Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation in its entirety; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) under solicitation number VA262-17-Q-1864. The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-74 & 75 effective May 30, 2014 & June 24, 2014, respectively. The solicitation is in accordance with FAR Part 12, 13, and 19, Acquisition of Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. This is a 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside. The North American Industrial Classification System (NAICS) code for this requirement is 238330; size standard $14 million. The Department of Veterans Affairs Veterans Integrated Services Network (VISN) 22, headquartered in Long Beach, California has a requirement for flooring replacement from carpet to luxury vinyl tile (LVT) at the Fisher House, at VA Greater Los Angeles Healthcare System (VAGLAHS):The Contractor shall provide the product at the Greater Los Angeles (11301 Wilshire Blvd. Los Angeles, CA 90073). SCOPE OF WORK The Contractor shall provide all labor, materials and parts as required to provide new flooring in office, elevator and bedroom suites. Existing floor finish is soiled and worn out from heavy use has never been updated since it opened 10 years ago. This project consists primarily of new LVT to be delivered to the above location. Final approval of finishes from Designer required before placing orders. Do not use VA dumpsters for disposal of scrap/waste without prior VA approval. TASKS Installation- Multiple phases Demo carpet, Minor floor prep, Adhesive Labor to install LVT. Delivery is to occur between the hours of 8:30 am and 12:00pm, during normal working hours.
