Description
This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources, small and other than small businesses, to perform a potential requirement. Respondents are required to be registered and in good standing in The System for Award Management (SAM) https://www.sam.gov/portal/SAM/#1 Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 541380 and sized standard is $15M. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and Center for Veterans Enterprise (CVE) verified in VetBiz Registry www.vetbiz.gov. Project requirements: To provide annual inspection and testing for medical gas systems. See Description of Services Section. Anticipated period of performance: October 1, 2017 through September 30, 2018 plus four (4) one-year options. Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide the services required in the Description of Service Section. Include any certifications/qualifications for accomplishing the annual inspecting and testing of the medical gas systems; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, small, etc.,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format. Description of Services Medical Gas Testing A.1 INTRODUCTION This requirement is for annual inspection and testing of the medical gas systems. A.2 BACKGROUND Annual requirement. A.3 SCOPE OF WORK The Contractor shall furnish all labor, supervision, and material to repair, inspect and test medical gas systems. A.4 U.S. LEGAL HOLIDAYS - The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday. Normal business hours are from 8a.m. to 4:30p.m. C.S.T. WORK REQUIREMENTS The medical gas testing shall include the testing of every medical gas outlet in the facility. The medical gas systems to be tested shall be the OXYGEN, MEDICAL AIR, NITROUS OXIDE, VACUUM and NITROGEN. Each medical gas outlet tested shall be purged at a minimum flow rate of 3.5 SCFM (100 LPM). Any medical gas outlets found to be defective shall be noted in final written report with recommendations. Testing shall include total evaluation of the following: FLOW RATE and OUTLET PRESSURE CGA E -10 6.4.3.1, & 6.4.3 PURGE FOR PARTICULATE CGA E-10 6.4.3.9 OUTLET INTEGRITY (RANDOM) CGA E-10 6.4.3.4 DEW POINT ANALYSIS CGA E-10 6.4.3.8 ZONE SHUT-OFF VALVES CGA E-10 6.4.2., & 6.4.2.3 & 6.4.2.4 VACUUM PUMP(S) CGA E-10 6.4.5.1 through 6.4.5.5 MEDICAL AIR COMPRESSOR(S) AND DRYER SYSTEMSS CGA E-10 6.4.5.1 through 6.4.5.5 MEDICAL GAS ALARMS (AREA & MASTER) CGA E-10 6.4.1 through 6.4.1.4 MEDICAL GAS MANIFOLDS CGA E-10 6.4.4.1 through 6.4.4.4 *CO & DEW POINT MONITORS CGA E-10 6.4.12 HIGH PURITY TESTING Medical Gas HIGH PURITY Testing is done with an Infrared Spectrometer. Testing shall be performed on a random outlet basis at selected end points with particular attention given to critical care areas. OXYGEN, MEDICAL AIR and NITROUS OXIDE shall be analyzed at each medical gas zone for: gaseous hydrocarbons halogenated hydrocarbons (as methane) carbon dioxide dew point particulate matter (4.5-micron filter) INSTRUMENTATION - Used to test on site shall include an infra-red spectrometer, oxygen analyzer, dew point monitor, and other instrumentation as specifications for both air and oxygen. Written reports shall reflect how the measurements were made, where they were made and compared with specified standards for each pollutant. Operating pressures and flow rates shall be measured and recorded for each outlet tested with locations noted on the written reports. PARTICULATE - Testing shall be done at selected end points on each zone through a 4.5-micron filter testing systems at a flow rate of 3.5 Standard cubic feet per minute (SCFM) for one minute. Filter samples shall be bagged, tagged, and turned over to the hospital. VACUUM - Inlets shall be tested by verification that suction does not drop below 12 inches of mercury when at least 3 cubic feet per minute (CFM) (or the maximum flow the inlet shall allow) are drawn from an adjacent or neighboring inlet. The written report shall reflect location, final pressure (Hg) and satisfactory operation of each outlet tested. NITROGEN - Outlets shall be purged at a flow rate of 5 SCFM for a least 30 seconds, checked for correct operating pressure, checked for particulate, and verified for specific gas as labeled. Random particulate testing shall be done at selected end points on each zone through a 4.5-micron filter at a flow rate of 3.5 SCFM for one minute. Filter samples shall be bagged, tagged, and turned over to the hospital with the final report. The written report shall reflect location, final pressure, specific gas, and particulate results. ZONE SHUT-OFF VALVES - Medical gas shall be tested for leaks, gauge pressure, and proper labeling. If zone valves are not labeled, the location of area served shall be so noted on the written report. All zone valve written reports shall reflect locations, area served, labeling, and specific gas for each valve tested. MEDICAL GAS ALARMS - Shall be checked and/or tested for all audible and visual signals, high alarm set point, low alarm set point, and correct operating pressures. Written reports shall reflect location; areas served, alarm function tests, set points, and correct labeling. MEDICAL GAS MANIFOLDS - Shall be tested for proper alarm functions, bank switch-over capability, and alternating sequence, correct gauge indication (not calibration), gas leakage, and relief valve operation. Written reports shall reflect location, correct manifold operation and labeling. VACUUM and MEDICAL AIR - Systems shall be checked for code compliance, proper suction or pressure delivery, automatic and/or manual alternation, proper voltage and amperage draw. Refrigerated air dryers, dew point monitors, and carbon monoxide monitors shall be checked for code. compliance, alarm capability, and correct operating pressure/temperature. Additional tasks: Upon completion of systems inspection, a written report (3) three copies shall be given to the COR. The report shall list all inspection criteria to include all recommendations and discrepancies. At this time a quote shall be submitted for the correction of the discrepancies submitted by area, section of facility, and department of hospital. Contractor