AMENDED TO UPDATE REQUIREMENTS AND EXTEND END DATE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-VA250-17-Q-1042 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/ 01-19-2017. The North American Industry Classification System (NAICS) number is 339113 and the business size standard is 750 employees. STATEMENT OF WORK Lead Garments & Accessories C.0. SCOPE: The Vendor shall independently measure employees for lead garments and accessories fitting on-site at the location of employee s department at two locations. All lead garments and accessories shall be 0.5 lead equivalent and lightest weight possible, except for glasses 0.75 mm lead equivalent. Provide lead garments & accessories within 30 days of award. b. Provide proof of lead garments & accessories efficacy. (3rd party vendor testing of products). The proposed contract listed here is Set-aside 100% for Small Business. FOB destination. Deliver to 5500 Armstrong Road, Battle Creek, MI 49037.
