- Past Performance Questionnaire Proposal Instructions Pricing Sheet COI Statement of Work Terms and Conditions The purchase, installation, and configuration of this hardware and software will assist CMS in complying with the HSPD-12 directive and allow the ESS and its PAM subsystem maintain its Authority to Operate. 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition is being conducted under the procedures in accordance with FAR 13. Simplified Acquisition Methods. 2. The solicitation number is HHSM-500-2017-RFP-171967 and the solicitation is issued as request for proposal (RFP). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-95. 4. This is a total small business set aside and the associated NAICS code is 541511. 5. This solicitation is for the procurement of agile services. Please see the attached document for more information. 6. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Please see the attached document for addenda to the provision. 7. The specific evaluation criteria to be can be found in the attached document for the evaluation criteria. 8. Offerors to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 9.
