- WAGE RATES FOR STL. COMMERCIAL CLAUSES SCOPE OF WORK This is a combined synopsis/solicitation for services prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The solicitation number is PNL003. This combined synopsis constitutes a solicitation and incorporates provisions and clauses in effect through Federal Acquisition Circular 2007. This acquisition is restricted. The NAICS is 713940. The contract type will be a firm fixed price service agreement. The government proposes to solicit from the attached scope of work / or / quotes for the attached items. 1. IF you are interested in submitting a quote, please review the enclosed documents. All submittals must be completed and forwarded to this office for consideration of your offer. 2. If you have any questions, pertaining to the Statement of Work, they should be submitted in writing to Andrew.D.Najar@uscg.mil. Questions should be received no later than September 17, 2017. Please use the contractor's request for clarification form. 3. This contract will be awarded as an option year contract. Please provide quote for Base Year plus 4 option Years. Please provide quote based on firm fixed price (FFP) for membership fees. The contract will be set up as follow: Base Year - Oct 1, 2017- September 30, 2018 Option year 1- Oct 1, 2018- September 30, 2019 Option Year 2- Oct 1, 2019- September 30, 2020 Option Year 3- Oct 1, 2020- September 30, 2021 Option Year 4- Oct 1, 2021- September 30, 2022 4. This contract will be awarded based on Lowest Cost Technically Acceptable. The Government will award a contract resulting from this Request for Quote (RFQ) to the responsible offeror whose offer conforming to the RFQ is the Lowest Price Technically Acceptable (LPTA) quote. The lowest price quote will be evaluated on technical acceptability. If it is found to be acceptable, an award will be made. If it is found to be unacceptable, the next lowest price will be evaluated. This process will continue until a technically acceptable offer is identified. BEST VALUE CRITERA AWARD OF THIS REQUIREMENT SHALL BE MADE TO THE OFFEROR WHOSE PROPOSAL OFFERS THE GREATEST VALUE TO THE GOVERNMENT. TECHNICAL FACTORS, PAST PERFORMANCE ARE MORE IMPORTANT THAN PRICE. "Proposals that require Coast Guard members to sign a usage waiver that holds the vendor harmless from any injuries or losses associated with Coast Guard member's reasonable use of vendor's facilities or equipment, may be eliminated from further consideration for award." Request you send a copy of any type of waiver that must be signed prior to starting a membership with the gym facility. Federal Acquisition Regulation (FAR) Clauses and Department of Labor Wage Rates for Services are included in this order and are applicable. Vendors doing business with the Coast Guard must be registered or be willing to register with the Systems for Award Management at www.sam.gov.
