DEPARTMENT OF THE AIR FORCE HEADQUARTERS 9TH MISSION SUPPORT GROUP (ACC) BEALE AIR FORCE BASE, CALIFORNIA Request for Quote Marriage Retreat BPA (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested. (ii) Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. (iii) This combined synopsis/solicitation is a Request for Quotations (RFQ) and the reference number is Written quotes are required (oral offers will NOT be accepted). (iv) This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-89, effective 15 August 2016, DPN 20160830, effective 30August 2016, AFAC 2016-0603, effective 03 June 2016. Other quote format should list exact models and part numbers quoted on. STATEMENT OF WORK For 548 ISRG & 9 RW Marriage Retreats 1. GOAL: To provide a retreat that is geared towards building stronger marriages and families. This Marriage Enrichment Retreat is designed to take place at an upscale venue in which couples meet in a larger group. Overhead training including discussions are facilitated by the chaplain and by the NCOIC of Chapel Operations. The training modules and discussions are focused on strengthening the couple's relationship. During the intervening breaks between the multiple modules, each couple is provided ample opportunity to more closely interact and bond. 2. DESCRIPTION OF SERVICES: The contractor shall provide lodging, conference space, food, and eating area with the staff necessary to set up, serve, and clean up the area after eating. The contractor shall provide all support needed at their location for retreat services that fulfill the 548 ISR Group and 9 RW Chaplain-led retreat requirements. The retreat requirements include the following task areas: Task 1 - Site Location Task 2 - Points of Contact Task 3 - Lodging Facilities Task 4 - Dining Area and Food Catering Requirements Task 5 - Parking Task 6 - Conference Room 2.1 Site Location: Four Star within 200 miles of Beale AFB. 2.2 Points of Contact: The 548 ISR Group and 9 RW Chaplain Team shall lead and host the retreat. The Chaplain Team shall coordinate with the contractor as needed. The contractor shall provide responsible points of contact to help resolve any problems or concerns that might occur during the retreat. Also, the contractor shall provide 24 hour emergency points of contact for lodging and facility issues. 2.3 Lodging: The contractor shall provide onsite lodging for two nights. Lodging shall be provided for each couple attending the retreat. Check in shall be Friday, and check out time shall be Sunday mid-afternoon. Lodging shall consist of 32 comfortable rooms with king or queen sized beds, private baths, and modern amenities. Each room shall accommodate at least 2 adults. All lodging facilities shall be cleaned according to applicable state law and come equipped with linens, towels, and other supplies adequate for the occupancy rate. Deliverable: List of available lodging, number of beds, occupancy limitations shall be provided in submittal. 2.4 Dining Area and Food Catering: The contractor shall provide buffet style or plated meal style catering service with premium hot meals for breakfast and lunch (two meat option, two vegetables, two starches, salad, and dessert) on Saturday & Sunday. A vegetarian option shall be offered to those who have a dietary restriction. Contractor shall provide coffee/water/juices with breakfast meal and tea/water/soft drinks with lunch. Provide napkins, silverware, cups and all other necessities to facilitate consumption of food and beverages. A coffee/tea, water service shall be provided throughout each day in the conference room. Deliverables: a. The contractor shall provide a menu for the dinner and breakfast in your bid submittal. b. The contractor shall provide a diagram of table and chair arrangements in bid submittal. 2.5 Parking: Contractor is responsible for providing parking for all retreat attendees. 2.6 Conference Room: The contractor shall provide one onsite conference room for training on each day. The rooms are needed on Friday from 6:00 pm to 9:00 pm, Saturday from 9:00 am to 12:00 pm, and Sunday 9:00 am to 11:00 am. The primary conference room shall accommodate a total of 64 adults in a round table configuration. Both rooms shall be furnished with chairs, tables, and a podium. The conference rooms shall have air-conditioning and heating capability for climate control. The conference space shall be set up with Audio/Visual Equipment. The contractor shall provide A/V equipment all three days to include projector, screen, & sound. The contractor shall ensure electrical outlet usage is possible and connection cables are available. Deliverable: Diagram of assigned facilities shall be provided in bid submittal for conference area. 3. GENERAL INFORMATION: The total proposed price shall be all inclusive (include any incidental charges, i.e. conference fees, surcharges, meal prices, etc.). The period of performance dates over the life of the agreement to be worked out between the chapels and the venue. The quantities and charges shall not exceed what is authorized and specified in the contract and associated CLINs. Final numbers of attendees will be provided to the contracted vendor 48 hours prior to the event. Contractor qualifications and specifications: The preferred location for the vendor to be located is downtown Napa Valley. Retreats include lodging, food, refreshments, and conference spaces. Note: Lodging shall be two nights per retreat. The vendor must be capable of providing all services and facilities listed in this solicitation/statement of work on premise. This is a pre-priced Blanket Purchase Agreement for lodging accommodations, meals and conference room space. The supplier shall furnish rooms and services, if and when requested by the contracting officer or authorized caller, from 29 Sept 2017 to 29 Sept 2022. Either party may terminate this agreement sooner with a thirty (30) day written notice. 2. The government is obligated only to the extent of authorized purchases made under the BPA. 3. No individual call under this agreement shall exceed $25,000. An individual purchase can consist of one or more line items, but the total cost per call shall not exceed $25,000. 4. A list of individuals authorized to purchase under this BPA shall be furnished to the supplier by the contracting officer and will be updated as necessary. 5. All services rendered under the agreement, shall be accompanied by delivery tickets or sales slips that shall contain the minimum information: a. Name of supplier b. BPA number c. Date of purchase d. Purchase number e. Itemized list of supplies or services furnished f. Quantity, unit price, and extension of each item, less applicable discounts g. Date of delivery or shipment 6. Technical Capability This is a competitive acquisition for the award of a contract resulting from the responsible offeror whose proposal represents the best value to the Government in accordance with the evaluation factors stated in this solicitation. Award will be based on Lowest Price Technically Acceptable (LPTA). Technical tradeoffs will not be made and no additional credit will be given for exceeding acceptability. The proposal must meet the Government's minumum requirements in order to be determined technically acceptable and to ultimately be considered for contract award. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. As used in this clause-- Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization. Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF). Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system. (b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports. (c) WAWF access. To access WAWF, the Contractor shall-- (1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and (2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site. (d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the "Web Based Training" link on the WAWF home page at https://wawf.eb.mil/. (e) WAWF methods of document submission. Document submissions may be via Web entry, Electronic Data Interchange, or File Transfer Protocol. (f) WAWF payment instructions. (End of clause) 5352.201-9101 OMBUDSMAN (JUNE 2016) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
