AMENDMENT #1 - MARKET SURVEY FOR FEDERAL AVIATION ADMINISTRATION (FAA) The purpose of Amendment #1 for the Market Survey is to provide the Government Response to public inquiries and change Section 2 of the Market Survey. Please refer to the revised Section 2; and Amendment #1 when providing Capability Response packages. FAA Headquarters Fitness and Wellness Services In accordance with Federal Aviation Administration Acquisition Management System (AMS) Procurement Guidance 3.2.1.2 “ the purpose of this announcement is to conduct a Market Survey for the purpose of soliciting statements of interest and capabilities from interested vendors. This market survey also seeks to identify potential vendors for this contract including groups of business teams. This market survey is being conducted in order to obtain the information necessary to aid the FAA in its determination as to whether adequate competition exists to set-aside the competition. The Principal North American Industry Classification System (NAICS) size standard code for this requirement is 56120, Facility Support Services of $38.5M. 1. Objective The Federal Aviation Administration ™s Office of the Assistant Administrator (AHR), Benefits and WorkLife, is conducting a Market Survey to: Inform the industry of the potential contract opportunity and solicit capability statements and other information from interested vendors capable of providing the services set forth in the draft Statement of Work (SOW)Solicit statements of interest and capabilities from qualified and capable small businesses 2. Purpose The Government has determined this requirement shall be a Small Business set-aside. This market survey is conducted in an effort to obtain the information necessary to determine whether adequate competition exist among small businesses, services disabled veteran owned small business (SDVOSB), or 8(a) socially and economically disadvantaged businesses (SEDB). SEDB must be certified by the Small Business Administration (SBA). The FAA will accept industry comments and capability responses from Small Business Concerns. The survey is limited to small businesses. This Market Survey announcement is not a SIR or a Request for Proposals (RFP) of any kind. This survey is for informational and planning purposes only. The FAA is not seeking or accepting unsolicited proposals. This notice should not be construed as a commitment of any type by the Government to take or not take any action. No evaluation letters or results will be issued to respondents. The Government will not reimburse any costs incurred by vendors in responding to this notice. 3. Background The Federal Aviation Administration, in accordance with Public Law 89-554 (5 USC § 7901), has established a health and fitness training program to promote and maintain the physical and mental fitness of their employees. Providing physical fitness education and resources to all FAA employees is an important element of the program. The benefits from past FAA workplace fitness onsite surveys indicated that members have reported using fewer sick leave hours, greater efficiency on the job and improved morale and overall health. The FAA operates its Fitness Center through membership dues and federal funds from FAA lines of business and Staff Offices. The FAA is seeking a Contractor to provide fitness training services and manage the overall operations of the FAA Fitness Center. 4. Description of the Requirement Respondents must be able to meet all requirements of this market survey in accordance with the draft Statement of Work Requirements. The Government may not consider incomplete responses. CAPABILITY STATEMENT “ Interested Parties must provide a Capability Statement demonstrating their past performance experience in all requirements of the SOW, namely Sections 2, 4,5,6,7, and 8. Completion of the following attachments (the package) is required with the submittal of the Vendor ™s information and response to this Market Survey: Completed Business Declaration Form “ Attachment A Small Business Administration 8(a) Certification letter, if applicable Veterans Administration SDVOSB Certification, if applicable Cover letter Capability Statement Certification/evidence of active Insurance in accordance with FAA AMS Clause: 3.4.1-12 Insurance (refer to Section 10 of the draft SOW) If the Contractor chooses, an affirmative Statement allowing the FAA to release the company ™s name, point of contact, address and contact information. An affirmative statement must include the following information: An Affirmative Statement that œThe FAA may publically release the following information: [Include the company ™s name] [Include the company ™s Point of Contact name and title, email and phone number] [Include the company ™s address]  Company ™s responses to this request are elective and their response has no impact on the acceptance or impact of the review of their Capability responses. Companies may decline the release of their information. If companies decline this request, FAA will not release their information. For those companies that agree, after the Market Survey closing date; FAA may publicly announce the names and addresses of Interested parties that responded to the Market Survey. 5. The Government will not respond to inquiries after this date and time. The Government will publish a singular public response to all questions that are received by the deadline. A complete Capability response package for this request (see section four above) must be provided with a cover letter on company letterhead. The Capability Statement is limited to five pages at 12 point font. The Cover letter is not counted in the five page limitation. All response submissions (the package) must be received via email no later than September 8, 2017, 2:00 p.m. EST. Late response packages will not be accepted. The Government is not liable for costs associated with the preparation, submittal of inquiries or responses for this announcement and will not reimburse any firm for cost incurred in responding to this public announcement.
