- Laserfiche Equipment List Evaluation Criteria for Laserfiche Support Statement of Work for Laserfice Support This is a Request for Quotations prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. Requirement number DENMHPO-393-2017-0003 is issued as a Request for Quotations. The Centers for Medicare and Medicaid Services (CMS) in Baltimore, MD, and the Regional Offices: Dallas, Denver, Kansas City, New York, San Francisco, Seattle, Atlanta, Chicago, Philadelphia, and Boston has a requirement for a contractor to provide Laserfiche upgrades and software maintenance renewals for one year. Detail information is provided in the attachments. The period of performance will be 12 months from the date of award. This is a firm fixed price procurement using simplified acquisition procedures. This procurement is being set aside specifically for small business concerns. The North American Industry Classification System Code (NAICS) is 423430. Responses must be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov or in paper no later than 3:00pm on September 8, 2017 to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd, C2-21-15, Baltimore, MD 21244. Your response shall be no more than 10 pages and shall include the following: 1) Contractor's plan to perform the Statement of Work 2) and Price - fully loaded labor rates and anticipated hours. All responses will be evaluated on a best value basis using the following Evaluation Criteria: 1) Technical Understanding and Approach; 2) Personnel Qualifications; 3) Management Plan; and 5) Price. An award will be made based on the lowest price that is technically acceptable meeting or exceeding the acceptability standards for non-cost factors. All responsible sources must be registered in the 'System for Award Management' (SAM) at wwww.sam.gov. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. Questions should be submitted electronically by September 5, 2017 at 2:00pm.
