Attachment 3 - RFQ Information Sheet - to be submitted with quotation Attachment 1 - Schedule of Prices - to be submitted with quotation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is NAMA-17-Q-0092 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 and 2005-95 effective January 19, 2017. This is an unrestricted procurement under NAICS 811212, Computer and Office Machine Repair and Maintenance. CONTRACT LINE ITEM NUMBER(S): See Attachment 1, Schedule of Prices. REQUIREMENT: The National Archives and Records Administration (NARA) has a requirement for preventative maintenance and repair services for 22 KEM flatbed film viewers located in College Park, MD. The following Federal Acquisition Regulation (FAR) clauses are incorporated and are to remain in full force in any resultant contract, with the exception of FAR provisions which will be removed prior to award: FAR 52.212-1. Instructions to Offerors - Commercial Items (Jan 2017). Addenda: Delete paragraph (h), Multiple Awards. - see below for additional quotation submission instructions. EVALUATION (In lieu of FAR 52.212-2): Evaluation and award will be in accordance with Simplified Acquisition Procedures at FAR Subpart 13.106, Soliciting Competition, Evaluation of Quotations or Offers, Award and Documentation authorized by FAR Subpart 13.5, Simplified Procedures for Certain Commercial Items. Selection of the Contractor to perform this contract will be based on NARA's assessment of the best overall value to the Government. Accordingly, best value for this procurement will be obtained through a lowest price technically acceptable evaluation. Exceptions taken to any terms and conditions stated in the RFQ must be clearly outlined on a separate page of the Contractor's quotation entitled "Exceptions." The Contractor must also include complete rationale, justification, and the cost impact of each exception noted. The Government reserves the right to make an award on the initial quotation without discussions of this procurement. For evaluation purposes, the Total Contract Ceiling Price will be used to determine the lowest price. The Total Ceiling Price for each CLIN will be determined by multiplying the Unit Price by the Quantity of Units. The evaluated Total Contract Ceiling Price will be determined by summing the Total Ceiling Price of all performance years (base year and all option years). Contractors must provide pricing (USD) for each CLIN in accordance to Attachment 1, Schedule of Prices. Contractors must complete and submit Attachment 3, RFQ Information Sheet. Failure to submit any of the required information and documentation with the quotation may result in rejection of the quotation. ATTACHMENT 2 PERFORMANCE WORK STATEMENT FOR PREVENTATIVE MAINTENANCE AND REPAIR OF FLATBED FILM VIEWERS 1.0 BACKGROUND INFORMATION 1.1 The National Archives and Records Administration (NARA) is an independent Federal agency that helps safeguard and preserve our nation's history by overseeing the management of all Federal records. The stories of our nation and our people are told in the records and artifacts cared for in NARA facilities around the country and people can discover, use, and learn from their documentary heritage. The mission is to ensure ready access to the essential evidence that documents the rights of American citizens, the actions of Federal officials, and the national experience. 1.2 The purpose of this maintenance agreement is to provide preventative maintenance and repair services on a flexible schedule for KEM flatbed film viewers located at NARA in College Park, MD (AII). The flatbed film viewers are located in research rooms and certain staff areas and are used to examine records that are either 35mm or 16mm film prints. This maintenance agreement will cover 22 KEM flatbed film viewers at Archives II. While there is no metric relating to customer satisfaction with the flatbed film viewers, we know from past experience that we need to maintain flatbed film viewers in good working condition to prevent complaints and to prevent any damage to motion picture film records. Priority for preventative maintenance and repair services will be determined by frequency of use and time since last maintenance. 2.0 SCOPE 2.1 Contractor shall provide all labor, materials, equipment, wearable parts (e.g., light bulbs, belts, headphone jacks, knobs, switches), test equipment, permits, fees, licenses, transportation, and tools for inspecting, reporting, servicing, and performing all preventative maintenance and repairs necessary to maintain the flatbed film viewers in a safe and reliable operating condition. 2.2 Contractor shall comply with the maintenance and repair requirements as stated in the manufacturer's specification for the flatbed film viewers listed below: Manufacturer Model Quantity KEM KS4-35 5 KEM KS4-16 5 KEM KS2.16 5 KEM KS2.35 5 KEM KSM6.35 1 KEM KSM6.16 1 2.3 On Site Repair Services 2.3.1 Contractor shall be notified via phone and/or email by the NARA Point of Contact (POC) or their designated representative in the event of a flatbed film viewer malfunction. Contractor shall respond via telephone and/or email to the NARA POC or designated representative within four (4) hours after receipt of telephone or email notification. 2.3.2 Contractor shall arrange for flatbed film viewers to be serviced within fourteen (14) business days of receipt of notification. Contractor shall consult with the NARA POC to schedule a visit for repair services. 2.3.3 Contractor shall coordinate all scheduled repair services with the NARA POC or their designated representative at least three (3) working days in advance of service visit. 2.3.4 Contractor shall report to the NARA POC or their designated representative prior to commencing work. 2.4 Preventative Maintenance 2.4.1 Contractor shall provide quarterly preventative maintenance service for the flatbed film viewers. For each quarterly preventative maintenance visit, the NARA POC or their designated representative will identify twelve (12) flatbed film viewers that will be serviced. 2.4.2 Preventative maintenance shall be performed in accordance with the flatbed film viewers manufacturer's specification. Preventative maintenance shall include at a minimum: 1. Perform operation checks and correct any deficiencies noted in accordance with manufacturer's specification; 2. Clean, lubricate, replace, and adjust, mechanical parts in accordance with the manufacturer's specification; 3. Replace excessively worn or defective parts. Excessively worn parts are those parts that if not replaced may fail prior to the next scheduled preventive maintenance visit; 4. Visually inspect the exterior of all flatbed film viewers for damage and cleanliness and report any discrepancies to the NARA POC or their designated representative; and 5. Consult with the NARA POC or their designated representative regarding flatbed film viewer performance. 