- STATEMENT OF WORK Generator Maintenance Contract VA Central Western Massachusetts, Leeds, MA 01053 GENERAL A The VA Central Western Massachusetts (CWM), located in Leeds, MA, has a requirement for a contractor to furnish all labor, parts, tools, and equipment to perform preventive maintenance/repair and emergency repair response on all emergency generators located on site, for a base year and four potential 12 month options. This contract is necessary to comply with Joint Commission Standards, VHA Directives, and NFPA 110. The contract will be for eight (8) generators (1 450KW Cummins, 5 350KW Kohler, 1 350KW Caterpillar, 1 150KW Onan). B The scope of work consists of annual preventive maintenance, repairs, and 7-day per week, 365-days per year, 24 hour emergency service. C The contractor shall provide all resources required in performing these maintenance services and inspections in accordance with the original equipment manufacturer specifications (OEM) to include but not limited to all parts, materials, lubricants, fluids, filters, equipment, tools, shipping, travel, and labor. Contractor shall be responsible to determine the nature and extent of any work required and to restore equipment to working and operational condition, as determined by VA, replacing parts as necessary. PROJECT SCOPE OF WORK A REFERENCES: All generator operations, tests, maintenance, alterations and repairs performed under this contract shall comply and be in accordance with the OEM s guidelines and standards for an Emergency Power Supply System (EPSS) as follows: Joint Commission Standard EC 7.40 2. National Fire Protection Association 110, Emergency Generator Testing Requirements Contractor shall follow (OEM) requirements and specifications on inspection, maintenance, and operation of each emergency generator. Contractor shall be responsible for following all State, Federal and EPA codes and requirements including proper use of Personnel Protective Equipment (PPE). The contractor shall follow all of this facilities safe work process & procedures. B PREVENTIVE MAINTENANCE (PM): Test, evaluate, repair and service all components of the generators on an annual basis to accomplish the following: Ensure their safe, reliable and continued operation. Prevent breakdowns due to worn parts. Maintain generators above the point where deterioration begins. Contractor shall be responsible for proper disposal off site of all debris, fluids and other general or hazardous waste collected as part of the service. Contractor shall follow all applicable Local, State, and Federal and EPA laws/regulations pertaining to proper disposal. All work will be coordinated with the contracting officer representative (COR). Preventative maintenance service may include (subject to OEM specifications and industry standards), but is not limited to: Replacing lube, oil, and filters Replacing fuel, coolant, and air filters Check coolant level and condition, protection rating and perform pressure test Inspect hoses and belts, adjust belts as necessary Inspect fuel supply system for any signs of leaking or deterioration Inspect exhaust system and condensation drain Check batteries and clean and protect terminals Check and adjust all engine and generator, shutdown and alarm systems Adjust voltage and frequency and confirm proper operation of gauges and AC DC metering systems Inspect transfer switch main contacts Check day tank and transfer pump for proper operation Check block heater operation and temperature Load testing During all PM Service, any specific device cleaning, lubrication, adjustment or part replacement, which is performed, shall be indicated on an accompanying service report and given to the COR within 7 calendar days upon completion of the PM service. C REPAIRS AND EMERGENCY RESPONSE/REPAIRS: Repairs shall be made using a firm fixed price labor rate (fully burdened) and any parts/materials shall be paid at actual cost. If the repairs cannot be completed within twenty-four (24) hours, the contractor shall present a completion plan/schedule to the COR for approval. The emergency service shall include up to 40 hours of emergency on-site service to include a firm fixed price labor rate (fully burdened) and any parts/materials will be paid at actual cost. The contractor shall respond to the site within 4 hours of being notified of a generator emergency. The contractor shall have the resources available to correct any generator emergency issue when requested. The generators directly impact patient care and are a necessity to the hospital to remain in fully operational and in good working condition. If the emergency Generator condition cannot be corrected or repaired immediately, the contractor shall make the condition safe by Lockout/Tagout, disconnect, or isolate it from service, so that the condition will not cause any further issues or damage to other operating systems, patients, staff or property. The contractor's personnel shall report to the COR, or authorized representative, upon arrival at the medical center and again upon completion of the required work. A copy of the work ticket containing a complete description of the work performed shall be given to the COR within 7 calendar days. Additional Repair Services /Parts This contract includes the option to purchase additional repair services/parts not covered under the final contract for preventative maintenance. Costs of repair, replacement of parts or materials determined to be defective during testing shall be on an as-needed basis as determined by the servicing technician and the COR. All deficiencies and defects shall be itemized and reported as cost of repairs. The pricing/quote for the additional repair services shall be requested at the time of need, in accordance with the labor rates provided below and all work shall be preapproved by the COR and Contracting Officer (CO) prior to commencement of work. Please note, the amount of emergency repairs is unknown at this time. These services are not guaranteed and will fluctuate based on the need and severity of the situation. Based on the cost of the additional repairs/services and prior to commencement of work, all additional work shall be authorized via a Government Purchase Card transaction, a dually signed modification or via an additional separate order accompanied by an authorized obligation number. Additional Repair Services/Parts Labor Rates: Emergency labor hourly rate for normal business hours $ Emergency labor hourly rate for after hours and Saturdays $ Emergency labor hourly rate for Sundays and Holidays - $ Parts Discount (if applicable): $ All repairs shall be performed to OEM s specifications by a trained technician. A response is required from the vendor within 24 hours when a request is made by the COR. Additional work found during normal service and preventative maintenance, and any work that will require extensive time and materials, must receive prior approval of the Contracting Officer. All work shall be coordinated through the Contracting Officers Representative (COR).
