Description

This is a sources sought notice only. The purpose of this sources sought notice is to gain market knowledge of potential businesses to provide Primary Care Physician services (see description of services and qualification requirements below.) The applicable NAICS Code is 561320, with a small business size standard of $27.5 million. All businesses capable of providing this service are encouraged to reply, as responses will be used by the Government to make determinations about the appropriate set-aside. No solicitation is available at this time. After review of the responses to this sources sought announcement, a synopsis and/or solicitation announcement may be published at a later time. Description of Services: The VA Sierra Nevada Health Care System (VASNHCS) has a requirement for a contractor to provide a Board Eligible or Board Certified candidate in the practice of Internal Medicine or Family Practice services to participate as part of a Patient Aligned Care Team (PACT). The practice is responsible for providing for all of a patient s health care needs or appropriately coordinating care with other qualified professionals. It is a model of practice in which a team of health professionals, led by a provider, works collaboratively with the patient to plan their overall health. Contractor s physician(s) shall be available and present in clinic during normal VAMC clinic hours which will be established, and may be revised, as deemed appropriate for patient care by the Chief of Staff. Currently, normal clinic hours are 8:00 AM to 4:30 PM. Qualifications: License: The contractor s physician(s) assigned by the contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property. Board Certification: All contractors physician(s) shall be board certified or board eligible by the American Board of Internal Medicine or Family Practice.
