- Pricing Schedule PAST PRESENT PERFORMANCE EVALUATION RESPONSE PAST PRESENT PERFORMANCE EVALUATION COMPANY INFORMATION Attachment 2 App. C Armor Sizing Templates Attachment 1 Delivery Locations Requirement Request for Quotation, Description of Requirement, Instructions and Evaluation Factors DESCRIPTION OF REQUIREMENT (See Attachment for full details): Approximately 4,800, not to exceed a count of 5,500, External Body Armor Carriers requiring accommodation of current issue body armor ballistic plates. CLIN 0001 - Initial order quantity approx. 4800 CLIN 1001 - Optional orders 30 day Fit Guarantee. If External Carrier does not fit properly, the offeror must provide a refit and/or resize option to the Government. Provide manufacturer warranty for 12 months. Shipping delivery to requested Federal Judicial Facilities provided in attachment. Initial order delivery within 21 days after receipt of order. Option orders delivery is expected no later than 30 days after receipt of order. INSTRUCTIONS TO OFFERORS (See Attachment for full details): Completed quotations shall be delivered electronically to osc.procurements@usdoj.gov by 12:00 PM (NOON) EST Wednesday, September 6, 2017. Subject line RFQ External Body Armor Carrier Quote - "Company Name". Quotations shall be considered binding for potential award for up to 90 calendar days after close of the request for quotes. Offerors must have active System for Award Management (SAM), www.sam.gov, registration. Quotations shall contain the following information, Price, Technical Approach, and Present/Past Performance Information. Incomplete quotes will not be considered for award.
