Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Include DUN & Bradstreet number on all quotes. The Department of Veterans Affairs in Buffalo, NY 14215, has a requirement to procure Compound Pharmacy Services. (ii) The combined synopsis solicitation number is VA242-17-Q-0274. It s issued as a Request for Quotation. (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular 2005-89 (7-14-2016). (iv) The combined synopsis solicitation is 100% Small Businesses. Offeror shall provide evidence of being an Authorized Reseller (Distributor) for product offered. This can be substantiated with a link to Manufacturer s web site that supports offeror s distributorship or other proof from the manufacturer. The NAICS is 325412 (or applicable NAICS code). Note, interested Small Business quoter must be registered in System for Award Management (SAM) www.SAM.gov prior to being considered for Award. Award will be made to the lowest price technically accepted quote that meets or exceeds ALL requirements of this solicitation (Firm Fixed Price). (and attachements) (v) This RFQ closes 7 Sept 2017 16:00 PM EST submit all quotes by e-mail to Denise.Edmonds@va.gov. Quotes received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition or are deemed to be in the best interest of the Government. If offering a similar product, the vendors are required to provide descriptive literature in sufficient detail to determine if the items quoted are technically acceptable. No grey market or refurbished item(s). The Government reserves the right to make no award at all.