shall apply for all applicable construction permits. All work shall be completed in accordance with local, State and Federal regulations (including the local Fire Marshal). Contractor shall provide daily cleanup of work area and trash removal from site. SUPPORTING INFORMATION B.1 Place of Performance VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104. B.2 Special Considerations B.2.1 Contractor Furnished Materials All labor, materials, equipment to perform the job. When furnish, provide, install, or similar term is used it shall mean a complete installation, ready for use. B.2.2 Government Furnished Materials and Services. Government furnished property, if any, shall be identified and shall only be used for the performance of this contract unless otherwise authorized by the Contracting Officer. The Contractor shall, at all times, take any and all steps necessary for maintenance and preservation of all Government-furnished property. The Contractor shall comply with all reasonable requests of the Contracting Officer to enclose, or specifically protect, Government-furnished property. B.2.3 Qualifications of Key Personnel Contractor s employees should be knowledgeable of standards associated with this type work. B.2.4 Supervision/Communications. At all times during the performance, the Contractor s Superintendent (if applicable) shall be available by cellular phone. Prior to beginning any work, the Contractor shall supply the COR with the telephone number for the Superintendent. The Contractor s Competent Person that meets OSHA training guidelines, is capable of making decisions and acting on behalf of the Contractor shall be physically located on site at all times during performance of the contract. If the Contractor s Competent Person is absent from the job site for an extended period of time, the COR may send all Contractor/sub-contractor employees off the job. B.2.5 Security Requirements: Contractor shall identify which employees shall be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, subcontractor employees, suppliers and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall be worn above the belt at all times while on Medical Center property. Contractor s employees shall return all I.D. badges to designated personnel at time designated in the task order. B.2.6 The Contracting Officer s Representative (COR) is responsible for the inspection of the work called for in this job. The Contracting Officer s Representative shall be identified in the task order by name, telephone number and location. Inspections made by the COR are for the sole benefit of the Government and do not relieve the Contractor of any quality control responsibilities. B.2.7 The Safety Manger is responsible for safety inspections of all contract operations. The Safety Manager shall be identified in the task order by name, phone number and location. B.2.8 Drawings General. Contract drawings (if applicable) shall indicate the extent and location of work. If any departure from the contract drawings are deemed necessary by the Contractor, the details of such departure and the reasons therefore shall be submitted to the Contracting Officer for approval as soon as practical. No such departure shall be made without proper written permission of the Contracting Officer. The Contractor is responsible for all information and requirements in drawings and specifications that pertain to the task order. B.2.9 Verification of Dimensions The Contractor shall visit the job site to thoroughly familiarize himself with all the details of the work and working conditions. The Contractor shall also verify all dimensions in the field and shall advise the Contracting Officer of any discrepancy before performing any work. The Contractor shall be specifically responsible for the coordination and proper relation of his work to the building, structure, phasing and ensure the safety of employees and workmen. B.2.10 Smoking Policy. The Contractor shall not allow smoking in any building. The Contractor shall ensure employees smoke only in designated outdoor areas, 50 feet away from buildings. B.2.11 Parking/Traffic Regulations. The Contractor shall not park on grassy areas unless approved by the COR and the Contractor agrees to restore areas back to VA standards. Drivers should be particularly concerned with pedestrian traffic. Yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are to be observed by operators at all times. Seat belt use is mandatory on the VA grounds. Contractor shall be able to review the VA shop drawings for reference as required to complete this work. Return any shop drawings to the COR once the work has been completed. Contractor and their staff are not permitted to take any pictures, videos or drawings of any type. Contractor and their staff are not permitted to take any pictures, videos or drawings of any type.  Taking of photographs/videos are prohibited while on VA premises. The contractor s staff shall check in with the VA police to receive a VA work authorization badge. The badge  shall be  worn facing forward and above the waistline. The contractors staff are required to wear the VA issued badge at all times while on VA premises. TRAINING: The contractors staff (those who shall be on VA property working) shall be required to complete the TMS (VA 20939) training prior to the work beginning. The contractor shall either personally bring the training certificates or E-mail the certificates to the COR so they can be kept on file for one year. You shall ensure each contract employee self-enrolls for a profile on the VA TMS by visiting https://www.tms.va.gov/plateau/user/login.jsp. Once there, employees should follow the steps below to create a profile, launch the mandatory training, and complete the content prior to their next day at VA. Upon completion by all employees, the contractor shall provide their Contracting Officer Representative with each employee s printed certificate of completion from the TMS. This certificate displays the employee s TMS User ID. The TMS training is only good for one year and the contractor is responsible to ensure their staff  or persons who shall be working on VA property are current.
