decision 60 days before the training not to attend. This is a contract deliverable option, not a requirement for that year, to be decided upon during the year of the contract, upon demand. If funding amounts are not available due to further expenses, then the government can decide to not exercise the option. The vendor will be required to have the option available, upon demand of the government, and clearly spelled out in all service contracts.
