Description
Schedule of Deliverables Scope of Work Amendment 1 of Synopsis/solicitation HHSM-500-2017-RFQ-0025. Per Far 5.101, 5.201, and 13.5, CMS is ammending this synopsis to request capability statements, in leu of proposals. This work was historically awarded on a sole source basisi to the American College of Surgeons, most recently on contract HHSM-500-2016-00066P. CMS is requesting capability statements to help determine if there are other viable vendors that are capable of performing this work. Interested vendors must follow the instructions for submitting capabliltiy statements below: Please provide a brief capability statement for our organization, tailored to demonstrate your relevant experience and/or ability to provide requirements of this magnitude. Please note that if individual capability information does not provide sufficient information for CMS to determine: (1) if the contractor has the potential to successfully perform the work described or (2) has the ability to handle the volume indicated, the evaluation of the information will reflect that the Offeror did not provide sufficient information to make a determination and CMS will not include the contractor for further consideration. The capability statement should also include any anticipated or planned usage of automated or technical solutions to meet the requirements of this contract. It should be denoted as to whether these solutions are currently available or would be developed. To be developed solutions should indicate anticipated timeframes for delivery. Please include the following in your response: Business information: 1. Must be able to certify to a complete absence of any Conflict of Interest (COI), on an organization basis, as well as, principal management and officers of the entity. COI would be any contract or financial relationship with insurance companies/providers 2. Be able to meet FAR 52.219-14, Limitation in Subcontracting requirements 3. Business Information and Identification 4. DUNS No. 5. NAIC Code 6. Company Name & address 7. Type of Business (e.g., 8(a), small disadvantaged, HUBZone, woman-owned, service-disabled veteran owned and veteran owned) as validated via the CCR. All offerors must be registered on the Central Contractor Registration system, located at http://www.ccr.gov. 8. Company point of contact to include name, telephone number and email address 9. Names, phone numbers, emails of individuals who can verify the capabilities/experience identified in the responses. 10. Ability to meet each requirement as specified for program and non-program tasks. 11. Estimate of personnel resources to meet all requirements. This Sources Sought Notice is for information and planning purposes only and is not to be construed as a commitment by the Government. This is not a solicitation announcement for Request for Proposal (RFP) and no contract or task order award will result from this Notice. No reimbursement will be made for any costs associated with providing information in response to this Notice. Respondents will not be notified of the results of the evaluation. All information submitted in response to this announcement must arrive on or before the closing date. Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality: No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
