- THIS REQUIREMENT IS A SMALL BUSINESS SET-ASIDE. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Requirement 2017-11999 is being issued as a Request for Quote. The Offeror must have an active Registration with the System for Account Management (SAM) prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a firm, fixed price purchase order in accordance with FAR Part 15. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-46. The North American Industry Classification System (NAICS) code for this requirement is 621551 (Medical Laboratories) and the size standard is $32.5 million. The Centers for Disease Control and Prevention (CDC) has a requirement for to obtain a large collection of stool samples each year from patients with rotavirus disease so that CDC can monitor the rotavirus strains circulating in the United States. Proposals are due by August 29, 2017 to Maria Shamburger at mshamburger@cdc.gov. All responsible small business sources may submit a proposal, which if timely received, shall be considered by the agency. Questions, if any, should be submitted to mshamburger@cdc.gov no later than 3:00pm EST on August 25, 2017. EMAIL INQUIRIES ONLY. Reference RFQ# 2017-11999. The resultant purchase order will be Firm Fixed Price BPA contract with a 36-month period of performance. The Government intends to evaluate offers and award a contract without discussions with Offerors providing the requested products/services. The initial offer should contain the offeror's best terms from a technical and price standpoint. Interested vendors capable of furnishing the government with the requested services listed in the attachment for this synopsis should submit a proposal to the email address listed above. The proposal shall include the itemized price and descriptive literature of the product/services proposed. The offeror must be able to provide all of the requested items listed in the attached document (Attachment 1). Evaluation factors are, Price, Technical Plan, Management Plan and Similar Experience.
