Description
This is a COMBINED SYNOPSIS/SOLICITATION for Brand Name Only commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0898 and is issued as a Request for Quotation (RFQ). The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) This solicitation is issued as a small business set aside. The North American Industry Classification System (NAICS) code is 334510, and the size standard is 1250 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of FAR 19.102(f). Contractors that deem themselves capable of meeting the requirement shall provide quotes to Michael Iffland, Contract Specialist, at michael.iffland@va.gov no-later-than Monday, August 28, 2017 at 5:00PM, EDT. Additional quad outlet 0012 Installation 1 EA FOR MORE DETAILS PLEASE SEE THE STATEMENT OF WORK BELOW DESCRIPTION/SPECIFICATIONS/WORK STATEMENT I. SCOPE OF WORK: The contractor shall provide materials, labor, equipment and supervision for the installation of an audiology sound booth and equipment in the audiology sound booth at the Wakeman VA Clinic, Building 1012, 3791 10th Street, Edinburgh, IN 46124. The booth and equipment shall include parts installed by a factory certified installation team. Cam-Locking Wall Construction: Otometrics single wall exam rooms configured from a single wall examination room with 4 thick steel walls and roofs. Air gap between the audiology exam room and host wall is 4. Wall panels shall be 4 thick with 16 gauge electroplated galvanized steel sheet on the outer surface. The inner faces are 22 gauge cold rolled steel perforated with 3/32 holes on 5/32 staggered centers, roll formed to strengthen them and make an array of anti-standing wave wedges. Panels are filled with 1 of water and moisture resistant, direction fiber structure and fire resistant Roxul Rockboard stone wool insulation board. Fill material is water repellent, chemically inert, non-corrosive, fungi and mildew non-promoting and made from natural and recycled materials. Panels are locked together using positive locking mechanisms and acoustical tape around a uniquely designed cam-locking male/female panel design for optimal acoustic performance. Cam-Locking Roof Construction: Ceiling panels shall be 4" thick with 16 gauge electroplated galvanized steel sheet on the outer surface. The inner faces are 22 gauge cold rolled steel perforated with 3/32" holes on 5/32" staggered centers, roll formed to strengthen them and make an array of anti- standing wave wedges. Floor Construction: Each booth will be provided with our unique Hi-Performance/Lo-Profile Floor. Floor panels shall be 1 9/16 (39.7 mm) tall with a ¼ plate painted steel sheet sub floor. Floor Panels shall sit on 1 natural rubber isolation pads. The Exam Room wall panels will sit directly on top of the Exam Room Panels and held in place by 16ga floor channels. Doors: Door panel construction will match fill and finish of wall panels. In addition, the door panel will be flush mounted on heavy duty Kason cam-lift hinges. A double magnetic gasket-sealing system and flush sill seal design assures the acoustic seal. Door handles are constructed of 11ga Stainless Steel. Door opening at 36 for ADA accessibility. Patient Observation Window: Acoustic window for patient observation, 24 wide by 30 high (limited to booth size). Electrical: All electrical will be wired using UL/CE approved components in accordance with NEC Code. All electrical outlets/light & fan switches will be recessed inside the wall panel inside a steel junction box. Wires will be run in steel conduit inside the wall panel. All conduit runs will terminate to a roof mounted and secured steel junction box. A 20 power cord will be provided for connection to a host outlet. All projects are inspected by certified UL & CSA representatives directly at the factory and ship with field UL inspection labels. Lighting: Roof mounted low profile HALO LED lighting will be UL/CE approved. Connections will be made inside a recessed steel junction box. Wires will be run in steel conduit inside the wall panel. Light will be constructed from die cast aluminum trim ring, and die formed aluminum frame. Finish: Booths shall be finished to match the color to the Wakeman VA Clinic. Cable Access: Invisible cable system allowing for the running of low voltage wiring behind the inner skin of the 16ga channels such that the raceway will run from panel to panel all the way around the room. Includes Freefit (PMM) Speaker, Non-functioning audiometer. Install equipment installation. Warranty and Maintenance: Pre-installation site evaluation by installers. Two-year warranty on sound booth. One-year equipment warranty on main unit. Six-month warranty on transducers and external components II. CONFORMANCE STANDARDS: All services provided under this contract must be performance in conformance with the Federal Regulations and BCVA policies and requirements. III. PREVENTIVE MAINTENANCE: Not Applicable IV. PARTS: To be supplied by vendor. V. REPORTING REQUIREMENTS: The Contractor shall be required to report to Laura Flood of the Wakeman CBOC during normal business hours. This check is mandatory. When the installation is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out with Laura Flood and submit the field report to the Contracting Officer Technical Representative (COR) for a signature, during normal working hours. If the COR is unavailable, a signed authorized copy of the field service report shall be sent to the COR after the work has been reviewed. VI. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of the occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or their representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, 52.203-98, 52.204-16, 52.204-17 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation.
