- Limited Source Justification For Acquisitions under the Simplified Acquisition Threshold (Simplified Acquisition Threshold is $150K) 1. Description of Action: Requirement: 0 New 1 Repeat Pricing: 1 Firm-Fixed Price 0 Time & Materials 0 Cost Funds: 0 OMA 0 Other Funds: _ DHP __ Total Dollar Value: $38,048.95 Name of Intended Contractor: Government Scientific Source, Inc. Street Address: 12351 Sunrise Valley Drive City, State, Zip: Reston, VA, 20191-3415 Phone: (703) 734-1805 3. Description of Supplies/Services: This requirement is for one Automated Blood Bank System to include reagents, consumables, and maintenance of the equipment. The Automated Blood Bank system shall be provided at no costs to the Government and only the requested reagents by Womack Army Medical Center Pathology Department will be paid for by the Government. A list of reagents is included in the Performance Work Statement. Period of Performance: Oct 1, 2017 through December 31, 2017. The estimated total cost of this requirement is $38,048.95 4. Authority Cited: 0 (a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); 0 (b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR 0 (c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR 0 (d) FAR 8.405-6(a) (1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR 1 (e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures. 0 (f) FAR 19.804-2. 5. Reason for Authority Cited: The Automated Blood Bank Analyzer and Reagents are needed in order to provide testing of patients while a long term contract containing a base period and four 12 month option periods is awarded. The blood reagents are highly sensitive and Government Scientific Source, Inc. can only be held responsible for correct testing results of testing that is done on the testing analyzer provided by Government Scientific Source, Inc. The testing equipment (Automated Blood Bank Analyzer) required to perform blood testing is already in place at WAMC. Failure to provide the required reagents and consumables would put WAMC in a position to jeopardize the health of beneficiaries requiring various testing. 6. Actions to Increase Competition: There are no actions being taken to increase competition for this requirement. The follow on contract associated with this requirement will be posted on the Federal Business Opportunities (FedBizOpps) to increase competition. 7. Market Research: Market Research was conducted, using the techniques identified in FAR Part 10, which considered a review of Government databases for relevant information. Market research on GSA Advantage shows that there are twenty three other Contractors listed in Group 66, Part III, Scientific Equipment. However time does not allow WAMC to obtain pricing for new blood reagents, train WAMC staff on a different testing analyzer and evaluate testing analyzer, validate testing analyzer and remove and replace testing equipment already in place at WAMC. 8. Procurement History: a. Previous Contract or Purchase Order Number: Contract W91YTZ-08-F-0017, 1 October 2009 through 30 September 2014 at a total cost of $422,613.00. Contract W91YTZ-14-F-0109, 01 October 2014 through 30 September 2017 at a total cost of $324,204.45. or 0 N/A b. Previously competed? 0 N/A 0 No 1Yes The competitive status of W91YTZ-08-F-0017 was intent to sole source. The competitive status of W91YTZ-14-F-0109 was limited sources (GSA Buy) two quotes were received. The authority for W91YTZ-14-F-0109 was 8.405-6(a) (1) (i) and (b) (1). c. Subcontracting Plan is not required in accordance with 52.219-9. 9.
