- RFQ Wage Determination Description: i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. ii. The solicitation number FA4528-17-Q-WT69 is being issued as a request for quotation (RFQ). iii. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. iv. This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 812990 with a size standard not to exceed $7.5 million (U.S.) v. 0001 Provide non-personal Group Fitness Instructor Services in accordance with attached statement of work No Later than (NLT) 30 Sep 2017 - 29 Sep 2018. 1001 Provide non-personal Group Fitness Instructor Services in accordance with attached statement of work No Later than (NLT) 30 Sep 2018 - 29 Sep 2019. 2001 Provide non-personal Group Fitness Instructor Services in accordance with attached statement of work No Later than (NLT) 30 Sep 2019 - 29 Sep 2020. 3001 Provide non-personal Group Fitness Instructor Services in accordance with attached statement of work No Later than (NLT) 30 Sep 2020 - 29 Sep 2021. See attached Request for Quotation. vi. Place of services is McAdoo Fitness Center 220 Tanker Trail, Minot AFB, ND 58705. vii. The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. viii. The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The government will award a firm-fixed price contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The other factors considered will be the ability to demonstrate the capacity to fulfill this requirement in accordance with the attached performance work statement. Award shall be based on best value to the government. Technical Acceptance Criteria will be evaluated on a pass/fail basis. In order to be technically acceptable the Offeror must: a. FACTOR 1 - RECRUITMENT/HIRING - This sub factor is met when the Offeror provides a detailed method of recruiting, hiring, and retaining qualified personnel who will be able to perform the service identified in the Performance Work Statement (PWS). b. FACTOR 2 - QUALIFIED CANDIDATES - This sub factor is met when the Offeror identifies candidates that possess the qualifications stipulated in the PWS and are ready, willing, and able to perform the required duties at Minot AFB, ND according to the period of performance established in the solicitation. ix. The government advises Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Offerors are also reminded to complete Offeror Representations and Certifications on-line at www.sam.gov prior to submitting an offer. x. Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. xi. Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.204-10, 52.209-6, 52.219-6, 52.219-28, 52.222-3, 19, 21, 26, 35, 36, 37, 52.223-18, and 52.232-33. xii. Clause 52.222-41, 42, 43, Service Contract Act of 1965 is applicable, and the additional FAR clauses and provisions applicable: 52.204-7, 52.204-9, 52.223-5, 52.223-6, 52.223-19, 52.225-13, 52.232-18, 52.232-40, 52.247-34, 52.252-1, 52.252-2, and 52.253-1. DFARS Clauses: 252.203-7000, 252.203-7002, 252.204-7003, 252.209-7999, 252.223-7006, 252.225-7001, and 252.232-7003. AFFARS Clauses: 5352.201-9101, 5352-223-9000, 5352.223-9001, 5352.242-9000, and 5352.242-9001. xiii. There is no Defense Priorities and Allocations System (DPAS) rating applicable for this procurement. xiv. All offerors must submit a quote by 10:00 am Central Standard Time on Monday, 30 Aug 2017. Quotes received after that time may not be considered. xv.
