- RFQ - P1C005 ATTACHMENT IV - TERMS AND CONDITIONS RFQ - P1C005 ATTACHMENT III - PRICING SHEET RFQ - P1C005 ATTACHMENT II - PERFORMANCE WORK STATEMENT (PWS) RFQ - P1C005 ATTACHMENT I - LIST OF CLINICS RFQ HSCG23-17-Q-P1C005 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The U.S. Coast Guard (USCG) Headquarters invites Women-Owned Small Business (WOSB) Firms under the Women-Owned Small Business Program, FAR Subpart 19.15, to provide customized dental appliances for the USCG Health, Safety and Work-Life Directorate (CG-11) in accordance with RFQ Attachment II, the Performance Work Statement (PWS). This acquisition will enter the awardee into a single-award contract with firm-fixed unit prices for customized dental appliances for various Coast Guard medical clinics, as listed in Attachment I: List of Clinics. All questions shall be submitted via email no later than Tuesday, September 5th, 2017 at 3:00PM Eastern Time. Questions submitted via any other method other than email will not be accepted. All questions or concerns regarding this RFQ shall be submitted via email to the Contract Specialist, Kerri Laws, at Kerri.L.Laws@uscg.mil. All quotations shall be submitted via email no later than Tuesday, September 12th, 2017 at 3:00PM Eastern Time. Quotations submitted via any other method other than email will not be accepted.
