- Notice of Intent to Sole Source Naval Medical Center San Diego (NMCSD) intends to award a sole source purchase order under the authority of FAR 13.106-1(b), due to only one source being available to meet the Government's needs, to Audiology Systems Inc at 50 Commerce Dr Ste 180, Schaumburg, IL 60173-5312. Reference number is N0025917N0105. Requirement is for an audiometric exam booth capable of delivering tele-audiology services. The booth needs to be delivered and installed at Naval Branch Health Clinic (NBHC) China Lake located at 1 Administration Circle, Bldg. 1403, Stop 1311, China Lake, CA 93555. Delivery and installation is required within 120 days of contract award. Audiology Systems Inc is the only vendor that meets all the requirements that are needed by NBHC China Lake as listed below: The booth needs to fit the measurements required by the facility as follows: 74.28" length by 63.38" width by 97" height. The audiometric exam booth needs to be outfitted for telehealth audio and video. Jack panel requirements include: XLR (female - female) jack, 1/8" stereo (female - female) jacks, HDMI (female - female), Ethernet (female - female), USB 2.0 [A (inside) - B (outside)], USB 3.0 [A (inside) - B (outside)]. The booth needs to meet the ANSI S3.1-1999 (Revision of ANSI S3.1-1991) American National Standard Maximum Permissible Ambient Noise Levels for Audiometric Test Rooms. This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the compatible air-fluidized beds for rent. Capability statements shall not exceed 5 (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order. Capability statements are due by 0700am PST, 30 Aug 2017.