2.4.3 Contractor shall coordinate all preventative maintenance with the NARA POC or their designated representative. 2.4.4 Preventative maintenance shall be completed on all flatbed film viewers within fourteen (14) business days of the coordinated date. 2.4.5 Contractor shall maintain, at a minimum, a 95% (less flatbed film viewer maintenance downtime) flatbed film viewer uptime or in-service rate for the research room flatbed film viewers. In order to determine the uptime or in-service rate, the NARA POC or their designated representative will periodically and randomly check all of the flatbed film viewers to verify their operability and then calculate whether the 95% uptime or in-service rate is being maintained. 2.5 Diagnostics 2.5.1 Contractor shall perform diagnostics on all KEM flatbed film viewers covered under this maintenance agreement during the base year of the order. 2.6 Replacement Parts 2.6.1 Contractor shall only use manufacturer's new original or factory refurbished parts in the performance of this maintenance agreement, except when the manufacturer or its successor companies no longer provide new or factory service parts for a malfunctioning machine. In such cases, the Contractor shall provide in writing to the NARA POC or their designated representative how it concluded that new or factory refurbished parts are no longer available. Replacement parts must be approved by the NARA POC. 2.6.2 If additional parts are required, Contractor shall notify the NARA POC or their designated representative before incurring additional expenses. The Contractor shall provide the NARA POC with a written cost estimate. Repairs shall not be made and parts shall not be purchased without written approval from the NARA POC or their designated representative. Any purchase in advance of NARA's written approval is at the Contractor's risk. 2.7 Reports 2.7.1 Contractor shall submit various Contractor-designed reports to the NARA POC or their designated representative upon work completion for each Contractor service visit. 2.7.1.1 Diagnostic Report shall be submitted after diagnostics has been completed on each flatbed film viewer. The report shall outline the work completed and any repairs that may be required. 2.7.1.2 Preventative Maintenance Reports shall be submitted after each preventative maintenance visit. Reports shall detail any findings. 2.7.1.3 Diagnostic and Preventative Maintenance reports shall be submitted five (5) business days after each Contractor visit. The reports shall include: 1. Contract Number 2. Date 3. Flatbed Number or Serial Number 4. Name of Service Technician 5. Maintenance Performed 6. Equipment Downtime 7. Part(s), if any, to be ordered 8. Signature and date from the NARA POC or their designated representative 2.7.1.4 Contractor shall submit a contractor-designed report to the NARA POC or their designated representative two work days after the end of each month. The report shall document the fact that a 95% or greater equipment uptime or in-service rate for flatbed film viewers in the research room has been maintained. The report shall include the following: 1. Flatbed Number or Serial Number 2. Date 3. Service Downtime (in Hours) 4. Total monthly downtime for all flatbed film viewers (in hours and percent). 2.8 Invoices 2.8.1 Contractor shall submit a monthly invoice for services provided the previous month. 2.9 Warranty 2.9.1 Contractor shall perform all work within the strict term and conditions of all existing commercial warranties (if any) on the equipment and must not perform work that would jeopardize said warranties. 3.0 PERIOD OF PERFORMANCE 3.1 The period of performance is one base year plus four (4) option years. 4.0 PLACE OF PERFORMANCE AND NARA POINT OF CONTACT (POC) 4.1 Preventative maintenance and repair services shall be performed at the following location: National Archives and Records Administration 8601 Adelphi Road College Park, MD 20740 4.2 The NARA Point of Contact for this order will be: Name: tbd Phone: Email: 5.0 HOURS OF OPERATION 5.1 The flatbed film viewers that are in use in Room 4000 and staff areas (Rooms 3350, 4350, B 730, and B 600) will be available for on-site repair and preventative maintenance from 8:45 am to 5:00 pm eastern standard time, Monday through Friday, excluding Federal Holidays. 5.2 The flatbed film viewers that are in the declassified and classified staff areas will be available for on-site repair and preventative maintenance when a cleared NARA staff member is available to accompany the Contractor or when the flatbed film viewers may be removed from the classified space. 6.0 PERSONNEL 6.1 Contractor shall ensure that all of their employees performing services under this maintenance agreement have successfully passed the Contractor's screening process; are free of excessive use of alcohol, use of illegal drugs, violent and/or anti-social behavior, and dishonesty. 6.2 The NARA POC or Contracting Officer may request the Contractor to immediately prevent any employee(s) from performing work under this maintenance agreement should it be determined that individuals performing services are disqualified for either suitability or security reasons, are found to be unfit to perform their duties, or pose an unacceptable risk to Government equipment. 6.3 Contractor shall provide the NARA Motion Picture Unit (RDSM) a list of the names of all employees having access to the Archives II location and shall provide the names of all employees who have a completed background/criminal check. This will be required for any additional personnel and replacement vacancies. 6.4 Contractor employees shall wear identification badges provided by NARA security while performing services at NARA. 6.5 Contractor employees shall sign in and sign out with the Security Desk and designated representative for each location before beginning and upon completion of any scheduled or unscheduled repair service and preventative maintenance services. 7.0 TASKS AND DELIVERABLES 7.1 Tasks and deliverables are discussed in paragraph 2.0. 8.0 MISCELLANEOUS REQUIREMENTS 8.1 NARA reserves the right, during business hours and without notice, to inspect the Contractor's processes, procedures, and deliverables during the performance of this maintenance agreement. 8.2 All work must be completed on a schedule mutually agreed upon by the Contractor and the NARA POC. 8.3 No transportation or travel expenses will be paid. (d) Comptroller General Examination of Record. FAR Clause 52.217-8 Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months.
