- This is a sources sought announcement only and is not a request for proposals or quotes. The Government does not intend to make an award on the basis of this Sources Sought Notice or otherwise pay for the information solicited herein. This notice is for the purpose of identifying potential sources and does not commit the Government to make an award and /or issue a solicitation. The Department of Veterans Affairs, VA Medical Center, Tuscaloosa, AL is seeking sources for a potential basic Ordering Agreement (BOA) to provide Homeless Veterans Halfway Housing Services for the Tuscaloosa VA Medical Center. Contractor shall provide services to the beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs and are homeless with substance abuse and/or other mental health diagnosis. The BOA period is for one (1) year from the date of award with four (4) one-year option periods, subject to the availability of funds. This solicitation adheres to the format defined in FAR Part 15 for the acquisition of commercial services and shall be awarded in accordance with FAR Part 15. A partial DRAFT Statement of Work is provided to assist the Department of Veterans Affairs in conducting market research of industry to identify potential contractors to provide the services identified. The applicable NAICS code is 623220 Residential Mental Health and Substance Abuse Facilities, with a small business size standard of $15M. Firms/Companies with interest and capability responding to these requirements should develop a short narrative including relevant documentation, specific evidence and any other information indicating the capability to successfully fulfill the aforementioned requirements. Interested parties should send their company s information via e-mail to the Contracting Officer, Estella Midy, at Estella.Midy@va.gov. Company information must include business type e.g. Service Disabled Veteran Owned Business, Woman Owned Small Business, etc. Information shall be provided no later than 4:00 PM EST, September 15, 2017. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s.) STATEMENT OF WORK 1. GENERAL RESPONSIBILITIES Contractor services will be furnished to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs (VA). The intent of this contract is to provide stable housing for seriously mentally ill veterans who are participating in the Healthcare for Homeless Veterans (HCHV). Unless specifically included in this contract, the monthly rate established will include the services listed in this document and will also include all services or supplies normally provided other persons by the facility without extra charge. Contractor to provide up to 12 beds with a monthly rate based on the complex care needs for each veteran for each bed used. Contractor shall bill only the number of beds that are filled each month (if a veteran did not occupy the bed for the entire month, the rate would be pro-rated based on the number of days the veteran actually resided in the home, by dividing the monthly rate by the number of days in that month). The Contractor will provide the following services in accordance with the requirements of this contract. The Contractor shall provide services to beneficiaries for who such care is specifically authorized by the Department of Veterans Affairs Tuscaloosa VA Medical Center (TVAMC), Tuscaloosa, Alabama. The Contractor shall furnish lodging services for the sum quoted. The Contractor shall furnish each veteran authorized care under this contract with room, board, 3 meals per day 7 days per week plus snacks, laundry services, transportation to and from outpatient appointments, medication monitoring, monitoring for changes in behavior/mental or physical condition. In addition to providing all services outlined for general CRC homes, the contractor will place emphasis on close supervision of these veterans due to their individualized complex care needs. The 12 lodging beds will be located at the CRC home in a designated space solely for use by veterans. The home must be located in the 35473 zip code. Referrals to the home for services will be developed from the Department of Veterans Affairs Tuscaloosa VA Medical Center (TVAMC), Tuscaloosa, Alabama. The expected length of stay will be determined by the veteran s treatment team, with an emphasis on stable housing to provide therapeutic services for seriously mentally ill veterans in the least restrictive environment. The HCHV and/or CRC staff will notify the Janet Lee home if a veteran is not eligible for VA benefits or interested in participation with the program. At that point, the TVAMC would no longer fund the veteran s lodging at the home. The Contractor will notify the TVAMC immediately when a medical emergency occurs, and make the appropriate referral to the local healthcare facility. The Contractor shall notify the TVAMC of absences from the facility. Absences in excess of forty-eight (48) hours shall not be reimbursable without prior approval by the TVAMC (CRC Representative). Facility is required to undergo and pass annual inspection given by Tuscaloosa VA Medical Center s CRC Inspection Team.
