- Attachment 6: SF25A Attachment 5: SF25 Attachment 4: Pre-Proposal Inquiry form Attachment 3: Renovation Specs Attachment 3: Drawings General Decision No. AZ170028 Solicitation See attached RFP and detailed Instruction to Offerors, Statement of Work, and applicable drawings and specs Purpose: This synopsis is issued to provide a notice to the public for project information to be performed in support of the Indian Health Service (IHS). IHS is seeking a qualified Contractor to renovate the Primary Care, Emergency, Physical Therapy and Specialty Departments at the existing Whiteriver Indian Hospital located on the Fort Apache Indian Reservation in Whiteriver, AZ 85941. Description: The proposed contract listed here is 100% set-aside for small business concerns. The Government will only accept offers from small business concerns. All other firms are deemed ineligible to submit offers. A Pre-proposal/Site Visit is currently scheduled for 30 August 2017 at 9:00 a.m. MST, and will be held at the Whiteriver Indian Hospital, Whiteriver, AZ 85941. There will be a site visit held for each seed project and they will be held consecutively, immediately following the pre-proposal conference. Interested Contractors are highly encouraged to attend the pre-proposal conference. Background : Whiteriver Indian Hospital is located at Whiteriver, AZ on the Fort Apache Indian Reservation. This hospital is located about three miles north of Whiteriver and serves approximately 17,000 tribal members and other Native American communities around the area. The hospital is a 45-bed facility, which is designated as a Baby Friendly Hospital. The hospital is JCAHO-accredited and staffed by 22 physicians, podiatrist, five nurse practitioners, five dentists, two optometrists, and about 79 nursing staff members. Whiteriver Hospital is also the base for major research projects that include gastroenteritis and pneumococcal diseases. The Scope of Work shall provide all necessary services to renovate the Primary Care, Emergency, Physical Therapy and Specialty Care Departments of the Whiteriver Indian Hospital. The renovation work will require the demolition and reconfiguration/reconstruction of these existing Departments. Work includes: Building structural, mechanical, electrical, plumbing, computer/internet and phone networks, security and access control systems, medical air and gas systems, fire protection, fan wall installation in central air handlers, installation of new medical equipment and miscellaneous casework and fixtures. In accordance with FAR 36.204(f) Disclosure of the magnitude of construction projects, the estimated cost range of this construction requirement is between $1,000,000 and $5,000,000. The North American Industry Classification System (NAICS) Code is 236220 Commercial and Institutional Building Construction, with a Small Business Size Standard of $36.5 million. Competitive Acquisition: It is anticipated that this requirement will be procured in accordance with FAR Part 15, Contracting By Negotiation and subject to FAR Part 36, Construction and Architect-Engineer Services. Contract Type: Indian Health Service intends to award a Firm Fixed Price (FFP) Construction Contract in support of this requirement. Period of Performance: The period of performance is anticipated to be 120 calendar days from the date of notice to proceed. IMPORTANT NOTICE : In formulating the Qualifications submission, potential respondents should consider that FAR 52.219-14, which will be incorporated into any resultant contract, requires the Prime Contractor to perform services under the contract equivalent to 15% or more of the contract value in personnel costs throughout the life of the contract.
