Description

(i) This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA250-17-Q-1132 and is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-95 (Effective January 13, 2017). (iv) This combined synopsis solicitation is Set-Aside for Service Disabled Veteran Owned Small Business (SDVOSB). The North American Industry Classification System (NAICS) code is 339113, and the size standard is 750 employees. Interested Service Disabled Veteran Owned Small Business offerors must be listed and registered in Vetbiz.gov to be considered for award. (v) The contractor shall supply a quantity of five (5) Mass Medical Storage SAS-18P-HG brand-name or equal Endoscope Storage Cabinets with the following characteristics: Metal storage full cabinet 41 wide with hinged doors with glass windows Able to hold 18 large diameter scopes Wall hook system for mounting of cabinets Powder coated metal sloped top 18 depth Forced air HEPA filtration system Replaceable HEPA filter Must meet HEPA standards set by the United States Department of Energy (vi) To be technically acceptable the products quoted shall be brand name or equal to the items listed above and meet or exceed the salient characteristics. If quoting or equal items, contractor shall submit detailed documentation showing how the items meet or exceed the Mass Medical Storage cabinets. (vii) Contractor shall deliver items FOB Destination to the Saint Joseph County VA Clinic, 1540 Trinity Place, Mishawaka, IN 46545. (viii) 52.212-1 Instructions to Offerors - Commercial Items, applies to this acquisition. Addenda to FAR 52.212-1 Instructions to Offerors Commercial Items: Offerors shall e-mail complete quotes to Jeffrey.Rozema@va.gov no later than 4:00 PM EST on Friday, September 8, 2017. Responses shall be on an all or none basis; no partial submissions will be accepted. No grey market or refurbished items. The Government reserves the right to make no award in response to this RFQ. FAR 52.203-18, 52.204-22, and 52.211-6 apply. (ix) 52.212-2, Evaluation - Commercial Items, applies to this acquisition. Award will be made to the offeror whose quote represents the best overall value to the government, considering technical, price, warranty, and delivery time. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items, applies to this acquisition. The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items, applies to this acquisition. The following addenda apply: FAR 52.203-99, 52.219-28, and 52.232-40; VAAR 852.203-70, 852.211-73, 852.219-10, 852.232-72, 852.246-70, and 852.246-71. (xii) 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 are applicable to this acquisition: 52.204-10, 52.209-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-50, 52.223-18, 52.225-1, 52.225-3; 52.225-13, 52.232-33.
